    Task 1

    Agrochem Co Ltd., manufactures agro-chemicals. The company has five directors, a

    General Manager, a Maintenance Engineer, a Production Engineer, and a Safety Manager whilst

    the maintenance section consists of three supervisors, two electricians, five fitters, ten welders

    and twenty five other support staff.

    The maintenance staff carries out the preventive maintenance and the breakdown (corrective)

    maintenance.

    There are five chemical mixers in this company for chemical formulation process. Theproduction section staff informed that there was an unusual noise emission from agro chemical

    mixer number 3.

    The maintenance staff identified that the noise emission was from the rotating mixing shaft.

    The mixing shaft of the vessel is needed to be adjusted for alignment at the bottom of the shaft.

    Details of the chemical mixer number 3

    The size and shape- 12 ft diameter, 18 ft height vertical cylindrical shape closed vesselTwo man holes of 3x3 ft size on top of the chemical mixer 15 ft of the mixer is installed underground and 3 ft of the mixer is above the ground.The mixing shaft is driven by a three phase motor through belt drive fixed on top of the

    mixer.

    Two pipe lines for feeding chemicals and one pipe line for feeding hot water are fixed atthe top of the chemical mixer.

    Mixed chemicals are transferred from a chemical pipe line at the bottom.

    The work permit systems shall be applied to normal repair of the chemical mixer. Residue

    of chemicals deposited at the bottom and wall of the vessel is called as sludge. Sludge developed

    inside the chemical mixer needs to be removed for proper alignment of the mixing shaft. It is

    expected to carry out rectification by two persons entering into the mixer. There are group of six

    other persons and a supervisor for the repair of the chemical mixer.

    The residue chemicals inside the chemical mixer are flammable and sludge inside the

    chemical mixer can cause lack of oxygen in the environment. The repair includes hot work such

    as welding and cutting. The repair might take more than seven hours depending on the condition.

    The maintenance staffs want to start work at 7.30 am on Saturday morning.

    1.1. Evaluate a range of permit-to-work systems needed and identify the types of isolation

    requirements for the given application. (P1.1)

    ABC Ltd., is an engineering consultancy firm. The company has five directors, a General

    Manager, fifteen engineers and ten technicians. The firm is giving consultancy on designing of

    manufacturing plants, purchasing of machinery, and maintenance of machinery. The company

    consults on safety matters related to manufacturing and service facilities. You are appointed as a

    Safety Engineer in this company. The General Manager of the company advised you to prepare

    a report for the following task.

    2.1 (a) Identify ten industrial work areas where current safety and health legislations would

    apply (P 2.1)

    (b) Describe the role of the Factory Inspecting Engineers according to the current safety

    and health legislation. (P 2.1)

    Part 2

    XYZ Co Ltd., is manufacturing glass items. The company has six directors, a General Manager,
