Telehealth is the use of information and communication technology to connect people to health care expertise at a distance. The Telehealth Foundations course is an introduction to telehealth and is designed to provide telehealth site contacts and other on site staff key information required to manage telehealth at their facility.

Telehealth is the use of information and communication technology to connect people to health-care expertise at a distance. (A secure, high-speed video link is used to connect a patient to health-care professionals at another site within the province, within the country or even somewhere around the world.) You are able to see, hear and talk to health-care providers in real time. A patient is able to see, hear and talk to their care provider on a television monitor within their local health-care facility to discuss results from their last set of tests for example. Rural and northern areas receive services without leaving their community. It allows access to care while remaining close to family, friends and community supports at a time when added emotional and physical help may be needed. It reduces the hardship and inconvenience of being away from work and home.

Clinical events - any event involving a patient. •Clinical events help to increase access to services that may otherwise be unavailable in a person’s home community. Some examples of clinical events are: specialist consultations, follow-up appointments, client teaching, case conferencing and discharge planning.
Education events - events that allow health-care professionals to stay at their own sites and/or in their own communities while continuing professional development. •Some examples of education events include: Continuing Medical Education, on-going nursing education, grand rounds offered by the University of Manitoba, and program specific education events. •MBTelehealth can also connect outside of Manitoba for National and International events. Additional charges may apply, for example, the current billing rate is $75/hour for the event as of 2016. Administrative events - events held for the purpose of conducting meetings and interviews. •Holding these types of events using telehealth reduces travel time for administrators and increases their ability to participate in multiple meetings all over the province regardless of distance. •Some examples of events that fall under the administrative events type are: regional planning meetings, joint committee meetings and inter-provincial interviews. Televisitation - an event where a patient who is on an extended stay in hospital is able to connect with family/friends in their home community. •One example of a televisitation could be when a mother with a new baby in the Neonatal Intensive Care Unit is able to connect with the father who has returned home to take care of other children or for work commitments.

Events can be scheduled in three ways: centralized scheduling, self-scheduling or self-registration. With centralized scheduling, all requests are submitted using Booking Forms which are available on the website. Once submitted, the MBTelehealth Schedulers will review availability and respond to you within 2 working days. Self-scheduling is done using the MBTelehealth scheduling system which is a web-based application that allows MBT to give access to users in order to schedule their own events; either clinical or non-clinical. Site contact access allows users with basic access to the scheduling application to add their sites to an existing event. This is used most often for sites who identify an interest in a particular topic or education session. The site contact is able to log in and add their site to the event without having to go through the central schedulers. It’s typically site contacts who self-register. There are many self-schedulers that book telehealth events (ex: at rural sites), it’s important for site contacts to review newly scheduled events. Where do I find the forms?

Who do I book with?
The forms are found by clicking on the Clinical and Non-Clinical section of the Home page on the MBT website. Clinical form: • If a patient is attending the session, it is considered “clinical” so you would be using the
clinical booking form (this includes televisitation). Non-Clinical form: • If a patient is not attending, then you would use the non-clinical form (eg education & rounds
or admin & meetings). • If the event is for discharge planning or a case conference with no patient attending, you
would use the non-clinical form; however, you would check the “clinical” radio button under the topic session.
The billing information on the bottom of the non-clinical form needs to be filled out in certain instances; such as for out of network sessions. Click on the Locations tab and then the site for contact information.

Important tips about Clinical Booking Forms: • fill out the client information section correctly so that the correct patient will be booked in the
system, and so that there is not a duplicate patient record created • If you do not know your telehealth site, it’s better to leave that field blank rather than put in an
incorrect site number

Important tips about Non-Clinical Booking Forms: • confirm attendance at your site before you submit the booking form • ensure you fill out the site & room fields - if the schedulers receive a form without these fields
filled in, they will have to call you and you will need to re-submit the form; the booking will not be completed until they receive the correct form
• host/presenter/person booking on their behalf selects the layout for a multi-site event

Booking Forms • requests must be received before noon on the day prior to event • pay attention to the Video Conference Unit (VCU) that you are scheduled to connect to because connecting to the wrong VCU will connect you to the wrong event Clinical booking forms must be faxed only (not emailed) because they contain personal health information. Non Clinical forms can be faxed or emailed. and are willing to support the event as required (i.e., delivery of equipment, assisting the patient to the room and helping to connect if needed).
Additions that are out of province – best efforts to connect the same day will be made by MBT staff • It is not always possible due to circumstances beyond our control. The requester must
provide contact information for the other site in order to facilitate this request. Changes and cancellations - contact the MBT schedulers to inform them of any changes or cancellations • The MBT scheduling system needs to be updated and the appropriate parties need to be
notified.

that you are connecting to. Remember! Some towns/cities have multiple telehealth locations.

