ClassiFiCation: I. Open fracture – The new classification that the

committeehasdevelopedisabouttobepub-lishedinJournal of Orthopaedic Traumainoneofthenextcoupleofissues.Wehaveworkedonthewordinganddiscrepanciesbetweenfacultyand resident use of the classification. The classi-fication is based on assessment of five categories: 1.skin;2.muscle;3.arterial;4.contamination;and 5. bone loss. The classification is driven bytheanatomicandpathologicfeaturesoftheinjuryandnotcharacteristicsoftreatmentdeci-sions.Weareworkingonvideostoformthebasisofanobserverreliabilityproject.Ifanyonewouldliketocontributevideopleasecontactmeorothercommitteemembers.Wearelearn-ingthatitishardertogettheseitemsthanyouwould think. III. The name of the classification–Thishasbeena topicofdiscussionthathasevenreachedthe boardlevel.Itistoolongastoryforthenewsletter and generates lots of strong opinions. Suffice it tosaythatthecommitteethinksthattheuseof the alpha numeric classification should be promotedastheinternationallanguageoffracture classification to improve future research and patientcare.

IV. Work with AO-CTF–Wecontinuetowork closely with AO-CTF on classification projects such as the scapula classification.

Database: UseoftheOTATraumaRegistrydatabasecontinuestoincrease.Weareworkingontransferringthenewimagesandcodesfromthe2007compendiumpublicationtothedatabase.Thisprojectisunderway.Pleaselookatthedatabaseandconsiderwhetherthismember benefit might help your research or education efforts.

OutComes: InourMFAoutcomeprojectwehavenearly50patientswithboth43’sand11’senrolled.Weareworkingon6monthand9monthfollow-ups.OurgoalistoprovidestandardMFAnormsatthesetimepointsforthevariousfractures.

TheMilitaryCommitteepromotescollaborationbetweencivilianandmilitaryorthopaedictraumasurgeonsintheareasofpatientcareandresearch.Januaryofthisyearmarkedthe5thyearoftheExtremityWarInjuries(EWI)symposium,heldannuallyinWashington,DCwhichisdesignedtopromoteresearchcollaborationbetweencivilianandmilitaryorthopaedictraumasurgeons.Thesymposiumisco-brandedbetweentheOTA,SOMOSandORS.Thesymposiumtitled“BarrierstoReturnofFunctionandDuty,”wasChairedbyCAPT(R) Michael BosseandCOL James Ficke,whichhadalivelypoint–counterdiscussionontheErtlamputation technique. Several of the most difficult problemsinorthopaedictraumawerediscussedwithregardtocurrentknowledge,areasofcurrentresearchaswellasprioritiesfornewresearch.AnewsessionwasaddedthisyearwhichfocusedonDisasterPreparedness.ThissessionwasmoderatedbyRADMThomasCullison,ViceSurgeonGeneraloftheNavyandDr. Christopher Born.ThehighlightofthissessionwasateleconferencewithCOL (R) Roman HaydawhowasvolunteeringhistimeandskillsontheUSNSComfortassistingwiththeHaitianearthquakevictims.ElevenOTAmembersvolunteeredtheirservicesontheComfortinassistanceofthedisaster.AnotherareaofcollaborationbetweencivilianandmilitaryorthopaedictraumasurgeonsistheMajorExtremityTraumaResearchConsortium(METRC)thisconsortiumfundedbytheDepartmentofDefensewillbelookingatsomeofthemostchallengingproblemsinorthopaedictraumaforthemilitary.Many of these problems do not have significant volumesforthemilitarytoanswerquestionsalone,andaconsortiumofcivilianandmilitarytraumacenterswillbepoolingresourcesandpatientstoattempttoanswerwhattypesofbonegraftingand fixation methods are best for severe fractures. Severalotherstudieslookingatchallengingproblemsareplannedaswell.AnotheravenueofresearchfundingformilitaryrelatedinjuriesistheAirliftResearchFoundation.OneofthegrantrecipientsannouncedattheAAOSannualmeetingwasDr.ChristopherBornforaprojectdesignedtopreventinfections.OTAmemberscontinuetoparticipateintheDistinguishedVisitingScholarProgram(DVSP)atLandstuhlRegionalMedicalCenterinGermany.Increasedcasualtyratesarebeingseenwiththesurge

