cfc patient story: david & verna ding David Ding has been a bartender at the Aberdeen Barn restaurant for many years. With two children and no health insurance, David and his wife, Verna, were barely able to cover the family’s routine medical care. Surgery a few years ago for David’s life-threatening thoracic aortic aneurysm put the family in debt. By the time they found the CFC in 2003, both were suffering from the effects of high blood pressure and David had an abnormal heart rhythm. Unable to afford preventive dental care, David had developed extensive dental decay, which could worsen his heart issues and affect his overall health.

Verna and David Ding are prototypical CFC patients—middle-aged adults with complex, extensive medical needs and no health insurance. Without ongoing support for their chronic health problems, their quality of life is compromised and they are more likely to die prematurely. For their high blood pressure, David and Verna need regular blood pressure checks, lab tests and medication. David needs a CT scan every two years to monitor his thoracic aortic aneurysm repair site, medication for his atrial fibrillation and extensive dental work. Verna’s diabetes has been

difficult to control even though she has been diligent about following a prescribed diet and exercise routine. She needs to be followed closely—regular checkups and lab tests as well as eye exams and foot care. She must take several expensive medications and needs a glucometer and an ongoing supply of test strips to self-monitor her blood sugar levels.

Fortunately, the CFC is able to provide almost all of the Dings’ health care

services for free through our medical and dental clinics and medical referral network. The Dings’ situation helps illustrate the complexity of providing health care and the reason why the CFC is constantly growing and evolving. We also hope that it gives you an appreciation for the enormous amount of support that the CFC receives from the community each year to help people like the Dings—from volunteer time and expertise to in kind medical and dental services and financial support.

dear friend of the free clinic, Amidst challenging times, it is clear this is a caring community. Because of our volunteers and donors, the Charlottesville Free Clinic continued expansion in every area of service, and achieved a record-setting number of volunteers and donations during FY10. We couldn’t have done it without you.

Looking back on the past year there are several examples that demonstrate we are a dynamic organization, ever evolving to better serve our patients and the community at large. Some highlights include the following:

launching the Physician Partners Network to serve more people each year by matching low-income uninsured adults with community primary care providers.

adding equipment to our dental and medical clinics to allow us to operate with greater precision and expand our scope of specialty care on site.

creating an electronic scheduling program to attain greater efficiencies with patient and provider scheduling.

providing leadership in the formation of the Community Mental Health and Wellness Coalition to better address our community’s unmet adult mental health needs.

With our accomplishments over the past year, we are well positioned for the challenges of FY11. Facing unprecedented requests for service, we will need to marshal our resources to do what we have always done: evolve to meet the community’s unmet health care needs. We can’t do it without you.

Some wonder what our role will be now that health care reform has been adopted. I can tell you that it means the following:

Until 2014, the patients

we serve will not have

noticeable relief from any

of the current provisions of

the legislation. Their needs

will remain dire. The CFC

will be there for them.

After 2014, when many of the insurance reforms are implemented, there will still be people falling through the cracks, because the Medicaid threshold still leaves them uncovered; because the “affordable” health care pool they can buy into isn’t affordable enough; because people will still be making decisions about their health care;

and because, for myriad reasons, some (an estimated 23 million) will remain uninsured.

Today and after health care reform implementation, the Charlottesville Free Clinic will continue to leverage volunteer time and donor dollars to achieve significant results for people who are left out of our country’s health care system.

The story of CFC patients David and Verna Ding illustrates the challenges many of our patients face and demonstrates the way the Charlottesville Free Clinic has been able to evolve to meet their needs.

Volunteers are the driving force behind the CFC’s mission to provide free health care services for uninsured people in our community. During FY10, 518 community members donated more than 13,000 hours to the CFC and our patients—a 20% increase in volunteers and a 6% increase in hours over FY09. In all, volunteers staffed 321 day/evening medical clinics, 585 pharmacy hours and 339 day/evening dental clinics during the year. The synergy created by people with widely varied professions and life experiences

coming together with a common purpose

is powerful and effective.

On any given day of the week, anywhere from 5 to 35 volunteers are at work at the CFC—treating patients, filling prescriptions, answering phones, registering patients, completing Patient Assistance Program (PAP) applications, reviewing test results, scheduling referral appointments, interpreting for patients, filing charts, planning fundraising events and much more.

The CFC’s evening medical and dental clinics start at 5:15 p.m. Most volunteers come straight from work or classes and often stay until 8:00 or 8:30. A cadre of local restaurants, caterers, civic groups and individual cooks generously provides dinner for our volunteers three nights each week. During FY10, our “staff stuffers” provided 140 dinners for evening medical, pharmacy and dental volunteers.

