2015 Community Wellness Report
This report is intended to be used by community-based Primary
Health Care Provider Teams, Clinic Teams, working groups, KTHR
staff and community leaders to inform and to develop strategies to
address unmet needs and emerging issues in the community of
Tisdale.
Health and wellness decisions are not simply a matter of personal
choice. What makes us and keeps us healthy is complex and
multifaceted. Determinants of health such as income, education,
environment, culture, coping skills, housing and lifestyle options all
influence a person’s ability to have access and to make the
healthiest choices.
The root cause of many illnesses in our communities stems from
poverty. For those with low incomes, poverty means more than just
living in a poor home, or having no access to affordable
transportation. There are a variety of detrimental impacts stemming
from being poor. One of the most devastating is the impact on
health and well-being that, increasingly, professionals across sectors
are beginning to identify. Looking at health more holistically has
focused attention on to the social determinants of health.
2 2015 Tisdale Community Wellness Assessment Survey Report
November 27, 2015
Many of our KTHR staff collaborate and partner with agencies and
other sectors to develop illness prevention and health promotion
strategies and work to improve the social determinants of health.
For example: Partners to End Poverty; Food For All Coalition; 0-3
Early Childhood Task Force; Northeast Housing Research Project;
Northeast Regional Intersectoral Committee; Cost of Healthy Eating
in Saskatchewan; Tisdale Suicide Awareness Committee; and
Population Health Working Groups: Eat Well, Play Well and
Tobacco Reduction.
In Primary Health Care we are committed to a health care system
that puts patients first by providing Better Health, Better Care,
Better Values, and Better Teams for our residents. We have clinic
teams that work on meeting the needs of those with illnesses or
urgent care needs. Ensuring all our residents have access to a main
care provider has become a regional priority. This helps to ensure
patients have continuity of care. Research tells us that people who
have a family doctor or nurse practitioner have better health
outcomes. Better health outcomes mean better quality of life.
Clinic team strategies include:
Impacting access to continuous and comprehensive care
Educating the public on the importance of attaching to a
family Dr/NP
Improving timely access to a family Dr/NP
Improving patient experience
Establishing and managing patient panels or roster. (A panel
is a list of patients that have an established relationship
with a Dr/NP. Once Dr/NP has a list of his/her patients,
they can better manage their care. For example, with
screenings (cancer, blood pressure, diabetes, etc.)
Promote Live Well with Chronic Conditions - a six week
self-management program for people who have an ongoing
or chronic medical condition. The program helps people
with chronic conditions build skills for managing their
health, so that they can maintain active and fulfilling lives.
The program offers information and teaches practical skills
to better manage symptoms and become an active partner
in their care. Participants gain confidence and motivation. In
Tisdale the goal was to offer 2 sessions per year. In both
2013/2014 and 2014/2015 only one session each year was
offered due to low enrolment numbers.
We also have Primary Health Care Provider Teams throughout the
region which brings health professionals and community
representatives together to work on unmet wellness needs in the
communities. Each team has conducted community wellness
assessments every two years since 2007 which allows the
community to tell us what their health and wellness concerns are.
The 2015 survey results will be discussed by the team and strategies
to address the various needs will be developed. Those needs that
are beyond the control of the team will be directed to the
appropriate agency or group.
Our community representatives are our voice from the communities
and play an integral role when it comes to developing strategies to
address needs such as: dementia workshops, free physical activity
events, and P.A.R.T.Y. Program (Prevent Alcohol and Risk-Related
Trauma in Youth).
3 2015 Tisdale Community Wellness Assessment Survey Report

Methodology:
The 2015 Community Wellness Assessment Survey was offered to
Tisdale residents as follows:
On-line through a link on the KTHR Public website from May
7th – June 12th
In person at Tisdale Co-op May 27th – Primary Health Care
Provider Team members were on-site to assist community
residents to complete the survey
Each team member was given 10 paper copies of the survey

Why does it take so long for a psychiatrist referral? From the time of referral to the time of seeing the specialist, things change. Then insurance plans aren't willing to give money. This is extremely frustrating and causes more stress.