This information applies to both clinical and non-clinical events: • Know what type of session you’re having – site to site or multi-site. • Locate the 5 digit number for the site/equipment that you are connecting to, enter it using the
keypad on the remote, and confirm the 5 digit number against your paperwork once again before you press the Call button to connect. Do not keep trying to connect to different rooms at the site until you see someone on the other end. If you are unsure of the room, please contact the MBT schedulers.
• Troubleshooting tip - if you’re in a room and the equipment connects and it is not your call, disconnect the call by pressing the red button on the remote.
Clinical • Remember, by connecting into the wrong room you may be interrupting someone else's
confidential clinical appointment (in some instances, it may not even be a telehealth event as the rooms are used for other clinical in-person appointments.
Non-clinical • Make sure you leave the meeting room on time as you may be interrupting the next person’s
session (meeting or appointment) • Bring your confirmation to the room with you so that you know which site you are to connect
to.

Check iScheduler daily and always ensure you’re relying on current information. For example, the equipment and/or the room may have changed since you received your initial confirmation. Find the 5 digit number for the site and equipment that you are connecting to – the number is found in brackets behind the VCU # on each of the reports. • Whiteboard - shows non-clinical and clinical events • Confirmation - received from iScheduler when an event is booked • Clinic List - shows clinical events If you have missed the connection to your multi-site event and you are registered for the event, you can use the self-serve option to connect: • Find the connection information on the whiteboard • Dial the appropriate number as shown on the whiteboard and enter the PIN when prompted

Mute your microphone when you are not speaking or are watching a presentation to cut down on background noise. This also protects the privacy of clients and their personal health information – others in close proximity may be discussing confidential matters and may not realize they can be overheard. Use videoconferencing equipment in an appropriate and confidential location to maintain the privacy of your clients and their personal information. •make sure room doors are closed •ensure the mobile equipment isn’t placed in an open area •when using MyMBT Videoconferencing (desktop video), ensure others around cannot overhear your conversation or view your computer screen Always consider the camera and microphone as live. This will help ensure there are no sidebar conversations taking place where patient information may be overheard by other sites. Everyone attending the event Important! Even though the session is taking place via videoconference, it should be treated the same as an in-person meeting or clinical appointment.

These are the most common videoconference options, but you may have other types of equipment at your site. Mobile carts have all 5 main components mounted onto the cart with wheels so that it can be used in multiple rooms for multiple applications within a facility as long as the room has a dedicated telehealth network drop.
The fixed system is wall mounted and has all the same equipment but is permanently installed in a designated room. Fixed systems can be used with large monitors as boardroom solutions or with smaller monitors in clinic spaces to reduce the amount of moving that is required with mobile carts. The MyMBT (Powered by Jabber) solution includes a webcam on your computer and a connection to telehealth from your desk. MyMBT also has mobile capabilities that will allow users to video conference from their mobile devices.

• The MyMBT Video service offers a different way of connecting to telehealth sites. • The application can be launched from your desktop or mobile device, wherever a high speed
internet connection exists. • It provides secure, real-time videoconferencing that can be used for both clinical and non-
clinical telehealth events. • If you are the “attendee” site while using MyMBT Video, you will need to accept the call, they
system will not auto-answer. • If you’re connecting to 3 or more sites, you need to book the event in iScheduler. Note: There is a Frequently Asked Questions document available if required.

Telehealth equipment consists of 5 primary components. While the components will be consistent, there are different models in place at various sites so your equipment may look a little different than the pictures show.
The Camera is a high-quality imaging device used to transmit near end images to the far end site.
The Codec is responsible for the compression of outgoing video, audio and data, the decompression of the incoming information and the transmission of this information between endpoints. The codec acts as the central processor for the videoconference system.
The Screen or Monitor is a commercial grade monitor used to display images from the near end and far end sites. The user controls what is displayed through the codec and remote control.
The Remote Control or Touchpad is used to manage the image display, place calls, adjust the volume, navigate screens and select options. It controls all functions of the videoconference equipment.
The microphone sends the audio information; there are table top, microphones affixed to the mobile carts as well as ceiling microphones. Note: Some equipment may also have a router. When connected to an MBTelehealth drop, the router provides a secure connection back to the MBTelehealth network.

Fixed
Telehealth equipment connections involve plugging in a power cable and an network cable. For fixed and desktop solutions – everything is usually already plugged in but you’ll need to have access to the remote. Pick up the remote to wake up the system. For mobile systems - the power cable is plugged into a standard power outlet. The network cable is plugged into an MBTelehealth drop that is configured for use on the MBTelehealth network. These drops will be identified by labels (often MBT or telehealth). Once you have those two cables connected you can turn on your videoconference equipment. One other thing to note is that some units are connected to a power bar which has a power button that controls the entire unit. Make sure the router, codec and monitor are all turned on before trying to make a connection.

