If you’re uninsured and you don’t have a lot of money, you might overlook an ache or pain here and there. You might even try to ignore that seizure you had a couple weeks ago because seeing a doctor is so expensive, and that seizure was probably just a fluke anyway. With a three-year-old daughter and an eight-months-pregnant wife depending on you to be working, you can’t exactly afford to be sick.
But Carlos was very sick and getting worse. A year ago, he was finally rushed to the emergency room and admitted to the ICU where he spent weeks in a coma. While in the hospital, Carlos suffered three heart attacks and underwent procedures to address long-unchecked problems with his pancreas, kidneys, liver and lungs. Emerging from the hospital six weeks later, Carlos returned home with tubes draining an infection from his body and several diagnoses of serious chronic illnesses. Two days later, his wife gave birth to their new son.
Because Arlington Free Clinic automatically takes in eligible referrals from Virginia Hospital Center, Carlos was able to come directly into the care of a medical home prepared to support him throughout his recovery. Without the Clinic, he likely would have been back in the ER within days. Because his condition was so fragile, Carlos was initially seen at the Clinic every other day. In all, Carlos has had over 45 appointments and undergone 6 procedures through Arlington Free Clinic and its extensive network of volunteers and community medical partners. Now just over a year after his initial crisis, he’s stable and gaining strength, though still unable to work. Carlos has a long road ahead, but because of Arlington Free Clinic, he and his family will be facing the future together.

In January and throughout calendar year 2014, we will celebrate Arlington Free Clinic’s 20th Anniversary of providing high quality medical care to those in need. In twenty years of service to the community, we have made a difference in thousands of patients’ lives. Before the Clinic was founded in 1994, thousands of low-income, uninsured Arlington residents had no access to regular, dependable, affordable medical services. For many, getting sick meant choosing between exorbitant doctor bills or ignoring the problem and just hoping for the best. From the beginning, the Clinic offered extraordinary opportunities. For low income residents, AFC was an opportunity to obtain high quality medical care. For volunteers, donors, medical partners, businesses and civic groups, AFC was an opportunity to make a significant difference in the lives of thousands of people who live and work in our community. Along the way, we have significantly expanded the number of patients we see and the scope of care we provide annually. For the past two decades, we have worked hard, worked smart, and have earned the trust of more than 12,000 patients in addition to the confidence of countless volunteers, donors and community partners. We remain focused on the future and intent on continuing our tradition and mission of providing top quality medical care for the uninsured. In 2014, we will also celebrate the last 20 years. We will keep you informed of opportunities to join us throughout the year as we both honor this milestone and look ahead to the future.

Although the medical landscape seems to be constantly shifting, we continue to pursue our goals of safe, effective, timely, efficient and patient-centered care at Arlington Free Clinic. The term patient-centered care is thrown around a lot in the health care system these days. The Institute of Medicine (IOM) defines patient-centered care as: “Providing care that is respectful of and responsive to individual patient preferences, needs, and values, and ensuring that patient values guide all clinical decisions.” This type of care must necessarily take into account the patient’s social and cultural contexts as well. As you can imagine, the cultural backgrounds of our patients are diverse. One significant improvement in this patient-centered focus has been our adoption of a telephone interpreter service in the Clinic which has improved our ability to communicate effectively with patients who are not fluent in English.
In addition to improving our ability to communicate with our patients, our nursing staff has been developing individual care plans for many of our patients. These care plans are written assessments of the needs of each patient, and they are developed with the patient’s active input. Maura Constance, one of our nurse practitioners, is in full swing with a program designed to target very poorly controlled diabetics and she is seeing some significant improvement in this area. Maura spends a lot of time getting to know her patients in order to determine their individual preferences and obstacles to health.
Lastly, I wanted to use this opportunity to recognize the tremendous nursing staff at Arlington Free Clinic: Sheila Ryan, Martha Ware, Jyl Pomeroy, Mary Anne Taylor, Carol Marsh and Surekha Cohen. I feel so fortunate to be able to work with this group of nurses. They are the strongest group that I have ever worked with, anywhere.

