CLINICAL ACTIVITIES Clinical Activities Summary

The focus of my clinical administrative activities over the past 2 years has been the reorganization of the Inpatient Service with major changes in medical staff documentation of clinical care to meet more stringent regulatory requirements, dramatically increased clinical programming on the units, and the development and implementation of a recovery model-based approach to patient care. We have worked closely with SF DPH Compliance staff, DPH Transitions leadership and ZSFG Quality Management to insure high quality care at the appropriate level and maximum reimbursement for services.

Within the SF DPH, the SF Office of the Conservator, and the Superior Court of SF, I have continued to develop and implement the Community Independence Pilot Project (CIPP), an innovative program designed to allow patients to receive involuntary treatment in the community rather than in a locked treatment setting. The CIPP, which operates in a manner similar to a psychiatric advanced directive, seeks to address the major issue of medication non-adherence which limits psychiatric treatment for so many individuals with serious mental illness. In a related approach to this issue of non-participation in treatment, our Citywide Case Management Program was asked by SF DPH to provide case management services for the inaugural Assisted Outpatient Treatment (“Laura’s Law”) service initiated by SF last year

UNIVERSITY AND PUBLIC SERVICE SERVICE ACTIVITIES SUMMARY During the past two years my service activity has continued to center on efforts within the San Francisco Department of Public Health to improve the system of care for patients with serious, disabling psychiatric illness in a fiscal environment that has resulted in major funding cuts to the SFGH Psychiatry Department. These efforts have included memberships in key multi-agency committees (e.g. Community Behavioral Health [CBHS] Medical Directors Committee) and task forces, as well as building individual relationships with members of major divisions and agencies providing and managing care for our SFGH psychiatric patients (e.g. the DPH Placement Team, CBHS Leadership) My efforts within SFGH have continued to focus on improving quality of care across the hospital via active membership in the SFGH Performance Improvement & Patient Safety Committee, the SFGH Risk Management Committee and the SFGH Fair & Just Culture Committee. I have made multiple presentations of the Just Culture program to large groups of SFGH staff. Over the past two years I have provided periods of coverage as Acting Medical Director of SFGH Risk Management in the absence of the Medical Director, helping to manage the SFGH response to critical incidents hospital-wide.

Commitment to diversity and equal opportunity for patients needing treatment, as well as faculty, staff, and trainees has been a major focus of my work as a UCSF faculty member in Psychiatry at SFGH. My initial role was as unit chief of the AIDS Focus Inpatient unit. Subsequently, with my leadership our unit developed and added the Lesbian, Gay, Bisexual, and Transgender (LGBT) Focus Inpatient Team, one of the few such teams in the nation. In recent years I have been responsible for restructuring and maintaining the Inpatient Focus Teams (Asian, African-American, Women's, Latino, LGBT) during a period of budget cuts and bed closure. My commitment to diversity has led to a continuing strong track record of recruiting and retaining an extremely diverse faculty for the Inpatient, Psychiatric Emergency Services (PES), and Citywide Case Management program in our department.

TEACHING

TEACHING SUMMARY

Teaching efforts over the past two years have also continued to focus on orienting and maximizing the educational experience of the PGY-1 and PGY-2 residents on the on the inpatient service. My weekly meetings with the PGY-1 and PGY-2 residents have centered on understanding clinical care of seriously mentally ill individuals in the public mental health system, and on developmental adaptations to the physician role in a multidisciplinary inpatient setting. This has included assisting the residents in adapting to changes in clinical services necessitated by budget imperatives, and understanding the importance of treating patients at the lowest appropriate level of care. In addition, major teaching efforts have been made to increase the residents' sense of safety in an acute inpatient setting.

MENTORING

MENTORING SUMMARY

Mentoring activities have focused on regular meetings with SFGH clinician teacher faculty in the Psychiatry Department. I have provided encouragement and support for early and mid-career faculty, helping them to build on clinical and teaching strengths and directing them to opportunities for growth and development within our system. Examples include mentoring Dr. Fumi Mitsuishi in her initial Medical Director role at Citywide Case Management. We have worked together to manage challenging multidisciplinary personnel issues, and to successfully recruit and retain an increasingly diverse medical staff. Over the past six years I have worked closely each year with the UCSF Psychiatry Chief Resident at SFGH. Another example is the mentoring of Dr. Emily Lee as she has successfully managed the leadership challenge of reorganizing the Inpatient Service with completely new clinical programming, significant changes in multidisciplinary supervision structure, and multiple iterations of a new documentation system. Over the past six years I have continued to work closely each year with the UCSF Psychiatry Chief

RESEARCH AND CREATIVE ACTIVITIES

RESEARCH SUMMARY

Research activities have focused on actively supporting research efforts in the ZSFG Psychiatry Department, including increasing attention to primary medical care needs in our Citywide Case Management Program. An additional area of focus has been quality improvement studies on the medical record documentation of inpatient psychiatry care at ZSFG. Areas of study for the future include patient flow dynamics within the public mental health system with an emphasis on emergency and inpatient services.