During FY10, volunteer providers treated 1,142 patients with 3,201 visits to our evening medical clinics. A majority of these visits (79%) were for primary care, which included treatment for routine acute illnesses, management of chronic diseases, health screenings and preventive care such as flu shots.

Additionally, many of our primary care providers address mental health as part of their patient care. Volunteers from the Women’s Initiative provide wellness consultations for patients on a weekly basis.

In addition to providing ongoing care for patients with chronic illnesses, Barrie saw dental patients who needed antibiotics or who needed to be evaluated to ensure they were healthy enough for dental treatment. Barrie also adjusted her schedule to accommodate appointments for patients with acute needs who couldn’t be scheduled soon enough during the evening clinic.

CFC MEDICAL PATIENT DEMOGRAPHICS FY10

In January 2010, the CFC medical and dental clinics switched to an electronic scheduling program custom-designed by our IT consultant, Joanne Rogol. In addition to its myriad benefits, our new program has helped us schedule patients and providers more efficiently and track no-shows and schedule changes more effectively.

During FY10, volunteer pharmacists and pharmacy technicians filled 28,396 prescriptions for CFC patients. In addition to community volunteers, several pharmacy residents from Martha Jefferson Hospital have been volunteering with us regularly.

Through the Pharmacy Connection and Rx Partnership

programs, we secured free medications for 696 patients

valued at $1,334,268 during FY10.

For many years the CFC had to refer patients to community dentists for Panorex x-rays; now, however, thanks to Dr. Larry Brannon, we have our own Panorex. Panorex x-rays provide a panoramic image of a patient’s entire mouth—teeth, bones and soft tissue. They are extremely helpful in diagnosing gum disease, abscesses and oral cancer and are necessary before and during oral surgery. Not only can we provide our volunteer and community dentists with a more complete evaluation of each patient’s dental needs but now we can also perform minor oral surgery on site.

During FY10, the CFC served 1,466 dental patients with 2,838 visits to our day, evening and Saturday dental clinics—a 54% increase over FY09.

In its second year of operation, the daytime dental clinic,

staffed by Dr. Conrad Jenkins, increased the number of

patients seen by 113% and the number of visits by 77%.

OF COMMUNITY MEDICAL AND DENTAL PROVIDERS.

Although the CFC is able to provide some specialized medical care on site, patients often need to see a specialist. Additionally, lab tests, x-rays and specialized radiology services are critically important tools that the CFC cannot provide on site. Over the years, the CFC has developed a network of specialty health care providers who see CFC patients in their offices or provide their services free of charge or for a reduced fee.

In addition to providing acute and restorative dental care on site, the CFC has established a network of local dental professionals who have agreed to provide free dental services for CFC patients in their offices. The Community Dental Program (CDP) has grown from a handful of dentists to more than 40 dental professionals, including endodontists, oral surgeons, periodontists, orthodontists and prosthodontists.

During FY10, CDP providers treated

228 CFC dental patients for routine and

specialized dental issues—a 90% increase

over FY09.

During FY10, the CFC established a partnership with the MCV/VCU School of Dentistry. Supervised by local dentists, VCU dental students treated patients during seven Saturday clinics starting in December 2009. Not only has this helped us serve more people on the dental waiting list but it has also enabled us to work with clients from local organizations like the International Rescue Committee and Charlottesville Health Access—groups who may need interpreters and other special support.

During FY10, in addition to hundreds of lab tests provided by Martha Jefferson Hospital and the UVA Health System, 53 community health care specialists treated more than 500 CFC patients.

Striking a balance between taking on new patients and providing more primary and specialized medical services for our current patients has been an ongoing challenge for the CFC.

Our capacity for accepting

new patients is directly

related to the number of

primary care physicians

who volunteer with us.

For that reason, we have

focused on recruiting

efforts and being creative

about ways to engage more

volunteer physicians.

Fortuitously, Dr. Andy Macfarlan contacted us about an initiative he was interested in starting in Charlottesville whereby local physicians would agree to see low-income, uninsured patients for free in their offices. The advantages to a partnership were apparent from the beginning. Primary care providers would be able to see uninsured patients in their offices with free ancillary medical services provided by the CFC and its established partners, and the CFC would have an expanded network of primary care physicians to be able to treat more patients. Furthermore, by

placing patients with complex primary care needs with community PCPs, we can provide better continuity of care for these patients, as well as create more appointment slots on site. Finally, with more uninsured people having access to primary care, fewer will seek routine

care at local emergency departments (ED), lowering their medical costs/debt, freeing ED space for people with urgent needs and reducing costs for our local hospitals. The new initiative was christened the Physician Partners Network (PPN).

the PPN has engaged

13 doctors and nurse

practitioners who,

collectively, have agreed

to provide ongoing care

to 75 patients.

