Nursing Theorists inTranscultural Nursing

Madeleine Leininger—the founder ofTranscultural Nursing.Dr. Leininger studied anthropology in the 1950’s andearly 60’s. She earned her PhD in CulturalAnthropology. She then decided that nursing wasconstantly dealing with the lifeways and patterns ofpeoples of many backgrounds and that in caring forthese persons, nurses needed to be sensitive to theunique needs of all peoples. She felt strongly thatwe should NOT impose our views on otherswhenever possible.

Leininger’s Theory

She developed the “SUNRISE MODEL” as a basisfor assessment and research in nursing.

This model remains as a standard in TransculturalNursing Theory.

Leininger’s Theory (cont’d) Note that the model looks like a Sunrise and the

Worldview encompasses everything that makespeople who they are.

The 7 Cultural and Social StructureDimensions are the large areas that nurses needto learn about through interview and living amongthe people of different cultures.

Below the Individuals, Families, Groups,Communities, & Institutions are the DiverseHealth Systems that all persons deal with invarious ways.

Leininger’s Theory (cont’d) Generic or Folk Systems are the

everyday remedies an individual, familyor group may use to promote wellnessand healing. Some examples mightinclude:

Chicken Soup Chamomile Tea or other Herbal teas Voodoo Sacrifices of birds or animals

to the spirits Prayer Curandero

Nurses need to assess in all theseareas to plan safe, effective care.

Professional Systems are different dependingupon the Health Care Delivery System of thatculture. Whether there is socialized medicine, private insurance, communal healthcare, pooreconomic support vs. great wealth, all contribute tothe influence of the utilization of Professionals toprevent illness, heal illness, and promote health andwellness.

Nursing must collaborate within the healthcaredelivery system to plan and implement safe, effective, culturally competent care.

How to ProvideCulturally Congruent Care:

Leininger suggests 3 approaches to planand implement Culturally Congruent Care.

The first is Cultural CarePreservation/Maintenance where we lookat what we as nurses can do to preserveand maintain the cultural practices of anindividual or family while receiving safe, holistic care somewhere in the HealthcareDelivery System.
