Introducing medspace.CT™

medspace.CT™ is a virtual X-ray computed tomography room complete with a realistic and fully functional CT machine, contrast injector and patient. Students have different viewing options to allow for true immersion and work flow process and can interact with all pieces of equipment via drop down menus or short cut keys. Key features include:

Interaction with all pieces of equipment via drop down menus or short cut keys

The ability to alter camera perspectives, such as: viewing as the radiographer; from either side

of the gantry; or the control area for exposure.

Extra features include skeletal view of patient to view anatomical landmarks.

Full student immersion in the “process” of performing a medical imaging examination.

Intravenous contrast loading and timing to simulate scan start times.

Licensees may upload their own DICOM data sets to customise the resultant images.

All default CT images are non-contrast, bone window Visible Human data provided under

license to medspaceVR by the United States National Library of Medicine, Department of Health

and Human Services on the 9th of August 2014.

Safety Warning Viewing of 3D Virtual software can make users feel unwell. If any of the following is experienced please

inform your educator and temporarily cease using the software. The following symptoms may occur

individually or in combination:

general discomfort

dizziness

difficulty focusing

vertigo

Headache

Fatigue

nausea

General Instructions

Click on a piece of equipment or patient to activate the relevant control panel.

Alternatively, select from the drop down menus.

Click outside of the panel to close it.

The medspace.CT™ Interface

To start medspace.VR™ go to the Windows start menu and navigate to the medspace.VR™ menu. Click on the medspace.VR™ link, with your Left Mouse Button (LMB). Alternatively, you can double click the

medspace.VR™ desktop icon with your LMB

Note: Load time will depend on the hardware configuration you are using. Refer to the “Administrators’ Manual”

for recommended hardware configurations.

Upon loading you will be presented with the Module selection screen. LMB click on the ‘CT’ button

When the medspace.CT™ module loads you will be presented with the 3D working interface.

The medspace.CT™ 3D Workspace Interface.

The medspace.CT™ interface is composed of two distinct areas. These are:

I. The ribbon bar File menus

II. The 3D workspace

The menus allow access to common functions such as ‘navigation’ and ‘help’ as well as detailed access

and control of all aspects of the software. Users can also choose to access machine and patient

functions through the file menus or directly through the 3D interface.

The 3D work interface allows for direct manipulation of the CT machine, the operator console and

patient head positioning. Users can move independently throughout the environment whilst interacting

with equipment and patient.

The Menus The menus consist of six independent headings. Each menu heading contains one or more drop-down sublevels. The menu items and their drop-downs are detailed below.

The 'File' menu gives users access to commonly used commands and contains two items.

The medspace.CT™ Workspace The 3D Work Space is where users conduct procedures. All aspects of x-ray acquisition process are

carried out within this 3D workspace. Users navigate within the space and interact with equipment.

The main medspace.CT™ 3D workspace.

The medspace.CT™ work environment consists of:

A Computed Tomography machine with associated workstation and image display system

A table

A Contrast Injector

An Operator Console

A Contrast Injector Console

Clicking with the LMB (left mouse button) on any of these above items will display their associated

interactive controls. Users can navigate freely within the environment whilst selecting items and the

patient for manipulation.

Refer to the ‘Navigation’ section of this manual for instructions on moving within the environment.

Refer to the ‘Workflow’ section of this manual for instructions on equipment controls and interactivity.

Keyboard Functions and Navigating There are a number of methods for interacting and moving around inside the medspace.VR™ 3D environment.

A combination of keystrokes and mouse buttons should be used to navigate.

Keyboard functions (Directional keys) Both the arrow keys and the W,S,A,D keys can be used as directional keys to move around the environment.

Arrow keys allow the following movement: Forward (Up arrow), Backwards (Down arrow), Pan Left (Left arrow) and Pan Right (Right Arrow)

W,S,A,D keys allow the following movement: Forward (W Key), Backwards (S Key), Pan Left (A Key) and Pan Right (D Key)

Mouse Functions (Selecting objects and looking around)

The mouse buttons serve various functions.

Left Mouse Button (LMB) – This button is the action button. You can use it to select objects in the exhibition and complete input actions such as check boxes.