In the few months the program has been running, the estimated value of donated services is more than $15,000.

More important, though, are the relationships these patients are establishing with care providers they can call when sick. The PPN “health assurance card” they have each received is just that: assurance of getting necessary health care rather than either utilizing an emergency department or not seeking care at all, which 65% of CFC patients report they would have done if our services were not available.

The Pharmacy Connection (TPC)program helps us streamline the process of applying for free medications from pharmaceutical companies’ Patient Assistance Programs (PAP) on behalf of individual patients. Through the RxRelief Virginia program—funded locally by the CFC, United Way-Thomas Jefferson

Area and Martha Jefferson Hospital—a medication assistance caseworker is working on PAP applications at the CFC 10 hours each week. Through the Rx Partnership, the CFC receives a free bulk supply of certain medications. Each year many private health care providers donate equipment and

supplies. Additionally, more than 100 local medical, dental and health care specialists see CFC patients for free or at a reduced fee through our Physician Partners Network, Medical Referral Network and Community Dental Program.

IN KIND DONATIONS FY10

The CFC is committed to providing our services in the most cost-effective manner possible. Low overhead is one of the primary reasons we have chosen to expand within the Health

Department building. The community supports us generously with donated time, expertise and services, which also helps us keep our expenses modest. Our health care partners—Martha

Jefferson Hospital and the UVA Health System—provide free lab tests and diagnostic radiology services for CFC and PPN patients.

Our operating budget jumped dramatically

in FY10 (to $1,000,298) as we assumed

the full cost of a paid dental staff for our

daytime dental clinic.

Fortunately, increased support from the City of Charlottesville and Albemarle County offset most of this increase. As in past years, during FY10 the CFC’s primary expenses were program-related, including pharmaceuticals as well as dental and medical supplies.

GRANTS AND IN KIND—THAN ANY YEAR IN THE HISTORY OF

THE ORGANIZATION.

Noteworthy facts about our community’s generosity to the CFC in FY10 include the following: Most of our donations came from individuals. About 60% of the funds raised came from a group of approximately 1,000 loyal donors—individuals, foundations, businesses, civic groups and churches—who provide consistent annual support for the CFC.

calendar year 2009.

Notably, a generous donor provided a $50,000 matching gift challenge for new or increased donations to the CFC’s annual fund campaign, boosting the drive by 22%. We also received many more gifts this year in memory or in honor of friends, colleagues and family members.

Another reason for the substantial increase in our FY10 fundraising total was a $197,768 funding increase from the City of Charlottesville and Albemarle County. When the CFC took over the Health Department’s dental suite full time, Charlottesville and Albemarle reallocated funds designated for the Health Department’s dental clinic to CFC dental programs.

COMMONWEALTH OF VIRGINIA CAMPAIGN CY2009

The Commonwealth of Virginia Campaign (CVC) is an annual charity fund drive for state employees. Since 1997, employees of the University of Virginia and local state agencies have generously supported the CFC through the CVC. For the past three years, the CFC has been the number #1 designated charity for UVA’s CVC campaign and statewide. Our congratulations and thanks to UVA employees and CVC volunteers for a highly successful CVC campaign last fall. UVA raised more funds than any other educational facility or state agency—surpassing its closest competitor, the Virginia Department of Corrections, by more than $400,000. Notably, the Charlottesville Free Clinic was again the #1 charity choice of UVA employees. We received 442 pledges totaling $80,423—a 3% increase over the CY2008 campaign!

The Charlottesville Free Clinic’s signature fundraising event, our annual benefit concert, has allowed us to confidently expand our services knowing that we can count on the success of this event each year. This year’s concert didn’t disappoint. Sheryl Crow and Colbie Caillat drew a sellout crowd to the Charlottesville Pavilion on Monday, June 21, 2010, and provided outstanding performances. Thanks to the generosity of our sponsors, in kind donors and the community members who purchased tickets, the concert netted $130,000 for the CFC.

For the past eight years, the CFC is fortunate to have been the beneficiary of a growing annual golf tournament hosted by the Central Virginia chapter of the National Association of Insurance & Financial Advisors. The 2003 tournament raised $4,290. Thanks to the generous support of the tournament sponsors, in kind donors, raffle/silent auction prize donors and teams, this year’s tournament generated more than $20,000.

CFC FOUNDERS CIRCLE

Established in 2005 in honor of CFC founders Drs. Mohan Nadkarni and Paul DeMarco, the CFC Founders Circle recognizes generous supporters who have made cumulative donations of $25,000 or more to the CFC. During FY09, 14 donors became eligible for the Founders Circle. In October 2009, the CFC honored new members of the Founders Circle at a reception at the home of Susan & L.F. Payne.
