And the QHSE objectives were found clearly identified at relevant level and functions with approval form top management

The scope of QHSE management system were found clearly identified, and all aspects are determined and all processes and equipment related to QHSE scope are determined.

4.3 Determining the scope of the QHSE Management System

(ISO 9001 + ISO 14001 + BS OHSAS 18001)

Requirements Objective evidences*

The organization shall determine the boundaries and applicability of the QHSE Management System to establish its scope. When determining this scope, the organization shall consider: a) the external and internal issues referred to in 4.1; b) the requirements of relevant interested parties referred to in 4.2; c) The products and services of the organization. The organization shall apply all the requirements of this International Standard if they are applicable within the determined scope of its QHSE Management System. The scope of the organization’s QHSE Management System shall be available and be maintained as documented information. The scope shall state the types of products and services covered, and provide justification for any requirement of this International Standard that the organization determines is not applicable to the scope of its QHSE Management System. Conformity to this International Standard may only be claimed if the requirements determined as not being applicable do not affect the organization’s ability or responsibility to ensure the conformity of its products and services and the enhancement of customer satisfaction.

United Engineering has identified its QHSE scope including the boundaries and applicability of QHSE. The MS Manual is developed conforming to the standard requirements of ISO 9001:2008, ISO 14001:2004 & OHSAS 18001:2007 with a documented QHSE Policy, scope of QHSE, justification of exclusions (8.3), interaction of the processes as well as referencing of the procedures developed for the implementation of QHSE United Engineering was found considers the internal and external parties, the requirements of interested parties, the products and services during defining its QHSE scope. The scope of United Engineering was found available and maintained as a documented information. Clause 8.3, Design & Development is not applicable to United Engineering QHSE due to its nature of business as a service provider with no special process required. The scope of United Engineering was found include the types of products and services covered and the justification for any requirement of ISO 9001:2015 / 14001:2015 & OHSAS 18001 are determined not applicable to the scope of its QHSE Management System.

Discussion with top management, shows that they are fully committed and supports the QHSE management System implementation and maintenance. The QHSE objective (operational indicators) and quality policy had been issued and signed by the GM and are compatible with the context and strategic direction of the organization; The QHSE policy is approved by the MD The policy is reviewed and updated on 25th December 2015 Necessary infrastructure required to comply with the QHSE requirements has been provided. The monitoring and measurements resources were found clearly defined. An annual production plan is prepared and carried out. Evaluation of the audit

Introduction

TÜV INTERCERT has been appointed by United Engineering to carry out an audit.

The aim was to verify whether the requirements for a QHSE Management System (IMS) stated in the relevant IMS Standard are met and whether the TÜV INTERCERT certificate can be issued or, respectively, whether the validity of the TÜV INTERCERT certificate continues to be valid. The audit programme include a two-stage initial audit, surveillance audits in the first and second years, and a recertification audit in the third year prior to expiration of certification. The three-year certification cycle begins with the certification or recertification decision. The determination of the audit programme and any subsequent adjustments consider the size of the client organization, the scope and complexity of its management system, products and processes as well as demonstrated level of management system effectiveness and the results of any previous audits. Where a certification body is taking account of certification or other audits already granted to the client, it shall collect sufficient, verifiable information to justify and record any adjustments to the audit programme.

This audit is based on documented requirements of TÜV INTERCERT certification body, drawn up in accordance with the relevant guidance provided in ISO/IEC 17021-1 and ISO 19011.

During the audit the application of the IMS was demonstrated and confirmed with relating records for some parts of the company. It was checked whether the working processes proved conformity with the IMS Standard as well as the descriptions laid down in the Quality manual and respectively in the relevant working and inspection instructions. This happened in a sampling way by questioning and by inspecting the corresponding documents.

