Collaborative efforts continued with community partners to assure the provision of primary health care services. In addition new partnerships developed which included the partnership with PharmBlue to open a full service Pharmacy on our main campus in Green Valley. Another partnership was formed to allow United Community Health to open a satellite clinic on the Green Valley Hospital Campus. An agreement was reached with Arizona Pediatric Cardiology to provide clinical space to consult at our main campus in Green Valley which provides convenience to the parents and saves them a trip into Tucson. These partnerships enabled the organization to give back to programs, staff and the community through program development and community involvement. Greater staff resource availability and improved employment retention rates will increase continuity and quality of care. United Community Health Center is fortunate to have such a committed staff, Board members and community partners to assure that the growing health care needs of the communities served are met. This commitment will ensure that United Community Health Center becomes a Premier Federally Qualified Health Center dedicated to delivering the highest quality care possible. In 2015 United Community Health Center continued its mission to deliver quality care that is accessible, comprehensive, innovative, compassionate and culturally sensitive in an atmosphere of trust and respect to the communities it serves. Collaborative efforts continued with community partners to assure the provision of primary health care services. Improving the level of community wellness

REACH OUT AND READ

UCHC proudly continues to implement the Reach Out and Read Program which pro-motes early literacy during well child checks. Children ages 6months-6 years are provided age appropriate books by UCHC’s pediatric providers and parents are encouraged to read with their children. In 2015, 1000 books were given to chil-dren through UCHC's collaborative Reach Out and Read program.

DICTIONARY GIVEAWAY

UCHC strongly believes in the importance of early education. For the past 3 years, UCHC has partnered with The Dictionary Project in order to donate dictionaries to middle school children who may not be able to afford them. In 2015, UCHC do-nated 96 dictionaries to Robles Elemen-tary School from the Altar Valley School District in Three Points, AZ.

FIRST THINGS FIRST

First Things First (FTF) is a federal pro-gram that is one of the critical partners in creating a family-centered, comprehen-sive, collaborative and high-quality early childhood system that supports the devel-opment, health and early education of all Arizona’s children birth through age five. In November, 2014, UCHC became part-ners with FTF. Since then, UCHC has distributed FTF’s Arizona Parent Kit Bags as well as educational information regard-ing early education.

The National Committee for Quality Assurance (NCQA) announced that United Community Health Center of Southern Pima County has received NCQA Patient-Centered Medical Home (PCMH) Recognition for using evidence-based, patient-centered processes that focus on highly coordinated care and long‐term, participative relationships. The NCQA Patient-Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve pa-tients’ experience of care and reduce costs. Medical homes foster ongoing partnerships between patients and their personal clinicians, instead of approaching care as the sum of episodic office visits. Each patient’s care is overseen by clinician-led care teams that coor-dinate treatment across the health care system. Re-search shows that medical homes can lead to higher quality and lower costs, and can improve patient and provider reported experiences of care. “NCQA Patient-Centered Medical Home Recognition raises the bar in defining high-quality care by empha-sizing access, health information technology and coor-dinated care focused on patients,” said NCQA Presi-dent Margaret E. O’Kane. “Recognition show that Unit-ed Community Health Center has the tools, systems and resources to provide its patients with the right care, at the right time.” To earn recognition, which is valid for three years, United Community Health Center demonstrated the ability to meet the program’s key elements, embod-ying characteristics of the medical home. NCQA stand-ards aligned with the joint principles of the Patient Centered Medical Home established with the American College of Physicians, the American Academy of Fami-ly Physicians, the American Academy of Pediatrics and the American Osteopathic Association.

CHANGE IN EXECUTIVE MANAGEMENT PHARMBLUE PHARMACY AND UCHC PARTNERSHIP In 2015, United Community Health Center and PharmBlue formed a partnership in order to better serve the community and UCHC’s patients. In this collaboration, PharmBlue Pharmacy will open a new location inside of United Community Health Center’s Continental Family Medical Center, located on 1260 S Campbell Rd, Green Valley AZ. The new location will be a full service pharmacy that will be available to UCHC patients as well as to the community of Green Valley. Clint Sanders will be the onsite pharmacist, bringing with him over ten years of experience.

PharmBlue is a National Specialty Pharmacy with more than twelve years of history serving the needs of members. Their mission is to provide specialty pharmacy services to individuals with chronic condi-tions taking multiple medications, and the clinicians who serve them. They serve thousands of enrolled members throughout the country and fill all member pharmacy needs with a special focus on the needs of HIV/AIDS, Hepatitis C, and Behavioral Health Members. With this experience of service United Community Health Center is confident in PharmBlue’s ability to fill the pharmacy needs of all our patients.

