2015 Community Wellness Report

This report is intended to be used by community-based Primary

Health Care Provider Teams, Clinic Teams, working groups, KTHR

staff and community leaders to inform and to develop strategies to

address unmet needs and emerging issues in the community of

Tisdale.

Health and wellness decisions are not simply a matter of personal

choice. What makes us and keeps us healthy is complex and

multifaceted. Determinants of health such as income, education,

environment, culture, coping skills, housing and lifestyle options all

influence a person’s ability to have access and to make the

healthiest choices.

The root cause of many illnesses in our communities stems from

poverty. For those with low incomes, poverty means more than just

living in a poor home, or having no access to affordable

transportation. There are a variety of detrimental impacts stemming

from being poor. One of the most devastating is the impact on

health and well-being that, increasingly, professionals across sectors

are beginning to identify. Looking at health more holistically has

focused attention on to the social determinants of health.

Many of our KTHR staff collaborate and partner with agencies and

other sectors to develop illness prevention and health promotion

strategies and work to improve the social determinants of health.

For example: Partners to End Poverty; Food For All Coalition; 0-3

Early Childhood Task Force; Northeast Housing Research Project;

Northeast Regional Intersectoral Committee; Cost of Healthy Eating

in Saskatchewan; Tisdale Suicide Awareness Committee; and

Population Health Working Groups: Eat Well, Play Well and

Tobacco Reduction.

In Primary Health Care we are committed to a health care system

that puts patients first by providing Better Health, Better Care,

Better Values, and Better Teams for our residents. We have clinic

teams that work on meeting the needs of those with illnesses or

urgent care needs. Ensuring all our residents have access to a main

care provider has become a regional priority. This helps to ensure

patients have continuity of care. Research tells us that people who

have a family doctor or nurse practitioner have better health

outcomes. Better health outcomes mean better quality of life.

Clinic team strategies include:

Impacting access to continuous and comprehensive care

Educating the public on the importance of attaching to a

family Dr/NP

Improving timely access to a family Dr/NP

Improving patient experience

Establishing and managing patient panels or roster. (A panel

is a list of patients that have an established relationship

with a Dr/NP. Once Dr/NP has a list of his/her patients,

they can better manage their care. For example, with

screenings (cancer, blood pressure, diabetes, etc.)

Promote Live Well with Chronic Conditions - a six week

self-management program for people who have an ongoing

or chronic medical condition. The program helps people

with chronic conditions build skills for managing their

health, so that they can maintain active and fulfilling lives.

The program offers information and teaches practical skills

to better manage symptoms and become an active partner

in their care. Participants gain confidence and motivation. In

Tisdale the goal was to offer 2 sessions per year. In both

2013/2014 and 2014/2015 only one session each year was

offered due to low enrolment numbers.

We also have Primary Health Care Provider Teams throughout the

region which brings health professionals and community

representatives together to work on unmet wellness needs in the

communities. Each team has conducted community wellness

assessments every two years since 2007 which allows the

community to tell us what their health and wellness concerns are.

The 2015 survey results will be discussed by the team and strategies

to address the various needs will be developed. Those needs that

are beyond the control of the team will be directed to the

appropriate agency or group.

Our community representatives are our voice from the communities

and play an integral role when it comes to developing strategies to

address needs such as: dementia workshops, free physical activity

events, and P.A.R.T.Y. Program (Prevent Alcohol and Risk-Related

Trauma in Youth).

Methodology:

The 2015 Community Wellness Assessment Survey was offered to

Tisdale residents as follows:

On-line through a link on the KTHR Public website from May

7th – June 12th

In person at Tisdale Co-op May 27th – Primary Health Care

Provider Team members were on-site to assist community

residents to complete the survey

Each team member was given 10 paper copies of the survey

to have clients/families and friends complete

129 surveys were completed: 93 paper copies and 36

surveys were completed on line. In comparison with the

survey done in 2013, 128 surveys were completed, 99 paper

copies and 29 surveys were completed on line.

Next Steps: The Tisdale Primary Health Care Provider Team will

be reviewing the survey results and developing strategies to meet

some of the unmet needs. The Team will also bring the report

forward to community leaders such as Mayors, Town Councils and

Interagency Committee. The final report will be posted on the KTHR

Intranet and Internet. Once all the Community Wellness

Assessments have been completed a Cumulative Report of the

findings will be developed and will be made available on the KTHR

Intranet and Internet.