PRESETS are only used with the Touchpad 10; not with the TRC6. It is still important to understand camera “positions” as a best practice as it is useful information during an event. Presets allow you to switch back to other views quickly. MBTelehealth recommends the use of 3 standard camera presets. The remaining number pad buttons can be used to set up alternative camera presets to suit the needs of the event.
“Sign” view – • It’s an off screen camera view that allows users to move about the room without being on
camera • It identifies the sites that are connected. The sign can be placed on a table (tent sign) or
hung on a wall • The camera should always be set to the sign view before connecting to an event “Participant” view • This is a close up view of the focal person(s) of the event (such as the patient, provider or
presenter) • This view allows far sites to see the person clearly • A good rule of thumb is to frame the participants from the waist up
“Room” view • This provides a view of the entire room and identifies everyone in attendance • Part of video conference etiquette includes making the sites you are connected to aware of
everyone involved in the event • This view is also helpful for question and answer periods

In addition to the different event types, there are also different connection types. These connections can be used for all of the event types.
The first and most common connection type is a Site to Site connection which is a direct connection between two sites. The event is connected using the telehealth address book or by dialing a number provided for an out of province site.
Multi-Site Connections are events that involve 3 or more sites. The event is connected automatically by MBTelehealth using a multi-point connection unit or MCU, which is also referred to as a bridge.
An Out of Province or out of Network Connection involves connecting to a site that is not on the MBTelehealth network. This can be a connection to another provincial telehealth network or to a local Manitoba site that is not a network member. Out of province connections can be site to site or multi-site. These connections are completed by dialing the a long distance number that is provided with your event confirmation. Out of province multi-site connections are automatically connected by the MCU just like all other multi-site events.

The Multi-site connection is an automatic connection that takes place when 3 or more sites are scheduled for the same event. •Plug in and turn on the equipment prior to the session •All of the sites will be connected to the event automatically – meaning, you do not need to use the phone book to connect to any of the sites, even if you are the attending site •If you miss the connection, you can connect to the call by using the dial-in information found on the Whiteboard or Confirmation. •If you still are not able to connect, call the Service Desk.
Note: Multi-site (MCU) sessions will be connected to the event automatically at the scheduled start time and automatically disconnected at the scheduled end time. Press the red End Call button on the remote at the end of every call. Also – if no attendees show up after 10 minutes ensure you disconnect the call. By ensuring all calls are disconnected – you are protecting patient privacy.

Site to Site Far End Control – all steps are listed on the QRG that is located on our website: • Navigate to the Participants icon at the top of the screen. • Press OK, scroll down to select the participant and press OK again. • Arrow over to the camera icon and press OK. • Press the Camera icon and adjust as needed using the pan/tilt arrows and zoom +/-. • Press the Back button to exit.

They offer additional display options or are used in a telehealth consultation to provide additional information to a health care provider. Laptops - used to display presentations and collaborate on documents Patient Cameras - used to display close up images of a patient’s body (ex: wounds, skin, surgical incisions) Otoscope – used for ear, nose and throat (ENT) examinations over telehealth; it has excellent imagery for lower sinus and upper throat examinations Other Devices - MBTelehealth also has other devices located at certain facilities throughout the province (for information on locations and access, contact your Facilitator-eHealth Solutions) Document Camera – isn’t used anymore, but in previous years it was used for showing paper copies, printed text/pictures, 3-dimensional objects such as a person’s hand, and transparencies to far end sites

Video Sources • Each video source button on the TRC4 remote references a video input located on the back
of your codec. On the TRC6 remote, the Share feature is used to show the images on the screen from the connected peripheral
• The video inputs on the back of your codec allow you to connect various peripheral devices to your video conference equipment for display to the far end site(s)
• The type of connection used depends on the peripheral device; the four types of cables used to connect peripheral devices to video conference equipment are shown below
VGA Cable • Has a 15–pin connector that carries analog video signals • Used to connect laptops and PCs to the videoconference equipment Display Port • Digital audio/video connection used to connect laptops to videoconference equipment
HDMI Cable • An HDMI cable connects digital audio/video devices to each other so that data can be
transmitted from one device to the other; used to connect peripherals.
There are multiple inputs and outputs located on the back of the codec that are used to connect peripheral devices.

Trouble shooting • If your laptop does not display on the screen after pressing the presentation button, press the
Window key + P key on your keyboard and select “Duplicate”. You may also need to change the resolution of your laptop to 1280x720.
• If you are sharing a presentation and it suddenly disappears, you may need to press Share presentation in call again as another site may have accidentally tapped Share on their end.
