Disability Strategy
The Suffolk Learning Disability Partnership has
made a plan. This is called the Joint Learning
Disability Strategy 2015-20, and was agreed by
Suffolk’s Health & Wellbeing Board.

The Learning Disability Partnership is responsible
for setting priorities and making sure people
are doing a good job.
It’s important that Transforming Care and the
Strategy work together.
Self-advocates and family carers helped
shaped the key messages and priorities. This is
called co-production.

The strategy has a 7 Workstreams, each with a
Workstream Lead and a Champion (either a
family carer or self-advocate). The Workstreams are:
The Strategy is also looking to create a young
person network, to support and encourage
co-production with young people.

The Strategy involves support providers through
the Provider Network.
The Strategy involves support workers and
personal assistants through the Support Worker
and PA Network.
The Strategy involves self-advocates through
Suffolk People First events.

Each Workstream has projects and priorities
that help work towards the vision of the Joint
Learning Disability Strategy.
The Transforming Care Partnership Board
influences the activity of all Workstreams and
makes sure there is a focus on the Transforming
Care ambitions.
A role of the Transforming Care Partnership
Board is to pull together the activity and
delivery in all of these areas to make sure
we’re all working together.
The Transforming Care Plan in Suffolk reflects
the priorities in the strategy.

Transforming Care is for people of all ages and
includes people with autism without learning
disabilities.
The Joint Learning Disability Strategy is for
people with learning disabilities over the age
of 14.
It’s important that all this work links together,
and that there are no gaps.

Disability Strategy
People with learning disabilities will be included
in all redesign decisions within Transforming
Care through Suffolk People First events.
The Learning Disability Partnership Board will
have regular updates on progress from the
Transforming Care Partnership Board.
Updates will also be shared through Learning Disability Partnership Board meetings.
The My Life, My Future projects are working to
improve Suffolk County Council funded
community resources for adults, listening to what the Joint Strategy says.

Suffolk’s co-produced definition of Co-
production is that: “Co‐ production is people,
carers and professionals working together as
equal partners to: design, develop,
commission, deliver and review services,
information and advice”.
Co-production
Co-production is a way of working where
people who use services, family carers, service
providers and other professionals work together
to create services which works for them all.
We worked together and came up with what
co-production means in Suffolk.
Co-production
Norfolk and Suffolk Foundation Trust have co-
produced a plan of action which responds to what the Strategy says.
An ‘Intensive Support at Home’ team will work
with people in the community stopping them
from going into hospital or supporting an early return home.
Norfolk and Suffolk Foundation Trust,
sometimes called NSFT, provides mental
health and learning disability and autism
diagnosis services in Norfolk and Suffolk.