Couldn't get an office appointment with sick child as no appointments were available with regular doctor. Phones at 10 and went to ER at 2. Told Dr not in until 5. Tried over the counter remedy - didn't work so days later back to ER in an evening. With sick children could they not be prioritized at clinic so ER is not required?
I have phoned for an appointment with my main health care provider and I had to wait a month, that’s too long. Need to have two or three appointments every second day set aside for people who have an emergency and I mean with their doctor. And when an 81/2 month pregnant woman needs a two week appointment why give her an appointment for a month later.

Use taxi/family sometimes no $
Getting in for an appointment when required
have to take time off work
I have not found a doctor I feel comfortable seeing. When anyone refers me to a doctor they are not accepting new patients
Some programs are only available during work day hours when I am at work
No answering machine at the clinic for me to leave a message
Lack of doctors taking new patients
I visit my husband in senior care
Confusing question...
Difficulty in arranging an appointment. Reception staff need to be trained to ask questions so that I, as patient or client access the service we require, i.e. a ‘complete' rather than wasting the Drs time and mine
Have to drive to Melfort
Not being able to change Dr or to see a Dr in another community
Waiting for Dr. appointments and can't get appointment with the Dr I want
Not much choice for healthcare providers as many not accepting
new patients...can't go to other communities to choose a provider due to restrictions!
The warfarin dosing clinic in Melfort was stopped. It was run by the
pharmacists and they did a wonderful job. This is a huge loss for a
lot of people.

Try to eat more fruit/vegetables Stay away from bad friends
Access to female doctor or NP for women. A workshop for spouse of person with diabetes for the person with diabetes
Participation community activities...Promotion of health and wellness... Like activities that focus on "positives" and not on the negative
More accessibility around men's health
Better access to health care providers. Less wait time, appointments, etc.
We must stop farming with chemicals
I am a KTHR employee therefore workplace wellness, women's health and inactivity could be workplace initiatives
Easier to see a doctor
To be allowed access to gender of health care provider that I feel most comfortable seeing - if not available in Tisdale I should not be denied access in Nipawin or Melfort or wherever
Current method to get a shingle vaccine requires 2 trips to Dr/NP and 2 to pharmacy that is lost time from work and inefficient use of Dr. I would like the travel nurse or public health nurse to deliver the shingles vaccine. this would be one appointment for the patient. I do not mind paying the $200 for the vaccine itself but the system is inefficient
Classes that can be taken in my own community for support groups in town for community members to help one another i.e. Al-anon, Al-a-teen
It would be nice to have a program for new parents regarding healthy lifestyles for young children
More women Dr.'s Language barrier sometimes with the Dr's
Advertise more on where or when to get help
Ones who live in city have problems getting coordinated care
Use social events like come and go sessions to share information and make it interesting so people catch the feeling to need to attend. Make it sound important but not so to scare people-use positive messages
Too many frequent flyers people taking time and costs - they cost the system to much
Anything to get the kids off their electronic devices
Having a family doctor who I can clearly understand
Easier access to specialists
Be more active, eat healthier
More awareness
Available resources for increasing activity in adults We take steps to address the above in elderly
We are doing very good
Education sessions
Able to see the doctor asked to see
Guide to GOOD on line information
More accountability from those in administration for what actually takes place in the overall health "system"...is a concern for us. In particular, the relationship between the health care providers, Home Care and, those who require "home care" is, in my opinion, something that needs to be addressed. It is obvious we do not have, and, perhaps, may never have enough "Assisted Living" for seniors and others who will require care at home. So, we need to talk about what we see around us now and what Administration thinks is happening. If, and, when we require care at home, we would like to know the caregivers are trained, are compassionate and have time allotted to do the job. And, simply put, Supervisors need to supervise.
access to doctor
Doing family activities (biking, hiking, skiing) Pack more snacks/lunch when on the run Everyone making a conscious effort to decrease computer time Chronic condition isn't much of an overwhelming issue, but the ones had could flare up and become issues when out of control

Being able to see a Dr. when something unexpected comes up- not being told "sorry can't see anyone"

Having a medi-clinic in town.
Any activity which promotes wellness and provide support in caring way - binding together for everyone in community- cross age groups, gender and socioeconomic levels. Also good to promote age appropriate activities but could involve others
Community involvement with education and support
Support groups for people affected by cancer and mental health
receive telephone call re results of A1C instead of another appointment to see Diabetic Educator
Staffing issues in long term care-very poor lack of primary physicians
Eat good food, balance of good food and exercise.

