Dear Students and Parents,

Welcome to Maxson Middle School. We are excited to begin the school year with PBSIS, Positive Behavior Support in Schools. The main goal of Positive Behavior Supports in Schools (PBSIS) is to create a positive learning environment in schools via methods developed using cur- rent research and practices. One of the objectives of PBSIS is to create an encouraging atmosphere at Maxson Middle School through changes in attitude and behavior. The PBSIS Team has loads of events planned for the school year, all created to encourage positive behavior and cooperative attitudes amongst student, teachers and staff.

Our school theme, “Soar to Success,” identifies our school’s expectations. Our Falcon Code of Conduct strives for students to: Be Respectful, Be Responsible, Be Safe, and Aim High. Further, we have defined the meaning of good behavior in ways that exemplify those expectations for all areas of our school.

At the start of the school year we will teach our students the behavioral expectations and throughout the year we will recognize them for living up to our ideals. As we proceed, you will be able to ask your child, “What are the expectation at Maxson Middle School?” and “What happens when a teacher sees you fol- lowing the expectations?” Teachers and staff members will be on the lookout for students following the Falcon Code of Conduct. When they catch a student following the code, they will receive an MMS Falcon Feather coupon. All coupons will be placed in a grade level bin for a chance to receive a variety of weekly and monthly prizes.

By establishing and teaching behavioral expectations and recognizing our children’s appropriate social behavior, we will provide common language for everyone in our building: students, teachers, secretaries, custodial staff, paraprofessionals and parents. We believe that by helping children practice good behavior, we will build a productive school community where all children can learn and grow. Each month we will highlight a different character trait to ensure that our students will develop and adopt the values we endeavor to exemplify ourselves and instill in our children.

Your support with uniform compliance, punctuality in terms of attendance and setting high

academic expectations for your child will make a tremendous difference in our success. We

share a collective responsibility to support and uplift this generation of students spiritually,

emotionally and academically. We have to develop future leaders in order to secure our future. I

know that with y our support, we can and we will. We look forward to our best year ever!!

A rubric, a scoring tool, is used to measure what is important to learn relative to a subject. The purpose

of the student achievement rubric below is to provide a common language for discussing student progress

as it relates to the objective of a0 unit or course.

Exceeds Standards 93-100=A

The student has a complete and detailed understanding of the important information related to the

objective without significant errors and the fluency. The student understands the key features of the

skills or processes necessary to achieve the objective.

Meets Standards 93-92=B

The student has a complete understanding of the important information related to the objective but not

in great detail. The student can perform the skills or processes important to the objective without making

significant errors. The student understands the key features of the skills processes.

Approaching Standards 73-82=C

The student has an incomplete understanding of the topic and/or misconceptions about some of the in-

formation related to the objective. However, the student maintains a basic understanding of the

objective. The student makes some significant error when performing the skills or processes important to

the topic but still accomplishes a rough approximation of the skills or processes.

Attendance

All students are required to report to homeroom every day no later than 8:10 a.m. After 8:15a.m., students

are considered late. If a student is late, he or she must arrive with a note from his or her parent . The note

must be given to the guidance office.

If your child( ren) are going to be absent, you MUST call the attendance office (908) 731-4303

immediately to inform the school of the impending absence. Upon returning to school, the student

Must give note to the guidance office.

Whenever a student is absent, you will receive an automated call noting the students absences.

If a student is absent more than 18 days in a school year, they may not be promoted to the next grade.

On or after the 6th cumulative absence, parent will be required to meet with school social worker . The

intent of this meeting is to apprise parents of the importance of consistent attendance and the

consequences for not having students in school.

On or after the (12) cumulative absence, a referral will be made to the building principal regarding the

possibility of legal action. On or after the eighteenth cumulative absence, the principal will initiate the

filing of court papers and additional actions may be taken.

Dress Code

The Plainfield Public School District has adopted a school uniform policy #5132.1. Consistent with

this policy, listed below you will find the dress code for Maxson Middle School

White, Light Blue, Dark Blue Collared Shirts

The following items are prohibited:

No exceptions!

Headgear—hats, hoods, head bands, bandanas, do-rags, rollers, or any other head covering, are

not to be worn by male or female in the school building, with the exception of a religious or

cultural headdress which must have administrative approval.