Arlington Free Clinic is grateful for the extraordinary support of community partners, volunteers and donors. Working together, our major achievements of FY13 include the following:
• Numbers that Count: AFC provided a total of 9,002 medical visits for 1,682 individual patients. All patients are uninsured and have income at or below 200% of the Federal Poverty Level which is $46,000 for a family of four.
• New Patients: With our ongoing commitment to accept new patients, we brought in 438 Arlington residents who previously did not have regular access to medical care. Of these, almost 39% came to us by referral from in-patient care at Virginia Hospital Center and from community shelters.
• Volunteers Making a Difference: Volunteers have always been integrated into all aspects of patient care and clinic operations. During the year, 560 volunteers provided over 19,247 hours of service. The value of volunteer hours is estimated to be $1,092,724. Physicians, nurses, interpreters, financial advisors, fundraisers—all contribute to AFC’s ongoing success.
•Women’s Breast Health: We are grateful to Susan G. Komen for the Cure for awarding AFC a generous two-year grant to help support our important breast health services. The focus of the grant is enhancing continuity of care for low income Arlington women. With support from Komen and other funders and the remarkable commitment of volunteers and our community network of medical providers, our patients have access to high quality services throughout the continuum of care: from screening mammography to breast cancer treatment and survivorship.
• Commitment to Excellence: We are proud to announce that Arlington Free Clinic has just been awarded our fourth consecutive 4-star rating from Charity Navigator, the leading charity evaluator in America! Receiving four
out of a possible four stars indicates that AFC adheres to good governance and consistently executes its mission in a fiscally responsible way. Only 6% of the over 6,500 charities rated by Charity Navigator have received at least 4 consecutive 4-star evaluations, indicating that Arlington Free Clinic outperforms most other charities in America.
•All Stars: Fantastic AFC volunteers getting well deserved recognition for their outstanding community service:
• Joan Ritter – Toyota Tribute to Working Women: Dr. Ritter, AFC’s Medical Director, was one of seven women who received last year’s Toyota Tribute to Working Women Award for having made a dramatic difference in the community.
• David Lee – Wakefield High School Hall of Fame: Wakefield graduate and long-time AFC volunteer and former board chair, Dr. Lee was one of the Clinic’s first volunteer specialty physicians. Over the years his medical practice, Dominion Anesthesia, has provided pro bono anesthesia services for hundreds of AFC patients who require surgery.
• Improving Health with Disease Prevention: During the past year, patients received over 1,724 inoculations for
diseases including flu, hepatitis B, pneumonia, tetanus and shingles. The number of inoculations given to AFC patients last year increased by 36% over the previous year.

Arlington Free Clinic is indebted to the following medical practices that so generously provided a wide variety of specialty medical services in their offices for hundreds of Clinic patients.
For a stretch of over ten years, Joyce Kaiser would show up like clockwork for her volunteer work in AFC’s pharmacy. Monday afternoons were the time that she dedicated to working along with Clinic staff to provide services for AFC patients. She knew that her volunteer work distributing medications required attention to detail and careful double checking. Her position also provided the opportunity to interact with patients – which she said is what made her work so worthwhile.
Although still active at the respectable age of 95, Joyce has retired from volunteering. But that doesn’t mean that she’s retired from supporting the Clinic.
We were delighted to recently learn that Joyce has included AFC in her will. She says this decision was made as a way to give back to the community in honor of the assistance she received as a first generation American. Helped by people who were part of her life in the U.S., Joyce completed her education and then went on to a successful career that included being the Appointments Secretary to the Secretary of Agriculture during the Eisenhower administration. During her long life, Joyce has learned to make decisions carefully. She says that she knows her legacy gift will be used wisely and will help ensure the future of Arlington Free Clinic. We are grateful for Joyce’s generosity and her commitment to our essential work in the community.