Thelastfellowshipcolumnfromme:itfeelsgood.Thisisjustonemorereminderthattheformaltrainingiscomingtoanend.NotthatIamcountingthedaysmarkingthroughboxesonthecalendaroranything,butultimatelythisiswhateveryonehasbeenworkingtoward.We’vebeenwaitingfortheopportunitytostruggleallonourown.ButinallhonestyIthinkthatwearemorethanreadyforthetransition. Havingspentsometimewithmanyofthecur-renttraumafellowsfromacrossthecountryIhavequicklysurmisedthatawelltrainedgroupofyoungsurgeonsareabouttodescendontheworld.Wearereadyinverydifferentways,butreadynonetheless.It’sfunnybecauseIalwaysassumedthatonceyoucompletedafellowshipyouwouldmagicallypossessthesameskillsetaseveryothergraduatingfellowacrossthecountry.Thatisnotthecase. Justlikeresidencyprograms,thevariousfellow-ships have definite strengths and weaknesses, and our individual approaches to problem solving reflect our differencesintraining.Giventhesamepatientwemaycometoasolutionfromseveraldifferentdirec-tions.Alltheseapproaches,ifappliedcorrectly,canleadtosuccess.However,youcannotcountonthesamestrategytoworktimeandagainwithoutfail.That’swhentheexperienceandtechniquesofyourpeersandtheirteacherswillbailyouout.Thereisalevelofdiversityandingenuitythatexistsintraumathat makes it the most exciting field of orthopaedics (nobiasatall). Therealtrickis,nowthatyouwillbeoutintheworld,howtomaintaintheconnectiontothetraumacommunity.Ifyouarejoiningalargeacademicprac-ticeyougetsomeofthecamaraderiebuiltin.ButyouarestilllockedintotheSeattle,Baltimore,Tampa,orwherever-you-arewayoflifesotospeak.IfyouhaveelectedtogointoprivatepracticeIgatherthatyourpartnerswilllargelyignoreyourendeavorssolongasitdoesn’taffecttheirelectiveschedule.There’saneasyanswer:Doexactlywhatyou’vebeendoingforthelast6years. Therehasbeenmuchrecentscrutinyontherelationshipbetweenindustryandorthopaedicsurgeons.Aprincipledassociationisnecessaryfortheadvancementofthespecialtyandtoenhancepatientcare.Whiletheattentionhasbeenfocusedonsurgeonswhohavecompletedtheirtraining,therearealsoissuesthatorthopaedicresidentsandfellowsmustbeawareofwhendealingwiththeirindustrycolleagues.ResidentsandfellowsareheldtothesameAAOSStandardsofProfessionalism(SOPs)onOrthopaedist-Industry Conflicts that all orthopaedic surgeonsareexpectedtoobserve.TheseSOPswereupdatedin2007andwentintoeffectin2008,andareavailableforreviewontheAAOSwebsite.Followingaresomeofthemorecommonissuesthatarisewithresidentsandfellows:

Washington,DC.ThiswastheUSBJD’smajorundertaking in 2009, and represented the first time inhistorysuchabroad-basedassemblageofmusculoskeletalorganizationsmetintheUnitedStates. WithaHostCommitteeofmorethan100Senators,CongressionalLeaders,andAmbassadors,theconferencebroughttogethermusculoskeletalhealthcareprofessionalsandpatientsfrom56countriesandtheirBJDNationalActionNetworks,aswellasmorethan40USBJDparticipatingorganizations,manyrepresentedbytheirleadership.Intotal329delegatesattended. This high profile event provided a special opportunitytoshowcasemusculoskeletalhealth. OTAMemberswhowerekeypresenterstotheGlobalBoneandJointDecadeEventincluded:OTAPastPresident,Bruce Browner, MD,whoprovidedanoverviewoftheburdenoftraumaticconditionswithafocusonthecontinuedneedforfocusonpreventionof death from road traffic accidents. OTA President-Elect,Andy Pollak, MD,chairedthesymposiumdiscussiononTraumaandInjury.HewasassistedbyOTAActiveMember,Eric Farrell, MD.Theoverridingpriorities of the group were identified as road traffic crashes,andincreasedresearchintothepreventionandtreatmentoffragilityfracturesandimprovingoutcomesinpatientcare.OTAActiveMember,James Waddell, MDofBoneandJointCanadawasrecognizedforachievementsbyitsnetworkinraisingawarenessandimprovedaccesstocarethrougheffectivemusculoskeletalmanagementprograms. Thetwoprimarygoalsoftheconferenceweretoraiseawarenessandadvocateformusculoskeletalhealth,andtodeveloparoadmapforthefuturetobuildonthemomentumcreatedbytheBoneandJointDecade. Nearly200patients,patientadvocates,andhealthcareprofessionalsgatheredforAdvocacyDayonCapitolHill,Thursday,October22.CongressmenCharlieMelancon(D-LA)andDaveCamp(R-MI)addresseddelegatesduringthemorning,aswellasFormerAssistantSurgeonGeneraloftheUnitedStates,RearAdmiralSusanBlumenthal,MD.Delegatesthenparticipatedinatrainingsessionandreviewofthe

OnFridayandSaturday,leadingexpertsaddressedtheburdenofdisease,advancesinresearch,preventionandcaremanagementdevelopmentsoverthelasttenyearsandopportunitiesforthefuture,covering osteoarthritis, inflammatory conditions, backpain,bonehealthandosteoporosis,andtrauma.Therewerealsosessionson:Howadvancesintreatingmilitarycasualtiesareleadingtoopportunitiesforcivilianapplication;rehabilitationofvictimsoftheSechuanearthquakeinChinalastyear;howthePonsetiInternationalAssociationhasusedtheDecadetoexpandawarenessandthetrainingofthePonsetimethodfortreatingclubfootaroundtheworld;howtoadvancethehealthpolicyagendaformusculoskeletalconditions.ParticipantsintheUSBJD’sYoungInvestigatorInitiativeprogramdisplayedpostersoftheirwork. Thesesessionsprovidedthefoundationfortherestoftheconference,strategicplanningforbeyondtheDecade.SixmultidisciplinarygroupsofexpertsfromtheU.S.andaroundtheworldhadspentthemonthsleadinguptotheconferenceidentifyingtheissuesandprioritiesaffectingloweringtheburdenofmusculoskeletaldiseaseanddevelopingplanstoaddresseachpriority.Theseplanswerediscussedbydelegatesduringtheconferencewhowerechargedwithreviewingandprioritizingtheissues,andaretobepublished.Thepre-conferencedocumentspreparedbythegroupsareincludedintheconferenceprogram,startingonpage49,availableatwww.usbjd.org/rd/?BJD09.