What has our community-based Primary Health Care Provider Team been up to since our last assessment in 2013? Partnered with community groups to offer a Blood Donor

Clinic twice in 2013 and once 2014. Community groups have

since taken the lead on organizing regular blood donor

clinics 2-3x/year

In 2013 Suicide Awareness Working Group begins to meet

regularly (a result of health providers and community

members brainstorming session held in 2012). Since 2014,

the Suicide Awareness Working Group has become a

community driven initiative with health care professionals

participating

Team members participate in the Suicide Awareness Day

event 2014, 2014, 2015

Team continues to lobby Town/Chamber of Commerce to

increase free Wi-Fi access sites within the community. Need

identified at local Poverty Free Café meetings in 2013

Offered ATV Safety Clinic in 2013, 2014 and 2015

Offered a Pole Walking Clinic in 2013

Partnered with community groups to sponsor Bike Rodeo

2013, 2014, 2015. In 2014 and 2015 Bike Rodeo was

combined with Community Bike Ride

Partnered with community agencies, Tisdale Hospital and

T.M.S.S. High School to offer the P.A.R.T.Y. Program –

(Preventing Alcohol and Risk-related Trauma in Youth) in

2013, 2014 and 2015

Co-sponsored series of Yoga Classes in TMSS for grades 9-12

students

In 2014 team members offered ‘Pack for a Pack’ at Tisdale

Trade Fair. Patrons could exchange a package of cigarettes

for a package of nicotine gum.

In 2014 developed and conducted a Northeast Health Plex

Access Survey – 91 completed the survey. Survey conducted

again in 2015 once Health Plex open to ensure findings

remained the same

Posted health and wellness messages on the Primary Health

Care Provider Team’s health promotion sign at the Tisdale

Hospital/Healthplex grounds

In 2014 ‘Cooking for One’ classes were offered with no

registrants

Partnered with town/school for Mayor’s Challenge in 2013

and 2014. Tisdale vs Nipawin – Tisdale declared the winner

both years

2013 updated the Community Services portion of the

Community Guide for Chamber of Commerce

Lifestyle Balance Program (7 sessions offered in 2013)

2014 offered a Dementia Workshop to the public and to

health care providers

NP develops a road map of Tisdale’s Suicide Awareness

Committee to share with other Primary Health Care

Provider Teams in the region

2014 Mental Health team members develop a Suicide

Bereavement Support Group for survivors affected by

suicide. This group is open to other communities.

Why does it take so long for a psychiatrist referral? From the time of referral to the time of seeing the specialist, things change. Then insurance plans aren't willing to give money. This is extremely frustrating and causes more stress.

Couldn't get an office appointment with sick child as no appointments were available with regular doctor. Phones at 10 and went to ER at 2. Told Dr not in until 5. Tried over the counter remedy - didn't work so days later back to ER in an evening. With sick children could they not be prioritized at clinic so ER is not required?

I have phoned for an appointment with my main health care provider and I had to wait a month, that’s too long. Need to have two or three appointments every second day set aside for people who have an emergency and I mean with their doctor. And when an 81/2 month pregnant woman needs a two week appointment why give her an appointment for a month later.

Immunization for shingles

I would like to have a Dr who is concerned about my overall general health. Not much conversation, doesn't ask more than required questions regarding my concern for the visit where I lived in bigger city even the medical people (massage, chiro, physio) asked more questions and I felt they cared about my overall health rather than treating the 1 particular symptom. I am very fortunate to be healthy however I feel the dr's just care about the particular ailment and not my overall/general health

I do not mind paying the $200 for the vaccine itself but the system is inefficient

More accountability from those in administration for what actually takes place in the overall health "system"...is a concern for us. In particular, the relationship between the health care providers, Home Care and, those who require "home care" is, in my opinion, something that needs to be addressed. It is obvious we do not have, and, perhaps, may never have enough "Assisted Living" for seniors and others who will require care at home. So, we need to talk about what we see around us now and what Administration thinks is happening. If, and, when we require care at home, we would like to know the caregivers are trained, are compassionate and have time allotted to do the job. And, simply put, Supervisors need to supervise.

More small group meetings...i.e. in community spaces. Example....my concern re-Home Care comes from years of observing how it works and how it has changed. It is a perception in our community that families have little or no say. Perhaps, small group meetings with those facing life changes, and, health professionals could help design a better way of caring for those who will need care in their home and beyond.

My family and I are very concerned about the staffing issues at New market Place--absent staff not replaced, a regular consistent staff in each facility which has been recommended by the advocate to make residents feel more comfortable, and very poor organization when serving meals.

No private care facilities or assisted living. Elders may not be eligible for either subsidized senior housing or a place like Caleb

Rural people are treated as less important than urban people. As I

grow older and have more health concerns, I fear the healthcare

Sidewalks and streets are poor especially for seniors - street sweeper lots of stones/rocks
