Diagnosis that empowers patients

Prostate cancer is not one cancer It ranges from slow-growing tumours that may never threaten a manrsquos life to aggressive tumours that quickly spread beyond the prostate gland Faced with this reality and the many treatment options available to them men often find the diagnostic process confusing and stressful

Our prostate centrersquos diagnostic team will use state-of-the-art technology to determine with unparalleled accuracy whether a man has prostate cancer along with the type and aggressiveness of his cancer Only by knowing such things can men confidently proceed with their care team to make treatment decisions that best suit their individual cases

A fusion biopsy patient reviews imaging scans with his doctor

No Tumour Undetected

Conventional prostate cancer imaging technologies and even standard biopsies can sometimes fail to detect tumours

This causes considerable stress for men with alarmingly high PSA test results Where appropriate our prostate centre will provide men with answers by using state-of-the-art magnetic resonance imaging (MRI) to pinpoint hard-to-find potentially harmful prostate tumours We will also use MRIrsquos superior cancer-imaging capabilities to help determine a tumourrsquos aggressiveness and plan delivery of precision treatments

A Better Biopsy

Our MRI expertise allows us to conduct better biopsies to aid diagnosis

Experts in our prostate centre will perform fusion biopsies which are guided by both MRI and real-time ultrasound Doctors know exactly where the tumour is and can zero in on it with just a few biopsy needles In contrast conventional biopsies involve systematically placing 12 or more needles ndash under the guidance of real-time ultrasound imaging alone ndash into the prostate gland in order to remove small samples of potentially cancerous tissue for testing

Calculating Risk

As much as prostate cancer diagnostic methods have advanced they remain imperfect

There is for instance no fail-safe way to identify aggressive tumours that actually appear to be low-risk To reduce uncertainty and improve outcomes we developed the Sunnybrook Prostate Cancer Risk Calculator It uses data from blood tests family medical history and physical exams to determine a manrsquos risk of having prostate cancer especially aggressive types

Daniel Sternfels

Daniel a 60-year-old hospitality and airline executive has always been an optimist and after triumphing over an aggressive form of prostate cancer in 2013 his appreciation for lifersquos blessings has grown even deeper

ldquoIrsquove gone from macro to micro appreciation On a beautiful spring day I see the birds the buds on the trees and the colour of the flowers I see the beauty of liferdquo

Danielrsquos journey from fear to awakenshying was aided by Sunnybrookrsquos diagnostic capabilities under the direction of Dr Robert Nam head of the Odette Cancer Programrsquos Genitourinary Cancer Care Team MRI pinpointed the location of Danielrsquos cancer and a biopsy revealed it was an aggressive type of prostate cancer likely to grow and spread fast Surgery was planned quickly

ldquoEven though it was a difficult timerdquo says Daniel ldquoI was thankful and very comforted to know that Dr Nam understood exactly where the cancer was and what kind it wasrdquo

Danielrsquos surgery was a success and he is appreciative and grateful for time spent with his wife of 36 years and his two adult daughters ldquoI love life even more than I did beforerdquo he says

ldquoEven though it was a difficult time I was thankful and very comforted to know that Dr Nam understood exactly where the cancer was and what kind it wasrdquo

Precision treatment that saves lives

Our centre will offer the widest range of prostate cancer treatments in Canada many of which will be ultra-precise image-guided therapies that minimize disruption to menrsquos lives

In many cases Sunnybrookrsquos experts have played a lead role in advancing these approaches to managing or treating prostate cancer There will be no better authority than our centrersquos multi-disciplinary team to provide men with a thorough understanding of the options available to them

Patients will have access to powerful and precise external-beam radiation treatments

Active Surveillance

Sometimes the best approach for low-risk prostate cancer is a more hands-off one

Under our active surveillance program men with low-risk disease forego treatment in favour of a vigilant monitoring program ndash using blood testing imaging and biopsies ndash to ensure their cancer remains low-risk Treatment is recommended if there are any signs the cancer has become higher risk A large majority of the programrsquos patients never require treatment making it an excellent option for maintaining quality of life

Radiotherapy

Sunnybrook is a national leader in delivering image-guided prostate cancer radiation treatments

Prostate cancer patients will have access to some of the most innovative forms of radiotherapy including high-dose-rate brachytherapy This fast single-session treatment involves temporarily placing tiny radioactive seeds directly into cancerous tissue to strike tumours with higher and better-targeted radiation doses Patients will also have access to a powerful and precise external-beam radiation treatment called stereotactic body radiotherapy which can cut the number of sessions from 39 to five

Surgery

For many patients surgical removal of the prostate gland is the preferred treatment

Sunnybrookrsquos prostate cancer surgeons are among the countryrsquos best They use precise nerve-sparing surgical techniques such as radical prostatectomy to remove the cancer This approach increases the likelihood that a manrsquos sexual health will be maintained

