We are very excited to announce that we’ve signed agreements with Legacy

Health and the OHSU Knight Cancer Institute to join forces in delivering outstanding cancer care to our patients. The OHSU Knight-Legacy Health Cancer Collaborative began in 2013 to build a network of integrated cancer care clinics that improve patient access, outcomes and overall efficiency. TVC’s recent agreement to participate marks the Cancer Collaborative’s first expansion into Southwest Washington.

The OHSU Knight-Legacy Health Cancer Collaborative will benefit from working with our terrific oncologists and experienced, service-oriented oncology and infusion teams. The Vancouver Clinic oncologists and staff are key to the success of the Cancer Collaborative in Clark County.

Adult cancer care services will be provided at Legacy Salmon Creek Medical Center and TVC’s 87th Avenue location. These two centrally-located clinics will be managed and operated by Legacy Salmon Creek Medical Center as an outpatient department of the hospital.

Specific services include:

• infusion hematology and medical oncology clinic and infusion services at TVC’s 87th Avenue location;

• infusion and radiation oncology clinics at Legacy Salmon Creek Medical Center’s main campus and;

• hematology and medical oncology clinic also at the Legacy Salmon Creek Medical Center main campus, opening summer 2017.

DEAR PATIENTS, FAMILIES, AND FRIENDS

The OHSU Knight-Legacy Health Cancer Collaborative’s expansion into Southwest Washington will improve the region’s access to nationally-recognized care and research. We are very excited about this agreement because it will more closely integrate adult cancer care treatment options for our patients and ensure efficient utilization of health care resources throughout the region.

Sincerely,

SHARON CROWELL, MDBOARD CHAIR

Internal Medicine introduces a new approach to patients at Columbia Tech Center and Battle Ground

Reasons patients are choosing Team Medicine:

PATIENT-FOCUSED. We’ve been getting positive feedback from patients. Words used to describe their Team Medicine visit: compassionate, competent, effective, wonderful, professional, top-notch and perfect. One patient commented, “Best visit I’ve ever had!”

TWO, THREE, FOUR TIMES THE BRAIN POWER. Team Medicine is a simple idea – rather than having a single knowledgeable provider available to care for you, there is a team of professionals to call upon. Some of our most requested doctors oversee the work of nurse practitioners and are able to assist as needed on complex medical challenges. So when you come for your visit, you may have one or more of our knowledgeable team members attending to your medical needs.

COMPASSIONATE CARE. Nurse practitioners play an important role on the team to provide the caring, compassionate heart of a nurse and the technical skills to diagnose and treat health conditions. They are licensed to practice independently, and can prescribe medications.

ACCESSIBLE. Team Medicine is structured to respond directly and promptly to telephone calls and MyChart messages. When the clinic is open, team members will be in the office to provide same-day access to available appointments.

Scheduling an appointment with either team can be done by calling 360-882-2778.

Navin Nagaraj, MDBelieves doctor-patient

relationships should be based on open communication, trust, and

empowering patients to take charge of their health.

Nancy Schwarzkopf, NPSeeks to build lasting relationships

with patients by offering a safe, comfortable place to address health

concerns and set wellness goals.

Personalized Medicine

Based on Genetics

The Vancouver Clinic now has access to ActX Genomic Decision Support. The decision

to offer genomic screening is an important step toward our mission to excel at innovative patient-focused, quality-proven care.

With the implementation of ActX, patients will be able to obtain testing and the results will show up in their electronic health record. This will allow The Vancouver Clinic providers to use genomics to personalize medications, screening choices, therapies and manage hereditary risks.

Every time a provider orders medication, ActX will alert the provider in real time about potential adverse reactions, drug efficacy or dosing issues based on the patient’s unique genetic makeup.

Evidence-based decision support will also help providers take proactive steps toward minimizing actionable hereditary conditions. This functionality will allow providers to better understand the potential clinical implications of genomic findings and tailor their patient’s care accordingly.

_________________________________

Please note: As a new service, most insurance companies do not yet cover this test, however, we are offering an introductory price through the end of June for $60 off, making the test just $335 (regularly $395). If you are interested, please discuss at your next clinic visit since this does require a provider to order the test.

Did you know that 87 percent of people have genetic variants that affects how their bodies

respond to medications?
