TRANSITION SERVICESTransition Services is in its sixth year of providing

vocational services to American Indians with disabilities in High School.

Our goal is to develop specialized services to American Indian students with disabilities.

We can assist with job training or post-secondary education.

WHAT DO WE DO?Help disabled American Indian high school

students transition from school to work or post-secondary education.

Assist in helping maintain a job or post-secondary education

By providing for the consumer what ever may be hindering them from working or getting to school.

Services vary on a case to case basis.

QUALIFICATIONSMember of a Federally Recognized Tribe Enrolled Tribal Member Tribal Enrollment Card

Must Reside in one of the Iowa Tribe VR Service Area Counties

Must Provide Documentation of Disability

Note or letter from physician or doctor stating disability IEP is accepts as a proof of disability 504 plan is also accepted