Stan Rosenberger

ldquoHe was able to save a lot of the nerves in the prostate so I could still be sexually active which is a huge thingrdquo

You could say that Stan an avid motorcyclist left cancer in the dust after he underwent a precise nerve-sparing radical prostatectomy at Sunnybrook at the age of 48

Since then the father of two has gone on long motorcycle rides with his buddies purchased a second ldquohogrdquo motorcycle and completed a five-day hike with his daughter that traversed the rugged Alaska-British Columbia border

Dr Robert Nam performed the delicate nerve-sparing surgery to remove Stanrsquos prostate and surrounding tissues ldquoI was really grateful for Dr Namrsquos approach ndash he did amazing surgeryrdquo Stan says ldquoHe was able to save a lot of the nerves in the prostate so I could still be sexually active which is a huge thingrdquo

Stan is one of countless Sunnybrook patients who are able to live their lives the way they want thanks to our precision treatments

Research that shapes the future

The Soham amp Shaila Ajmera Family Prostate Centre will be home to multi-faceted and groundbreaking research that continuously pushes the limits of diagnostic methods and treatments so that more men can be cured without experiencing life-changing side-effects

Our researchers understand where innovation is needed in prostate cancer care They know because many of them work with physicians meeting with patients on a daily basis At Sunnybrook research is ingrained in clinical care By bringing our team together and enhancing collaboration the centre will spur breakthroughs that save more lives

Sunnybrook s prostate cancer experts are set to transform brachytherapy by leading the adoption of real-time MRI guidance

Advanced Imaging

Though the PSA test has improved early detection of prostate cancer it has its limits

An elevated PSA result doesnrsquot always mean a man has cancer and the test alone cannot distinshyguish between low- and high-risk tumours A better screening and diagnostic tool is needed The centre will support Sunnybrookrsquos leading-edge MRI research which includes testing MRI as a more effective way to screen for prostate cancer as well as to improve the monitoring of men on active surveillance

Genetic Clues

Transforming detection and diagnosis research to uncover the genetic clues behind the disease

Our prostate cancer team is analyzing tumour samples to find all genetic mutations that have a bearing on prostate cancer develshyopment Their aim is to develop a genetics-based test that can determine tumour aggressiveness with unprecedented accuracy potentially rendering the PSA test obsolete This research could also prompt new drug-based therapies to target prostate cancer

Next-Generation Radiotherapy

Our centrersquos researchers will invent the future of prostate cancer radiation treatment by leading the adoption of real-time MRI guidance

They will do so in two first-in-Canada treatment suites within Sunnybrookrsquos Odette Cancer Centre One suite will contain a hybrid MRI scanner and radiation machine capable of using MRI to simultaneously visualize and target tumours with a beam of radiation The second suite for brachyshytherapy will enable doctors to image tumours with MRI and then instantly place the brachytherapy seeds where needed In both suites doctors will also be able to immediately see the effect radiation is having on tumours

Bradley Chapman

ldquoIt took probably an hour to line everything up just minutes for the treatment and then they cleaned me up and sent me home Six days later I was doing my bun runrdquo

An innovative form of high-dose-rate brachytherapy that Bradley received at Sunnybrook as part of a clinical trial meant the 75 year old was able to continue his volunteer work uninterrupted

Bradley scheduled the treatment for a Thursday so he could make his weekly delivery of bread to homeless shelters the following Wednesday ldquoIt took probably an hour to line everything up just minutes for the treatment and then they cleaned me up and sent me home Six days later I was doing my bun runrdquo he says

The fact that Bradley was able to return to the activities of his daily life so quickly is a testament to the targeted nature of his treatment In his case the brachytherapy needles precisely delivered the equivalent of 29 weeks of standard radiation to his tumour ndash in less than 20 minutes

Those questions of intimacy and ability are a genuine concern

A prostate cancer diagnosis comes with questions that can be overwhelming Initially men want to know if theyrsquoll survive Next their thoughts turn to treatment and how side-effects may alter their lives especially their sexual health They worry treatment will leave them with a diminished sex drive or erectile dysfunction Fortunately Sunnybrook leads in developing innovative solutions that preserve as much natural function as possible

Itrsquos not just about saving menrsquos lives wersquore committed to returning them to the best quality of life possible through precise minimally invasive treatments that keep side-effects to a minimum

Sunnybrookrsquos Odette Cancer Program is home to Canadarsquos leading-edge prostate cancer team By enhancing collaboration among these experts and constantly striving to improve the patient experience the Soham amp Shaila Ajmera Family Prostate Centre will be the foremost of its kind in the country

Stan is one of countless Sunnybrook patients who are able to live their lives the way they want thanks to our precision treatments

rsquo
