Return to play protocol after injury All student-athletes must follow this procedure after being declared medically ineligible.• A student-athlete who has been declared medically ineligible to participate by
his/her treating physician due to an injury may not participate in athletics until cleared to participate by the District Medical Director.
• Once a student-athlete submits a note to the school nurse deeming them medically ineligible, the following must occur:• the school nurse will forward the note to the District Medical Director,
Athletic Director, Coach and Athletic Trainer• the student-athlete will be advised by the coach that the student may not
participate until he/she provides a note to the school nurse from a physician, following re-examination, clearing the student to participate.
• The student-athlete must return for a follow up doctor’s appointment regardless if a date of return to participation is indicated on the initial note from the treating physician.
• The student-athlete must submit the subsequent note from a physician clearing the student to participate as of a specified date to the school nurse for review by the Medical Director.
• Upon receipt, the school nurse will forward the clearing note to the Medical Director, Athletic Director, Athletic Trainer, for review and final clearance.
• The Medical Director will review the notes and render a final decision to whether or not the student is cleared to participate. If approved, the Medical Director must certify in writing that the student-athlete is cleared as of a specified date to the school nurse.
• The school nurse will then email the Medical Director’s clearance to the Athletic Director, Athletic Trainer and Coach.
• Once the Medical Director clears the student-athlete to participate, the student may return to play on the date indicated by the Medical Director. The student-athlete will be advised by the Coach that he/she may resume participation.
