TelephoneParents may call the Museum any time during camp at (402) 477-4000.

ClothingChildren are expected to wear shoes that are appropriate for playing at all times. Please note that some camp activities can be messy, so please dress children appropriately. The Museum is not liable for any damage to clothing.

Lost and FoundAll belongings should be clearly marked with your child’s name. Lost items are held at the Welcome Desk for approximately two weeks.

Severe WeatherMuseum sta� are trained in safety and evacuation procedures in the event of a severe weather emergency. If camp is cancelled due to severe weather, a Museum sta� member will notify parents and guardians as soon as possible.

Medical Information/Health PoliciesPlease do not allow your child to participate if they have a fever or other contagious diseases.

If your child becomes ill during camp, you will be noti�ed and requested to pick them up as soon as possible.

Museum sta� cannot administer medication of any kind, unless arrangements are made prior to camp.

The Museum asks for your assistance in identifying any health or behavioral concerns that would prevent your child from participating in certain activities.

Disciplinary PolicyThe safety and enjoyment of our campers is very important to us. Because we believe in setting clear expectations and using common language, our behavior expectations and disciplinary policies re�ect those used in Lincoln Public Schools.

We require all campers to be safe, respectful and responsible. These behaviors are recognized and reinforced through PBIS (Positive Behavior Intervention Support) a process through which positive behavior expectations are de�ned, taught, practiced and reinforced. If behavior expectations are not met by our campers, we follow the BIST philosophy (Behavior Intervention Support Team) through the use of a safe seat and thinking sheets.

If a campers behavior is deemed unsafe they will be removed from camp and no refund will be issued.

Participation Permission Form

Camper’s Name/Campers’ Names:

Your signature at the bottom of this waiver indicates that you give your child permission to participate in all of the activities listed below and release The Lincoln Children’s Museum from liability related to their participation in such activities.

EMERGENCY CARE: I, the undersigned, hereby certify that I am the parent or legal guardian of the camper. I hereby give permission for the staff of the Lincoln Children’s Museum to seek appropriate medical attention for the camper in the event of accident, injury, or illness during the period of the camp.

I will be responsible for any and all costs of medical attention and treatment. I understand that medical attention will be sought from Lincoln, Nebraska health care facilities. In the event that I or others listed are not available, I give my permission to the caregiver to provide

system and arranging for transportation to the nearest emergency medical facility.

I, the undersigned, for ourselves, our heirs, executors and administrators, waive, release and forever discharge the Lincoln Children’s Museum

causes of actions whatsoever arising out of or related to any loss, personal injury, including death, or property damage that may besustained or occur during participation in Camp activities or while at Camp. I acknowledge that said participation in this activity is elected by me and is not required. I voluntarily assume full responsibility for any risk or loss, damage, or personal injury, including death that may be sustained by me or my child as a result of such activity.

FIELD TRIP RELEASE: I, the undersigned, give permission for my child to be escorted away from the Lincoln Children’s Museum

facility to various locations in the Lincoln downtown area as part of the regular Lincoln Children’s Museum Camp activities.

PHOTO RELEASE: I, the undersigned, ____grant permission _____do not grant permission (if not marked, Lincoln Children’s

child’s participation in the Lincoln Children’s Museum Camp Program.