Footwear—flip flops, slippers, sport sandals, open toe footwear of any kind

Sunglasses

All students are expected to maintain their personal hygiene including clean and well-groomed

hair.

Cafeteria

The cafeteria offers a well-balanced menu of hot food, salads, desserts, beverages and milk. These

food items are available at low cost to all students.

The New Jersey Department of Education has issued a school lunch policy that states, in part:

Every student is required to file an application for free and reduced price meals under the

National School Lunch Program.

Eligibility will be determined based on an income criteria as established by the US Department

of Agriculture.

Applications will be disbursed within the first few days of school and must be returned during

the month of September.

Cafeteria Rules

1. Students should report to the cafeteria only during their assigned lunch period.

2. Student ID’s must be visibly worn around the neck with the correct color coded lanyard.

3. Students must take a tray for food and must dispose of tray is trash barrels after eating. Nothing is to be left on

the tables.

4. Students are responsible for keeping their areas clean. Items that fall should be picked up and disposed of

properly.

5. All food and beverages must be consumed in the cafeteria. Food can not be taken out of the cafeteria.

6. Students should remain in their seats until they are dismissed buy the administrator in charge.

7. Student are expected to behave appropriately at all times.

8. There will NOT be any food brought or delivered into the cafeteria from outside. (i.e., parents, siblings, etc.).

Students can bring food from home via, brown bag lunch.

9. FOOD FIGHTS WILL NOT BE TOLERATED. Students found to be in violation of the above regulations will

be subject immediate administrative disciplinary action.

All parent and members of the community are always welcome at Maxson Middle School. However, the

following guidelines must be adhered to in order to maintain the safety and security of all students and staff.

Any person who is NOT a Maxson faculty member must enter the building through the main door located on

East 7th Street. (by the marquee). All other entrances are off limits.

Once in the main lobby, a security officer will greet the visitors they will be asked to sign an and will be is-

sued a visitor’s pass. The security officer will then direct visitors to the main office to have appointments con-

firmed or to their designated location.

Maxson Middle School students are the only students permitted on the premises during school hours.

Any person in the building without having signed in and received a visitor’s pass will be considered a tres-

passed and subject to local ordinances regarding trespassing.

Nurse’s Station

Every student MUST have an emergency card with the names and

telephone numbers of those individuals filed in the Nurse’s Office.

(Parents, please know that having accurate information on the

student’s emergency card is crucial for contacting you in case of emergency).

A student who has an accident or becomes ill during the school day should request a nurse’s pass

from the teacher and report to the Nurse’s Office. (At no point in time, should the student use his/

her cell phone to contact a parent to come and pick them up.)

No student will be allowed to leave school for health reasons without permission of a parent or

adult relative.

If a student has been injured at home, the injury is not considered the responsibility of the school and

does not constitute an emergency. Diagnosis or treatment beyond first aid procedure is not the

responsibility of the school.

All students enrolled in the Plainfield Public School must comply with the New Jersey State

Sanitation Code, Chapter 14, “Immunization of Pupils in Schools”.

Medication

All medication that students are required to take during the school day must be dispensed by the

school nurse. Students are required to have a doctor’s note for any medication, prescription or over-

the counter. (Inhalers for asthma are considered medication and will be kept in the Nurse’s Office as

well). Students are not permitted to have with them or use medications during the school day.

School Accident Insurance

School accident insurance forms are distributed to students in September. A student can be insured

against injuries that occur during school hours, while traveling to and from school, and while

participating in school sponsored and supervised activities either away from school premises or after

school hours. This includes all physical education, playground or other school activities except inter-

scholastic athletics, which are insured separately.

Students must report any injury immediately in order to have a claim under the insurance policy.

Each student will be assigned a hall locker and a gymnasium locker. Lockers are supplied for the convenience of

students and protection of their property. The Board of Education and school authorities can assume no

responsibility for the loss of any articles by a student.

For security purposes, your combination that will be issued to you should be kept confidential and should not be

shared with anyone.

Students are urged to be extra careful with personal property. Money or jewelry should never be left in lockers.

