•Have a clear understanding of how your child’s illness will affect him/her academically.•Ask when/if your child can go back to school and what modifications your child might need upon returning to school.

•Inform medical personnel of any changes your child might be experiencing at school; academically, behaviorally or socially.

Request written documentation when needed to assist you in advocating for your child at school. Documentation should include:

•diagnosis/prognosis•side effects of treatment that could impact your child’s classroom performance•possible problems with infectious disease •environmental restrictions•activity restrictions/precautions•name of medical personnel to be used by the school as a contact