Right Mouse Button (RMB) – This button is used to rotate about the environment. Hold the RMB down and then move your mouse sideways to rotate about. Use this in combination with the direction keys to move freely around the environment. For example, you can simply use the forward and backward directional keys to move in those directions while holding down the RMB to steer. This is a very effective and efficient way to navigate the 3D environment.

You can travel faster through the 3D environment by holding down the 'SHIFT' key while moving.

Moving and Turning Use either the arrow keys or the W,S,A,D keys to move forward, backwards or pan sideways. Use the RMB in combination with the directional keys to turn corners and look around. The images below demonstrate moving forward in the 3D environment while using the mouse to steer.

Workflow The following section describes the tools and procedures used to perform a CT scan.

The first section is the basics of how to bring up each tool and what it looks like.

Accessing equipment interactively within the 3D workspace Clicking the equipment in the 3D workspace with the LMB or using the shortcut keys will display their

associated controls. The following section describes how to display and use controls.

The highlights above show the interactive elements within the main workspace.

Clicking with the LMB within any of the above highlighted areas will display the associated controls for

the selected item.

Individual Equipment Controls

CT Gantry Controls Clicking anywhere on the CT gantry with the LMB or using the ‘1’ key will display all the controls for the

machine. The following interface will display when the CT has been selected.

Contrast Console Clicking anywhere on the Contrast Console with the LMB will display the Contrast Selection Panel. Alternatively use the Shortcut key ‘4’ or the ‘Equipment’ dropdown menu. Individual controls on the console can be activated using the LMB. Set Dose and Injection Rate. The Visual output for the left syringe will simulate the volume selected.

The Main Console Clicking anywhere on the Main “Workstation” Console with the LMB will display workstation interactive

panel. Alternatively use the Shortcut key ‘5’ or the ‘Equipment’ dropdown menu. The main console is

divided into three distinct tabs.

through the resultant scanned slices

The Scan Type Tab The ‘Scan Type’ tab is displayed by default when the main console is selected. This tab is typically the

first one to be used. Select the body area that you want to scan by clicking on a region with the LMB,

then select a Scan Protocol from the ‘Scan Protocol’ column. Depending on which body area you select

the ‘Scan Protocol’ column will be updated accordingly to reflect your choice.

Students can only select a procedure that the Administration has previously created in

medspace.CT™. Administrators refer to the separate ‘Administrator’s Guide’ for information on

the administration interface and operation.

Once in the room, a user can scan any anatomical region, however the technical standard

comparison will be available for the selected scan.

The ‘Parameters’ tab is where you set scan parameters for a Topogram and the final scan. The following

parameters are available in the ‘Topogram’ column.

0/90 Degrees Choose between 0 Degrees (AP) and 90 Degrees (Lat) Topogram scan type

Table In/Out Choose table scan direction (in/out)

180mm – 1280mm Choose scan length

Take Topogram Take the Topogram by clicking the button with the LMB

Users can “stop” the Topogram scan to reduce simulated dose if anatomical area is imaged.

To select scan area/region of interest (ROI), LMB click on the Topogram and click and drag out a square

region. Detailed in the Step-by-Step guide.

This scan box can be manipulated by LMB click in each corner to alter the position.

The following parameters are available in the ‘Scan Parameters’ column after the Topogram has been

taken and a scan area has been selected.

Slice Thickness Set the slice thickness for the final scan

Slice Interval Set the slice interval for the final scan

Select kVp Set the kVp for the final Scan

The CT Scan tab is where you conduct the scan and view the visual results of your scan. You can scroll

through the images using the scrollbar or alternatively you can step through individual images by using

the arrow keys on each side of the ‘Slice Number’.

Starting medspace.VR™ There are two ways to start the medspace.VR™ program. You can either:

Double click the medspace.VR™ icon on the desktop with your LMB

OR

Click the medspace.VR™ start menu shortcut with the LMB.

Selecting a Module and Mode On start the ‘Module’ selection screen will appear.

Click on the ‘CT’ button with the LMB. The module selection screen will be replaced with the ‘Mode’ selection screen.

Click on the ‘Tutorial’ button with the LMB.

