Application Instructions: UCO Speech-Language Pathology Graduate Program ******************PLEASE NOTE**************************
Reading and following these instructions AND the application materials is IMPORTANT. When considering
applicants for a graduate professional program, the applicant’s ability to follow instructions is an IMPORTANT
factor. Application packets that are incomplete or do not comply with requirements in these instructions AND those
in the application materials will NOT be considered.
1. Application is a two-part process. You must apply to the SLP Program AND to the Jackson College of Graduate Studies.
2. SLP Program Application Packets: The SLP program application items (application, transcripts, and
recommendations) MUST be submitted simultaneously in the SAME envelope – otherwise the application will NOT
be considered. It is STRONGLY recommended that you submit your PACKET well before the deadline. This will
allow sending you any notices regarding the status of your application, dates for admissions interviews, and other
information relevant to the admissions process in a timely manner.
3. Prerequisites: Please note that to be considered for admission to the Speech-Language Pathology Graduate Program,
you must have completed or be completing the prerequisite courses (or their equivalents) listed on Page 2 of the
application form. Course descriptions for UCO prerequisite courses are included on page 3 of the application. The
applicant should compare his or her coursework to the course descriptions provided. If a course title does not clearly
indicate that it is an equivalent course, the applicant should include a course description or syllabus.
4. Please Note: The starting semester for enrollment in the graduate program is Summer.
5. Recommendations: A copy of the Speech-Language Pathology Interpersonal Skills & Academic Abilities
Recommendation form should be given to each of THREE university faculty/instructors who have taught you in
college level courses. At least two of the three recommendations must come from faculty members who have taught
you in Speech-Language Pathology/Audiology courses. The third form may come from an instructor in Speech-
Language Pathology/Audiology or it may come from a college level instructor you had in another subject area - e.g.,
psychology, education, statistics etc. Instructors providing recommendations should return the forms directly to the
student. Recommendations that are to be considered as confidential should be placed in sealed envelopes and signed
across the sealed flap.
Confidentiality: Students should read and sign the confidential waiver section on the form and indicate if the
recommendation is to be confidential – meaning the student will not be permitted to view that recommendation
form. Making recommendations confidential is optional. However, you should consider carefully whether they will be
evaluated as objective if instructors have not been assured that their evaluation will remain confidential.
6. Graduate Assistantships (optional): (Separate application and complete instructions are included on a separate page.)
If you will be applying to serve as a Graduate Assistant, it would be appropriate to indicate this to the instructors
providing your recommendations. Please note: Applicants for a Graduate Assistant position MUST attend admissions
interviews or be interviewed separately to be considered for a GA position.
7. Official Transcripts: An official transcript from each university you have attended is required. These should be
requested from each university and sent to you for inclusion in your application packet. Transcripts should be requested
ONLY after grades for the most recently completed semester prior to UCO admissions have been posted so that
grades for ALL COMPLETED COURSES can be evaluated during admissions.
8. Application folders will be considered CLOSED as of February 1. In order for an application to be considered, ALL
items must be received in the same envelope by February 1 or postmarked no later than January 29. Your graduate
SLP program application packet should be delivered or mailed to:
SLP Graduate Admissions
University of Central Oklahoma
100 N. University Drive, Box 80
Edmond, OK 73034
9. Application to the Graduate College: You must complete the Jackson College of Graduate Studies application and
submit SEPARATE items required for admission to the Jackson College of Graduate Studies. Being admitted to the
Graduate College is required, but does not guarantee admission into the Speech-Language Pathology program. The
application may be completed online. Contact the JCGS for more information (405-974-3341).
Graduate Program Admission Criteria: The following are minimal criteria to be considered for admission: 1) A minimum
GPA of 3.00 in all coursework or in the last 60 hours. Although the GRE is not required for admission, a total of 1000 or
greater on the two highest areas of the GRE may be used to admit a student on "probationary" status. 2) A minimum GPA
of 3.0 in all Speech-Language Pathology/Audiology coursework. 3) A minimum average rating of 3.0 on the three
Recommendation forms.

ASHA Standards The Council for Clinical Certification in Audiology and Speech-Language Pathology (CFCC) is the council that carefully writes the rules and regulations for certification in audiology and speech-language pathology. The CFCC has written the Standards for Certification which outlines exactly what is needed to receive the Certificate of Clinical Competence (CCC). These requirements must be met by all CCC applicants, whether educated in the United States or internationally. The Certification Department at ASHA ensures that all standards are equally applied to, and met by, all applicants. All courses and classed listed in the Standard IV-A through IV-C must be completed (passed) at the college level. They must appear on your undergraduate or graduate transcripts. Classes taken at the high school level are not eligible to meet this requirement, with the exception of advanced placement (AP) courses that appear for credit on your college/university transcript.
Below is a list of the required college/university level courses that are required by ASHA. Please write the course number, course name, and semester you completed the course with a passing grade. Example: BIO 1114 General Biology FA 2015. If you have not yet taken one of the courses please be aware that you must complete the missing course during your graduate level studies. Below the table is a description of the required ASHA courses.
