EMIS Web Post Death Audit Tool Document Template

Supporting Guidance This document contains guidance on completing a post death audit for your deceased patients within EMIS Web and then exporting this information to the relevant contact at the CCG at [email protected]

To get the Document Template into EMIS Web

As a pre-requisite your unit will need to ensure this template is loaded into the documents/letters section within the EMIS Web Template Manager as directed below: • Within EMIS go to Configuration>Template Manager

• Click on the Document Templates tab and select the folder that you wish to save the Post Death Audit Tool into. Then click on the Import button and choose Document.

• Locate the Post Death Audit Tool document that you will have saved into a folder from the email you received and then Import.

Completing the Post Death Audit Tool document and sending results Collating or sending results at the point of document completion in a patient record Once the document has been completed in a patient record, at the point of completion the user can choose to either:

• Send the completed document as an individual attachment via NHS.net email to the CCG contact at [email protected] (The benefit of this is that it allows you to send the completed document directly to your intended recipient as a single fluid process and we would advise that this is the recommended option).

or

• Export the completed document from the patient record to a specified folder on their network and collate the documents this way to (at a later date) send via email ‘en mass’ to the CCG contact.

or

• Print the document and store to collate and send via post ‘en mass’ to the CCG contact. Collating or sending results retrospectively Unfortunately, reports cannot be created for the free-text information in questionnaires within the EMIS Web environment, however, reports can be run on read codes which can be attached to the overall document (for convenience the “Post Death Audit Tool completed” read code EMISNQPO60 has already been added to this document). If you decide not to send the document via email at the time of completion, the only other option at a later date, is to run a report based on the *post death audit tool read code* (document type) and find the list of patients with this document completed. You will then need to either:

• Export the documents from each patient record one by one to a folder and then send an email to the CCG contact with these attached.

or

• Open the document from each patient record one by one and send via email individually.

NOTE: Please note this retrospective option will mean more work for the practice; it would be

more beneficial if the user sent the document via email at the point of completion in a patient record.

To launch the Post Death Audit Tool Document Template within EMIS Web

• Retrieve a relevant patient and click the ADD button, hover over the document option and select Create Letter.

• Click the spy glass to search for the Post Death Audit Tool document and press OK to launch. (You will have to search your practice folders for the document upon first launch, but after this it becomes one of your favourites):

• Complete the document.

NOTE: For yes/no tick boxes you have to remove document protection to enter content

into these, to do this, select the Developer Tab and click the Padlock (Restrict Editing pane). You may also have to de-select this option in order to enter content into the rest of the document.

• Once you are ready to save the document and you select “Save As”, you have the option to “Send Email” (via NHS Mail to [email protected]) or to ‘Export’ the document to a designated folder or ‘Print’ the document to send later.

• Once saved, this document is also available within the patient’s record under the “Documents” section.
