Some hints:-Valid color code today is BLUE- Equipment is Power Pack

Pad eyes are were inspected and color coded as well as the slings: OK

But is this enough?

Results of certification are not valid!

The pad eyes on the top of the power pack frame are not the only load-bearing points!The frame is BOLTED to the lower beam of the skid, which holds all the weight of enclosed equipment!Not inspected bolts invalidate the whole certification!

Potential for Lifting Accident

When incomplete inspection was identified, the decision was taken to remove and inspect the bolts, and also the bottom threaded receptacle plate.

Result is obvious:-Two bolts out of 8 have been seized and broken- Bolts’ shank and top threads bear the signs of corrosion

Whose responsibility? It is shared task!

When inspectors come to your site, the service personnel and equipment owners tend to believe that those guys with MPI toolboxes know everything…

Indeed, most of inspectors know only one thing: how to perform the NDT by MPI or visual inspection.

But what they don’t know are:- Your equipment

- Your work environment and history- Your requirements for inspection

Insure that your operational personnel identify the critical components for inspection, prepare equipment, and give clear instructions to NDT Inspectors! Your additional reference for equipment inspection is API Recommended Practice 8B / ISO 13534:2000, mainly Categories III and IV