2015 brought exciting new changes and additions to UCHC’s leadership. Rodolfo Jimenez, DO, became Chief Executive Officer. Dr. Jimenez believes in providing medical care to all those in need and has found that federally qualified community health centers do an excellent job at achieving this mission. He has over twenty years of experience providing patient care and supervision of provider teams at Community Health Centers. As CEO for United Community Health Center, Dr. Jimenez is looking forward to manag-ing the challenges, opportunities and growth that current health care reform will bring.

Dr. Jimenez is supported by a dedicated leadership team which includes Chief Financial Officer, Thomas Woodworth, Chief Operations Officer, Wen-dy Kibby, and Medical Director, Dr. Amy Rosenberg. Collectively, this team brings a diverse amount of experience in health care settings and leadership roles. The team is focused on good resource management, quality patient care and overall organizational growth and development. Their commitment, enthusiasm, experience and dedication will provide the drive and guidance needed to realize UCHC's vision.

UNITED COMMUNITY HEALTH CENTER REACHES OUT TO THE COMMUNITY

COMMUNITY OUTREACH EVENTS NATIONAL HEALTH CENTER WEEK Every year, UCHC makes community involvement a priority. It is thanks to UCHC’s community partners and residents that the organiza-tion is able to achieve its mission of promoting healthy lifestyles and improving the level of community wellness. UCHC staff attends numer-ous community events year round in each of its six geographic locations hosting 9 clinical sites throughout Southern Pima County to ensure and reinforce the bond between UCHC and the community. In 2015, UCHC attended twenty-two community events, ranging from health fairs, to fundraisers and parades. UCHC also attended over twenty school func-tions, varying from orientations, registrations and open houses, in order to extend the opportunity for parents to learn about the services for children that UCHC offers.

National Health Center Week provides an ideal opportunity for Health Centers to create public awareness of what they do and how they do it. For the 2015 National Health Center Week, United Commu-nity Health Center chose to showcase all of its health center locations by hosting Open Houses, inviting everyone in the community to meet providers, take tours of their facilities and have some face to face in-teraction with the staff after hours.

UNDISCOVERED TREASURES CRAFT FAIR United Community Health Center was proud to host its 2nd Annual Undis-covered Treasures Craft Fair fundraiser on February 22nd 2015. All proceeds went to benefiting UCHC’s Pediatric program and Dental Care for Kids, allow-ing them to buy vital equipment needed to serve children in some of Pima County’s rural communities.

The event took place at the Desert Diamond Casino and was free to the public. Thirty two vendors participated, displaying their wonderful creations. UCHC would like to thank all staff and attendees for helping to make this fundraiser a success. They extend a special thank you to the fair’s sponsors, which include: Desert Diamond Casino, West Press, KGVY Radio, CareMore, NextCare Urgent Care, United Healthcare and Wisdom Eldercare.

UCHC’S ANNUAL ALL STAFF MEETING At United Community Health Center we are always looking for ways to improve recruitment and retention throughout the organization. However, it is important to note that this process is not just about sustainability for the organization and the financial implications of employee turnover. Of course these things are very important and a key part of the consideration, but it really comes down to our staff. The success of this organization is highly dependent on the success and satisfaction of our staff. As we discover needed changes or improvements, or have new and exciting things to share, we have to have a good system to inform the staff. The organization has grown exponentially in its more than 30 years of existence and keeping the small company feel is an organizational goal, however, it is also a tall order to fill. One of the ways the management team has decided to handle this is through an annual “All-Staff” training. 2015 brought a lot of new faces and new discussions to the table for the staff. The event was held at Pima Community College’s Center for Performing Arts in Green Valley. The location allows the staff to join together in the large auditorium and meeting rooms for group discussion and also offers the availability to break off into smaller groups or departments for individu-alized trainings and discussion as needed. The training and development that occurs is vital to the continued forward progress of the organization but it is also a key component of the internal structure and stability. Fol-lowing the training, input from the employees is gathered to use in planning for future trainings and this part keeps communication open and ideas flowing. The All-Staff training team measures these successes and chal-lenges and strategizes ways to make each one better than the one before.

ELIGIBILITY AND ENROLLMENT

The 2015 Open Enrollment Period for both the expanded Medicaid pro-gram (AHCCCS) and the Health Insurance Marketplace took place from November 1st to January 31st. UCHC’s Enrollment and Outreach Specialists provide eligibility, enrollment, and patient advocacy services to the public at no cost in all of our six geographic locations. Throughout the Open Enroll-ment Period for 2015, UCHC’s Eligibility Specialists assisted Arizona resi-dents to enroll or renew a healthcare plan that fit their needs. They also provided enrollment fairs throughout the community that were free to the public. To date, UCHC has three Certified Application Counselors to assist the public with the ACA at no cost. FREE TRANSPORTATION SERVICES United Community Health Center offers free transportation services to UCHC patients in all service locations. Funded by DES, Pima County and CAA, patients have access to rides which are available by appointment, Monday - Friday, to clinic, labs and radiology appointments. UCHC provides ADA transport with wheel chair lift and has three transportation vans for patients’ use.
