The 1-10-30-600 Ratio
The relationships in the ratio indicate quite clearlyhow foolish it is to direct our total effort at therelatively few events ending in serious ordisabling injury when there are 630opportunities to identify TRAPS that providea much larger basis for more effective controland elimination of hazards.

TRAP
In an article titled Fatal Accidents onthe rise in 1997: A Cause forConcern
Near misses need to be considered aswarnings, analyzed, and used to preventmore serious accidents from occurring.Near misses are incidents which usuallyindicate the presence of a hazard - or inother words a trap.

TRAP
Given just the right combination of inattention, timing, and bad luck, atrap can spring its teeth with serious
results.Whats often discouraging is that, during many accident investigations,we find that the hazard, or trap, hadexisted prior to the accident (eveninvolved in a near miss), but wentunreported and uncorrected.

TRAP
Why?
The question is, Why did the trapremain until an accident/incident
happened? Who had the opportunity to identifyand correct the TRAP?
How many near misses had occurredin the past?

There are many other reasonsemployees dont come forward withinformation concerning hazards ornear miss situations. If only employees could understandthat near miss situations present the

Near misses are free opportunities toidentify, evaluate, and eliminateexisting traps in the workplace.Employees are in the best position torecommend solutions to theproblems when identified. They often know how to disarm thetrap situation, if asked to do so.

TRAP
The emphasis of this program is noton analyzing accidents which havealready happened, but on capturingnear miss information or identifyingexisting traps which have notproduced injury or lost time.
That is not to say it is not importantto properly analyze and correctreportable accidents.

TRAP
The key to the programs success is toget all the employees involved.
This starts by addressing the barriers toreporting near miss events and creatingincentives for identifying traps.
Companies need to encourageemployees to be TRAP hunters and findways to dismantle any traps found.