Selecting a Protocol Once you are in the CT environment select the main console by clicking on with the LMB or alternatively using the shortcut key ‘5’ or the ‘Equipment’ dropdown menu.

Click on the body region with your LMB that you want to scan and select the protocol from the ‘Scan Protocol’ column. In this example we have selected Abdomen and Pelvis: Renal calculi – non contrast.

Patient entry and positioning The next step is to place the patient on the table in the correct orientation for the selected protocol. From the ‘Patient’ dropdown menu, select ‘Entry Position’ with the LMB. The ‘Patient Pose’ panel will be displayed.

Choose a pose that is appropriate to the selected protocol. In this example we will choose to place the patient in the supine position with the head first. Click ‘Pose’ to complete the selection.

With the patient posed it is possible to carry out fine adjustments for head tilt and rotation only. To adjust the head tilt and rotation click on the patient with the LMB. The ‘Head’ positioning control panel will be displayed. Click on the relevant movement button with your LMB to adjust the head pose.

Table movement There are number of ways of accessing the table controls for positioning. You can:

Click ‘Table’ on the dropdown ‘Equipment’ menu

Select shortcut key “2”

Click on the table with the LMB

Select the gantry controls

Table controls

Use the table controls to raise and move the table with the patient into the correct position for

the scan.

You can hold down the ‘Shift’ key while clicking on a pedal to speed up the movement.

It is useful to turn on the laser lights via the Gantry control panel to assist in positioning the

table as in the example below.

It is also useful to switch between the various camera views in order to achieve precise

positioning.

Positioning using the Gantry panel To position the table using the Gantry controls, click on the Gantry with the LMB or alternatively use the shortcut key ‘1’ or the ‘Equipment’ dropdown menu. The Gantry control panel will be displayed. Use the Gantry controls in the same way that you would use the table controls.

Alignment - patient in scanner It is useful to turn on the laser lights via the Gantry control panel to assist in positioning the table. It is also useful to switch between the various camera views in order to achieve precise positioning.

Skeleton View Skeleton view can be enabled through the ‘Tools’ menu by selecting the ‘Bone’ option. Alternatively use the shortcut key Ctrl + 8. This enables students to interact with the bony landmarks, normally palpated.

Loading Contrast and selecting Injection Parameters In order to do a contrast scan you need to load the contrast injectors. To do this, click on the contrast unit with you LMB. Alternatively use the Shortcut key ‘3’ or the ‘Equipment’ dropdown menu.

The contrast is now loaded and you can move on to setting scan and contrast parameters

Selecting Contrast Parameters Clicking anywhere on the Contrast Console with the LMB will display Load Contrast Panel. Alternatively use the Shortcut key ‘4’ or the ‘Equipment’ dropdown menu. Individual controls on the console can be activated using the LMB. Set Dose and Injection Rate by clicking on the relevant arrows with the LMB.

Setting up the Scan Parameters in the Main Console The final scan parameters are all set up using the main workstation console. It is a good idea to follow

protocol at this stage and switch to the operator room for the rest of this procedure. To go to the

operator room use the shortcut Ctrl + 6 or alternatively use the ‘Cameras’ dropdown menu and select

‘CT Panel’ with the LMB. Select the Main Console by clicking on it with your LMB or alternatively use the

shortcut key ‘5’

Setting Scan Type, Protocol and Scan Details

1. Click on the body part or region of the scan to select the Scan type 2. Next select the scan protocol wanted. If not available, ask your administrator to add the scan

protocol you want 3. Finally select the specific scan for that protocol e.g. pre and post contrast scan

Inputting Topogram Parameters After you have selected the region for scanning you must set the Topogram parameters.

Switch to the ‘Parameters’ Tab

Input the Topogram parameters relevant to the selected scan protocol.

Input:

Image orientation – AP or Lateral

Table movement – in or out

Length of scan – 180mm, 320mm, 640mm, 1280mm

Once your parameters have been selected click the ‘Take Topogram’ button with the LMB.

The initial Topogram scan will now take place. While the scan is taking place the following window will

be displayed. If you wish to stop the Topogram at any point, press “stop”.

Selecting Topogram Parameters on the Topogram image Once the Topogram scan is completed a Topogram image will appear in the Image Window.