Students are permitted to go access their hall lockers (4) times during the school day:

Before reporting to homeroom

At the start of an assigned lunch period

At the end of an assigned lunch period

At the end of the school day

Lockers are the property of the Plainfield Board of Education. School officials have the right to inspect lockers

without notice. Locker inspection may be held at least once each semester and at other times when deemed

necessary. No item or instrument which is unlawful or against school polity is to be housed in lockers.

DO NOT share your locker with anyone. The lockers (hall & gymnasium) assigned to you are for your

individual use only.

Students who are habitually late to class may have their locker privileges removed

NO LOCKER USE AFTER 3:00 P.M.

Any locker violation will result in that locker being “pad-locked” by security.

All students MUST exit building by 3:00 P.M. Except for students under the direct supervision of a teacher,

advisor or coach.

Late students are not permitted to go to their lockers. They are to sign in and report directly to their class.

Reasonable Searches

With reasonable and probable cause established that illegal materials or activities are/or will occur, the princi-

pal and his/her designees may properly and legally search individual students, their lockers and book bags.

Even without an individual suspicion, the principal and his/her designee my still reasonably search the entire

student population when probable cause has been established that illegal or dangerous activity is imminent.

Student ID Cards.

Students must have student ID cards at all times. All ID cards should be visible for inspection upon entering the

building and throughout the day. The ID must be placed around the students’ neck, using the appropriate school

supplied lanyard.

ID cards must be worn for all sports, special programs and other events held at Maxson Middle School after the end

of the regular school day.

There is a $5.00 replacement fee for lost or stolen ID’s. $1.00 for replacement lanyards.

Damaged, Lost Instructional Material

Fines shall be charged to students who, whether through misuse or deliberate actions, damage or lose school prop-

erty. Under the law, a student who defaces or otherwise injures school furnishings or other property may be disci-

plined and student parent/guardian will be fined the amount of the damage.

When a student loses or excessively damages a textbook which is the property of the school district,

the school shall fine the student amount appropriate to the damage

Band and other instruments must be returned in appropriate condition or fines shall be levied for

damages.

Cell Phones -Electronic Devices

Cell phones are prohibited during the school day. Student must turn their phones off and put them in their locker

upon arrival. School phones are available for use in case of an emergency. Confiscated phones, Ipods etc. will only

be returned to parents. Maxson Middle School is not responsible for lost or stolen cell phones, or other electronic

devices.

Fire – Crisis Drill

Instructions

In case of an emergency,, the following procedures must be followed by students and staff:

Students should walk directly to designated exists in a quiet and orderly manner.

Do not use elevators during fire emergencies

There should be no talking during fire drill in order to hear directions that may be given via

intercom

Each classroom will line up at a designated meeting location a safe distance from the building

with their teacher

Everyone must vacate the building during fire drills/emergencies

Students not in class must proceed outside by the nearest exit and report to the nearest teacher.

At the administrations’ direction, students and staff may return to the building

Students

Responsibilities

Although student achievement is a primary factor on which grades are

based, it is important to provide continuous feedback to students on other

variables which support learning such as the self management of learn-

ing, responsible behavior, and attendance. It is also critical that students

understand their role in providing the effort necessary for learning.

Self Management of Learning

In order to be responsible for the quality of their thinking and learning, students must develop and

regularly use a toolbox of strategies for managing their learning. These self management strategies

include being aware of when one doesn't understand new information and taking the appropriate

steps to remedy the situation. Additionally, students are expected to seek understanding by asking

questions and formulating hypotheses. Students manage their won learning by evaluating the

feedback they get from others; making connections to their background knowledge; and evaluating

their progress toward a learning goal. Consistent use of these strategies supports students in

reaching or exceeding the standards.

Teachers should consider self-management of learning and other variables when reporting on

student progress which reflect not just the learning product, but the strategies students use to be

successful learners (Resnick2003). Students must make a conscientious effort to develop and

display self-management and responsible behavior.