20. Responses:
Talk to doctors/,nurses, support people in open honest manner
More education - public Initiatives
Mental health support for person with a chronic illness and alcohol abuse issues
Get our community to be proactive for "health and wellness"
Offer mental wellness info
Education and a healthy community mindset
KTHR needs to place more emphasis and resources in prevention and early intervention especially in the area of early childhood and poverty reduction so that we may reach our vision of healthy people in healthy communities.
More AA meetings
Increase level of privacy. How many people know about people's health situations with info that comes from employees?
Posters for healthy lifestyles and the benefits of living well
Have more specific Dr's or person to help with the different concerns
Community open houses and social events - use fun things rather than focusing on the negative aspects, to get the issue using positive approaches
I can't figure out why there isn't more doctors who stay here so a person can actually get an appointment
Bring back the Chelation treatment to our hospital
More regulations on food preparations GMP course on antibiotics
Making parents more accountable for children's health and wellness choices/conditions
More programs for all members of community
Balance work, family obligations, etc
Inform the public using every type of media. Focus on groups that may feel intimidated and are not willing to participate
More small group meetings...i.e. in community spaces. Example....my concern re-Home Care comes from years of observing how it works and how it has changed. It is a perception in our community that families have little or no say. Perhaps, small group meetings with those facing life changes, and, health professionals could help design a better way of caring for those who will need care in their home and beyond.
Better screening of elderly and providing required care
Extracurricular programs at school
More exercise places with extended hours
More activity programs for adults
Should be more aware for domestic abuse . No resources
Promote wellness and positive self-esteem through activities which involve all or others. Working together as a community. Valuing each other. Going and promoting roles of these who may not feel capable. Enable people at all levels to create a positive and healthy lifestyle
Support groups Need to ensure people are aware of them
support group for those with mental health issues, headed by MH professionals
My family and I are very concerned about the staffing issues at New market Place--absent staff not replaced, a regular consistent staff in each facility which has been recommended by the advocate to make residents feel more comfortable, and very poor organization when serving meals.

No private care facilities or assisted living. Elders may not be eligible for either subsidized senior housing or a place like Caleb
Not if there isn't enough doctors
Not old enough to need services
But needs stable Primary care health people
Needs to be assisted care home
Not sure what care will be available if we need more care
Rural people are treated as less important than urban people. As I
grow older and have more health concerns, I fear the healthcare
here will not be enough.
24. If no, please explain:
Need to be vigilant i.e. cell phone drivers, speeders, lack of activities for young people - all effect overall society
Police need to be seen more
I used to think so but now I've heard of a lot of murders, rapes and suicides
It's changing. There are very few inexpensive organized activities for children to participate in
Safe for me, but definitely not safe for all due to circumstances in which they live
++++drunk drivers
I always thought it was, but there seems to be more and more
violence here

25. If no, please explain:
We mainly walk
Sidewalks and streets are poor especially for seniors - street sweeper lots of stones/rocks
Ruined streets and sidewalks
Now hospital has moved not so easy to get to for seniors
For healthy persons, but location of medical facilities is not convenient to access, then go to pharmacy
Poor sidewalks have to walk on the road
Many incomplete sidewalks
But would be nice if we had more sidewalks
Poor sidewalks
Distance from downtown to Dr.'s office and hospital
Many services that are essential are away from the core
Lack of safe sidewalks and streets in some parts of the town
Lack of safe sidewalks, inattentive drivers and speed esp. around health-plex and hospital
Due to knee injury, I can't walk any distances.
Country
Sidewalks and better plowed walk ways in winter
Sidewalks are often nonexistent or unsafe
No sidewalks
Most things are downtown and within walking distance, except healthcare now