Unless the administrators have uploaded a data set from your institution file set, the default images will

be reconstructed from the Visible Human™ dataset.

LMB click and drag over the Topogram to draw a box depicting the scan region. This box can be

manipulated by altering each corner if needed, or delete scan and start again.

Setting Scan Parameters With the scan area now defined we can move on to setting the actual scan parameters.

Set the desired Slice Thickness and Interval by clicking on the arrows with the LMB.

Set the desired kVp and mA by clicking on the arrows with the LMB.

Note: only preselected values can be chosen

Select Dose Modulation, if required, by clicking on the checkbox with the LMB.

Set the desired Table Direction by clicking on the relevant checkbox with the LMB.

Set the desired Image viewing direction (‘Head to Toe’ or ‘Toe to Head’) by clicking on the checkbox with the LMB.

Select instructions from the drop down box (Breathe In, Hold it etc)

Input “comments” which will be displayed on final image. For example, “Non contrast”.

If the start of the scan box is outside of the Topogram image area, as in the above example, the box must start within the image field and then be dragged outside the image area.

Selecting more than one scan region More than one scan region can be defined after the initial Topogram has been taken and prior to

starting the scan.

To add another scan, simply click and drag another box with the LMB. The green highlighted box

corresponds to the Scan number and therefore the selected parameters for that scan. Each scan can

have its own independent scan parameters.

The resultant scans will not demonstrate a change in images due to contrast, however, different

ROIs and start/finish scans will be shown.

The user can type into the comment box to identify the contrast scan (or other).

Performing the Scan Once all the scan parameters have been set and the correct protocols selected the scan can be conducted.

Select the ‘CT Scan’ tab by clicking on it with the LMB.

Click the ‘Start Scan’ button with the LMB and the scan will start and the machine will conduct each scan sequentially if multiple scans have been selected.

At the end of the scan when the table has returned to its starting position the scanned images will be displayed.

If these are from the default dataset provided by medspace.VR™, the Visible Human™ bone window data set will be displayed reflecting the scanning parameters selected in the previous step.

Alternatively, institutional DICOM images uploaded by Administrator can be used.

Images can be displayed by selecting ‘Slice Number’ arrows or toggle the scrollbar.

If numerous scans were performed, progress to next scan number using the arrow keys

Comparing with the Technical Standard The final scans performed by the user can be compared against the Technical Standard set by your administrator. To compare your results with the Technical Standard click on the ‘View Standard’ button with your LMB.

The technical standard created by your administrator or tutor will be displayed.

Anything in RED highlights the pre-set parameters by the admin that did not match the student’s

selection. As shown below, student 1 selected the same parameters and student 2 chose a different

kVp, dose modulation and breathing instructions.

Scanning with Contrast injection As mentioned previously in this manual it is possible to conduct scans that require a contrast injection.

IT SHOULD BE NOTED THAT THE PURPOSE OF THIS SOFTWARE IS NOT TO GENERATE HIGH QUALITY SCANNED IMAGES

BUT TO TRAIN USERS IN THE PROCEDURAL ASPECTS OF THE CT X-RAY PROTOCOLS.

To conduct a scan that requires a contrast injection the following steps must be followed.

Ensure contrast is loaded (shortcut key ‘3’) and dose measure and rate is selected (shortcut key

‘4’)

When you are absolutely ready to scan – i.e. all scan parameters have been selected and you are

ready for contrast, press the yellow button on the bottom right of the contrast panel to start

injection

Immediately open the Scan Console to see the contrast timer so you know when to start the

scan

Once the timer has reached the desired period click the ‘Start Scan’ button with the LMB.

The default CT dataset supplied with the medspace.CT™ module is the Visible Human Data Set

provide under license to medspaceVR by the United States National Library of Medicine,

Department of Health and Human Services on the 9th of August 2014.

These images have a default window setting of bone, they do not have any contrast

enhancement, regardless of the scan chosen during medspace.CT™ procedures

As per post-partum imaging, the lung fields and other structures are noticeably different in

comparison with live patients.

Administrators can upload institutional DICOM images (with appropriate permissions) as they

see fit. Refer to the administrator’s manual for details.