Executive Summary

The QHSE Management System has been reviewed last 2/29/2016. The documentation includes the policy, objectives, system manual, documented information, actions to address risks and opportunities, Hazards, Aspect and Impact, Operational controls and related forms and records. Scope of the organization is “Fabrication & Erection of Steel Structures & Pipes, Scaffolding Works, Blasting & Painting, Trading & Agency Services” Following functions have been assessed during the certification audit MR Functions, HR/Training, administration, legal affairs, customer service & support department, risk management & assessment, HSE, organization and its context and leadership. Overall compliance of the standard was found to be satisfactory. The management was found aware about the effective implementation and maintenance of the IMS. The Staff of the organization has shown effective collaboration and they have suitable knowledge of the IMS which enables them to implement the system efficiently. All processes sequence & interaction of processes were defined in the QHSE Management System. During the IMS certification audit of UNITED ENGINEERING 5 minor nonconformities have been observed. Therefore, we recommend issuance of the certificate from the TUV InterCert and designate the next audit date with the customer which will be in March 2017

Management commitment, objectives and targets

Management review meeting are conducted yearly. Minutes of the last meeting dated 2/29/2016 were reviewed. There is one shift and adequate supervisory support is provided across the shift through. Internal audits are conducted once a year at least. Last internal audit was conducted dated 02/28/2016 Objectives are set on an annual basis and reviewed for follow-up. The Management Review, which has been conducted on 2/29/2016 includes evaluation of: - the status of actions from previous management reviews; - changes in external and internal issues that are relevant to the QHSE Management System; - information on the performance and effectiveness of the QHSE Management System, including trends in: - customer satisfaction and feedback from relevant interested parties; - the extent to which quality objectives have been met; - process performance and conformity of products and services; - nonconformities and corrective actions; - monitoring and measurement results; - audit results; - the performance of external providers; - the adequacy of resources; - opportunities for improvement - Minutes of the meeting were available and were comprehensive.

Internal audit, management review, and continual improvement processes

Audits are carried out enabling the whole system to be covered in a year. An audit plan is in place. Internal audits are planned appropriately. CA are followed up via using of “CA Follow up Form. Corrective actions are documented on the CA Request Form which includes reference to the person responsible for the corrective action and the target date for completion. Outstanding corrective actions are reviewed at management meetings to ensure they are closed in timely manner as possible.

System requirements and interrelationships, functions, processes, areas audited

UNITED ENGINEERING has planned its processes and developed relevant work procedure that covers the entire process from receiving customer inquiry up to delivery of the products and services. The company defines the methods to be used to ensure that the quality of the products and services is maintained. In purchasing process, all of purchasing information is defined as per need items that organization defined in service realization. Training needs analysis were found carried out by the departmental heads and subsequently the training plan for the year 2016 has been created. Planned trainings were conducted. Quality policy is used as to provide the framework for the development of QHSE Objectives at relevant levels and functions with the inputs from the departmental heads and the Top Management. Follow up on the status of the objectives is carried out regularly and final review is done during the management review meeting. All documented information determined by the company have been developed and effectively implemented. All documented information were found to be controlled and as per set procedures for the control of documented information. Documents are prepared by the MR who also review and then approved by the GM prior to issue. It’s the responsibility of the MR to ensure that the updated relevant documents are

provided to the relevant departments while the responsibility of implementing and maintaining the protocols defined in departmental procedure lies with the departmental heads. The software records are backed up monthly by external server. QHSE MR is responsible to monitor performance of the IMS. In case of non-conformance raised at any stage, NC report is raised and directed to the relevant department for the root cause analysis, correction and development of the corrective actions. Data Analysis is carried out annually for the following functions. - Number of corrective actions - Internal audit results- number of NCs, improvements - Conformity to service requirements - Process and service characteristics - Suppliers evaluation. -Customer Feedback -Customer Satisfaction

The results of the data analysis are reviewed in the management review meeting to gauge the current effectiveness level of the operation and IMS and develop further action plans for improvement.

The auditing is based on a sampling process of the available information and consequently there will always be an element of uncertainty in the audit evidences, which may be reflected in the audit findings. Those relying or acting upon the audit result and conclusion should be aware of this uncertainty.

The statements of the audit team, however, do not release the organization from its responsibility to ensure continuously meeting the requirements and observing the Quality standard.

The recommendation from the audit team to TÜV INTERCERT will be subject to an independent review, before any final decision is made concerning the awarding or maintenance of certification.

The issuing/maintenance of the certification, if there were non conformities is linked to the corrective actions sent to TÜV INTERCERT. These corrective actions will be checked by TÜV INTERCERT.

TÜV INTERCERT could modify the terms regarding the certification scope in order to grant its conformity to accreditation registers disposals.
