F R O M K E L LYDear Friends, Donors, and Colleagues,
Once again I write this letter to you with a sense of wonder. I am amazed by the work accomplished, work that you made possible by your investment and belief in our mission. In 2013, because of you, we saw several pieces of our long-term vision come together. We finished construction of the Conway Health and Resource Center, which opened in early 2014. We put into motion a system-changing assessment and plan to house DC’s homeless families. We also launched a workforce development program to train entry level workers with critical customer service skills. At the last graduation, I met La’Kisha who was completing the 30-hour-course, is studying to be a nurse, has been housed through us for several years, has a child mentored by one of our volunteers and is a patient at the Conway Center, close to her home. I am thrilled by the depth of services that we now provide to families.
We are pleased that Community of Hope continues to grow and thrive, which you’ll read more of in the following pages. But what really matters is our effectiveness. We are keeping families stably housed, getting people jobs, and see-ing their health outcomes improve, beating local and national standards.
Thank you for your role in this vision. 2014 is a year of visioning and planning for the next five years and I look forward to working with you on achiev-ing those goals too.

Being CommunityMentoring really makes a difference.Garrett has been mentoring since January 2013 and feels that “The chance to positively influence someone at an early age is priceless. I think that a mentor-mentee relationship can go a long way in a child’s development.” He has put a lot of work into building a healthy and meaningful relationship with his mentee focusing on sport and academic goals. As Garrett says, “He is a great kid with a bright future, and it is a pleasure for me to be a part of that...he and I have a bond that will last a lifetime!”
260 volunteersprovided $473,527of service by mentoring youth, reading, translating, celebrating birthdays, planning events, coaching birthing moms, and so much more.

A “Radiant” Night of HopeOn a warm October night, 250 friends of Community of Hope filled the showroom at the Room and Board Company on 14th Street. They sipped Hope-tinis, relaxed to the jazz guitar of Dr. Salvatore Selvaggio, and were inspired by Ms. Cynthia Poole, a Community of Hope client and veteran who became homeless after a rollover car accident that nearly took her life.
“My life has been a true battle from the bot-tom; I’m still climbing to the top. I was at my lowest point of existence and sometimes won-dered about ways to make it all end. I felt like my prayers weren’t working and I lived many days without hope. I want to say thank you to Community of Hope for being the answer to my prayers and providing my family with more than just rent but a second chance to be pro-ductive in our own lives.”—Ms. Poole

Washington, DC has five times the rate of homeless persons per capita compared to the U.S. average. As of January 2014, the number of homeless families has grown by 25% over the previous years as the result of skyrocketing rental prices and limited long-term solutions. But in this environment, Community of Hope served 91 more families than in 2012.
Families recently housed through our Family Housing Solutions program, which was supported by the Freddie Mac Foundation and DC government, said that what they liked best about their new homes was that they were…
A S S E S S I N G F O R T H E R I G H T L E V E L O F S E R V I C E SIn 2013, along with our partner, Transitional Housing Corporation, we performed in-depth assessment for over 850 families who were housed at DC General Shelter and other emergency shelters. Our goal was to use an evidence-informed assessment tool to inform what the right level of services is for each family.
We found that families broke into three categories:• Manyfactorscontributingtohomelessnessand

Families set goals that include maintaining their housing, increasing their income, putting their fam-ily first and reaching self-sufficiency.

For over 30 years, we have worked to improve our clients’ health and ensured that the children in our families have homes to call their own. In 2013, we launched a new program to increase critical skills for the workforce.
Our Workforce Training program provides entry level workers, especially from Ward 8, with customer service skills. The 30-hour training class provides education and role-plays in stress management, con-flict resolution, and professional relationships.
In 2013 and early 2014, we graduated our first three classes with a total of 37 graduates. Over 50% of the graduates had secured employment already.

Improving HealthThrough Compassionate CareGood health is made easier by having access to a caring provider that you trust, located close to your home. Community of Hope provides primary medi-cal care in three of DC’s four quadrants.
In 2013 our compassionate care reached 7,100 patients in 31,156 visits—the highest ever. Here’s how we know we are effective:• 82%ofdiabeticpatientsseenatleasttwiceinthe
year had their most recent HbA1c less than or equal to 9% (versus 2012 national average of 70% and DC average of 61.69%).
• BreastfeedinginitiationrateforAfricanAmericanmothers at FHBC was 88%, versus rates around 60% for the past few years nationwide.
• Pre-termbirthratewas4%andlow-birthweightrate was 4.2% (versus 10% DC average).

Improving HealthBy Increasing AccessIn some areas of DC, the health outcomes nearly mirror those of impoverished nations. In our new neighborhood in Ward 8, we learned that rates of death from diabetes and heart disease are double the rate in other parts of the District, 40% of pregnant women in this area receive inadequate or no prenatal care, and 1 in 6 DC residents report they do not have a primary source for healthcare. So with careful planning, tenacity, and the support of many, many friends, we built and opened the Conway Health and Resource Center.

EVEN THE ART I S OF THE COMMUNI TYCommunity of Hope envisioned the Conway Health and Resource Center as a place not just for essential health care, but for healing. Thanks to Barbara and Lewis Shrensky’s generous support of our art initia-tive, we’ve purchased art from local DC and Ward 8 artists. Some artists were as young as fifteen years old, like Malek Malik whose Big Mouth collection lines the halls of the dental suite (see page 14). Ward 8 artist Sheila Crider’s Airborne mobile sets aspirations soaring in the entry way (see page 14). A place of healing that is of the community and for the com-munity.
R E A C T I O N S O F N E W PAT I E N T S “Vanessa Herndon, 59, wasn’t sure what to expect Thursday when she became one of the first patients to take advantage of the spanking new, four-story health center in one of the District’s poorest, most neglected neighborhoods.
Herndon was thrilled that the clinic was within walking distance of her apartment, but she has had many bad experiences with doctors and hospitals. This time, she came away impressed and grateful.
‘They don’t rush you in and out; they call to remind you of your appointment; they’re really concerned about people,’ said Herndon, who went in for a worri-some cough that turned out to be merely a cold.
Other patients stressed the same. John Car-rington, 45, who was there to sign up for health insurance under President Obama’s health-care law, said, ‘I’ve been to other hospitals and they look down on you, but here, everybody is friendly.’
Feeling respected is a novel experience for resi-dents of this part of the Bellevue neighborhood in the District’s southernmost corner.
That’s one of several reasons why the opening of the $26 million Conway Health and Resource Cen-ter, built and operated by the nonprofit group Com-munity of Hope, is a landmark event.”

The center features a total of 20 medical exam rooms, two procedure rooms, eleven dental operatories, meeting rooms and a large educational room. Already hundreds of patients have come for medical and dental services. By the end of 2014, hundreds more will have been seen in multiple services including ophthalmology screenings, behavioral health, and—due to a partnership with Breast Care for Washington—mammography services.