Self-Management factors include:

Making connections

Evaluation Progress and setting learning goals

Keeping informed concerning assignments

Completing assignments in a timely manner

Going above and beyond minimum competencies

Coming to school ready to learn with the necessary assignment’s and materials

Becoming a proficient reader, problem solver and critical thinker

Attending classes daily unless ill or legally excused

Delivering home school notifications to their parents/guardians

The student uses a variety of strategies which are supportive of learning

prompting

The student works toward the attainment of group goals without being asked

Parents Responsibility

It is a joint responsibility of the school and home to develop and maintain an effective system of

communication which encourages parental involvement in student achievement and in academic

district concerns as well. Specific parents/guardians responsibility include:

Send students to school physically, emotionally and socially prepared to learn;

Make sure students are in uniforms at all times

Teachers Responsibility

A highly qualified teacher knows what to teach, how to teach and has a

full understanding of the subject matter being taught. Without a doubt,

assessment is also a critical component of instruction. Therefore, a highly

qualified teacher must know how to develop, administer and interpret

the results of formative and summative assessments and to report students’ progress. As the person most directly responsible for providing instruction, it is the teacher’s responsibility to:

Implement the district curricula with integrity using teaching strategies that are effective

for all students;

Implementing grading procedures following the grading guidelines;

Maintain a grade book (electronically / hard copy) with appropriate minimum number

(10) of grades per marking period;

Enter grades electronically in a timely manner adhering to the due dates established by

the district;

Make instructional decisions based on an analysis of data from multiple types of assessments;

Communicate with families in a timely manner regarding student progress;

Begin the appropriate intervention steps as soon as possible and maintain documentation;

POSITIVE BEHAVIOR SUPPORT IN SCHOOLS School-Wide Positive Behavior Support is not just another initiative. It is a lifelong change in how schools plan for and deal with discipline and problem behaviors. Over a thousand schools across the country have been successfully using School-Wide Positive Behavior Support to reduce occurrences of discipline and problem behavior. Soar to Success Falcon Code of Conduct

Be Respectful, Be Responsible, Be Safe, Aim High Everyone (students and staff) at Maxson Middle School is expected to Be Respectful, Be

Responsible, Be Safe, and Aim High. Every September and periodically throughout the year, students and staff will participate

in activities to learn and review what the Falcon Code looks like. What Be Respectful, Be Responsible, Be Safe, Aim High means and how we should

behave will vary across different settings. If you need a reminder, just look around for a Falcon Code poster that will tell you exactly

what is expected of you in that setting. Teachers and Staff are always on the lookout for students following the Falcon Code.

When they catch you following the Code, you may receive a Falcon Feather that looks like this:

Teachers and staff will hand out Falcon Feathers many times each day to students who are demonstrating the Falcon Code. You’ll never know when a Falcon Feather is coming so you should always remember to be Respectful, Safe, Successful and Aim High

When you get a Falcon Feather put it in your grade level’s bin located in your homeroom.

Each week, five names will be drawn from the grade level bins and the names of the

winners will be announced during homeroom. If your name is drawn you will get to choose one of the prizes offered that week. Prizes will vary each week and will include things such as: free admission to a dance, fast passes to the front of lunch lines, gift certificates, and many other interesting items.

REMEMBER: We are looking for POSITIVE BEHAVIORS.

Maxson Middle School “Soar to Success” - Recognition System

Use of Falcon Feather Ticket:

1. Staff will recognize students for meeting school-wide expectations and for exemplifying positive behaviors in the school setting

a. Staff member gives a ticket (signed legibly) with verbal recognition of the behavior observed. i.e., “Molly, thank you for being prepared today,” “Jimmy, it is good to see that you have your homework.”

b. Verbal recognition is also a positive reinforcer. Remember the 3:1 rule – 3 positive statements for every negative or critical statement.

2. Students will be responsible for signing their slips and placing them in appropriate grade level container in their homeroom class.

3. Reward tickets placed into the bins will be used for weekly, monthly, and quarterly drawings. The winners will be announced over the PA system.

4. When students earn rewards, the staff member(s) that recognize students earn rewards and become eligible for our monthly/quarterly drawings

“Soaring Staff” Days 1. Selected days will be announced that students will be given opportunity to acknowledge

staff members who “Soar to Success.” 2. All students will receive “Soaring Staff” certificates on designated days throughout the

year. The student will write a statement to a teacher/staff person of their choice related to the four expectations (Be Respectful, Be Responsible, Be Safe, and Aim High)

3. Staff will then receive rewards for exemplifying positive behaviors.
