II. TEACHING ACTIVITIES:
Teaching evaluations and other evidence of quality teaching must be provided and should accurately
summarize ALL of your teaching activities. The absence of teaching evaluations may adversely affect your
application for Promotion and/or Tenure.

Philosophy of Teaching:
Briefly describe your philosophy of teaching and how you apply it to different types of learners, e.g. medical
students, graduate students, residents, fellows.
I believe that educating future physicians and surgeons is both an obligation and a privilege. I have had
many mentors and educators in my experience who have shaped my career. I feel it is my responsibility to
pay that debt forward and educate the next generation. I believe that there are multiple ways to
accomplish this goal. Teaching occurs all the time: from the scrub sink to the wards to the classroom.

3. Educational Leadership:
Describe administrative responsibility for courses and other leadership activities, including formal
mentoring of junior faculty. Please list faculty members mentored and your role as mentor- letters from
mentorees may be included.
III. SERVICE ACTIVITIES:
1. Professional Service (Patient Care):
As an academic trauma, surgical critical care and acute care surgeon, I work within my division to provide
24-hour coverage every day for patients who are acutely injured, are critically ill or have an acute surgical
problem. On average, my service weeks consist of approximately 60-110 hours per week including one to
two in-house service calls for 24+ hour periods during the service week and 4 14-hour overnight shifts
every 7 weeks. Daily responsibilities include rounding on a service of 20 or more patients, providing
surgical consultation services (roughly 35-30 consults per week), providing semi-elective and emergent
surgical procedures (roughly 5-10 cases per week), management of critically ill trauma and surgery
patients, and supervising trauma activations. In addition, on ACS service weeks, 2 half days are spent
providing outpatient clinic coverage for follow up visits and continuing care for acute surgical patients.
There are several components involved in the care of these complex patients which include: evaluation and
management of trauma patients (those injured through various mechanisms - most commonly falls,
motor/motorcycle collisions, and interpersonal violence), evaluation and management of emergency
general surgery patients both inpatient and in the emergency department (with conditions including but
not limited to: acute pancreatitis, bowel obstructions, bowel perforations, acute cholecystitis, acute
appendicitis, skin and soft tissue infections), and surgical critical care services in the surgical ICU
(management of patients with organ failure in one or more organ system). We are contracted to maintain
productivity levels consistent with the MGMA median, which for this year is 6592 wRVU. My quarterly
wRVU productivity has been at or above the MGMA median for the last calendar year.
2. Academic Service:
In Academic Service, your contribution to the academic community should be clearly documented. Names
and dates of committees, task forces, or working groups should be provided. A brief description of the
significance of the group and explanation your role should be provided.

IV. RESEARCH AND SCHOLARLY ACTIVITIES:
Opening Statement:
Research is an important part of being an academic surgeon. As a clinically active surgeon, I believe that
my greatest impact can be made doing clinical projects and outcomes based research.
