Molina Medicare Options Plus HMO SNP is a Health Plan with a Medicare Contract and a contract with the state Medicaid program. Enrollment in Molina Medicare Options Plus depends on contract renewal.

Section 1 – Introduction

This directory provides a list of Molina Medicare Options Plus' network providers. To get detailed information about your health care coverage, please see your Evidence of Coverage.

Network providers such as healthcare professionals, medical groups, hospitals and other types of providers have been contracted with Molina Medicare Options Plus to provide you with covered services. You may utilize this directory to help you select a Primary Care Provider, Specialist, Hospital, Skilled Nursing Facility, Outpatient Mental Health Provider and/or Pharmacy.

Choosing your Primary Care Provider (PCP)

Your provider can be a physician, nurse practitioner, or other health care professional who meets state requirements and is trained to give you basic medical care. Health professionals are eligible to provide services as PCPs in our Plan when they practice in the areas of family medicine, general practice, geriatrics, internal medicine, nurse assistant, nurse practitioner or obstetrics/gynecology.

You will have to choose one of our network providers listed in this directory to be your Primary Care Provider (PCP). Generally, you must get your health care services from your PCP. A PCP has a contract with Molina Medicare Options Plus to deliver primary care services to members.

Your PCP is responsible for all your routine health care services; therefore it is important for you to call your PCP first with any health concerns. Selecting a PCP is up to you. Your relationship with your PCP is an important one. We strongly recommend that you choose a PCP close to home. Having your PCP nearby makes receiving medical care easier through developing a trusting and open relationship with your PCP.

If there is a particular specialist or hospital that you want to use, check first to be sure that your PCP can make referrals to that specialist or uses that hospital. A specialist is a provider who renders health care services for a specific disease or part of the body. There are many kinds of specialists such as oncologists who treat patients for cancer; cardiologists who care for patients with heart conditions; and orthopedists who care for patients with certain bone, joint, or muscle conditions.

If you discover any errors in this directory, please call Member Services so that we can correct the issue and assist you with selecting a new PCP.

How to change your PCP?

You may change your PCP for any reason, at any time. To change your PCP, simply call Member Services at (888) 665-1328, 7 days a week, 8 a.m. to 8 p.m., local time. TTY users should call 711.

When you call, be sure to tell Member Services if you are seeing specialists or getting other covered services that needed your PCP’s approval (such as home health services and durable

medical equipment). Member Services will help make sure that you can continue with the specialty care and other services you have been getting when you change to a new PCP. They will also check to be sure that the PCP you want to switch to is accepting new patients.

Sometimes a doctor, specialist, clinic, or other network provider that you are using might leave the Plan. If this happens, you will have to switch to another provider who is part of the Molina Medicare Options Plus plan.

If your PCP leaves Molina Medicare Options Plus, we will let you know and help you switch to another PCP so that you can continue to receive covered services. Member Services will change your membership record to show the name of your new PCP, and will tell you when the change will take effect. They will also send you a new membership card that shows the name and phone number of your new PCP.

Getting care from your PCP

Your PCP will provide, arrange, and/or coordinate all your routine medical care. You are encouraged to develop a close relationship with your PCP. Your PCP will get to know you as an individual, become acquainted with your medical history, and assist in developing a health care program to fit your lifestyle and medical needs. Your PCP will provide most of your care and will help you arrange or coordinate laboratory tests, x-rays, therapies, specialty care, hospital admissions, and follow-up care. Except for emergency services or as stated elsewhere in the Evidence of Coverage, services and supplies obtained without an advance referral from your PCP will not be covered under your Plan.

Getting care from specialists and other network providers

Routine medical care will be provided by your PCP. During the course of treatment, your PCP may decide that you need specialized treatment and give you a “referral” (approval in advance) to see a network specialist.

Molina Medicare Options Plus maintains a network of specialty providers to care for its Members. A referral from your PCP is required (unless otherwise allowed – i.e. women’s health services, etc.) to receive specialty services; however, no prior authorization is required when your PCP refers you to a network provider.

The network providers listed in this directory have agreed to provide you with your health care services. You may go to any of our network providers listed in this directory. However, some services may require a referral.

Each PCP has certain network specialists they use for referrals. The Molina Medicare Options Plus specialists you can use may depend on which PCP you choose. If there are specific specialists you want to use, find out whether your PCP refers patients to these specialists.

You can change your PCP at any time if you want to see a network specialist that your current PCP does not refer to. If you want to change your PCP, simply call Member Services.

If there are specific hospitals you wish to use, find out whether your PCP uses these hospitals.

It is very important to get a referral from your PCP before you see a network specialist or certain other providers (there are a few exceptions, but not limited to the following: physical exams and women’s preventive healthcare). If you don’t have a referral before you get services from a specialist, you may have to pay for these services yourself.

If you have been going to one network provider, you are not required to continue to go to that same provider. In some cases, you may get covered services from non-network providers.

If you have any questions about whether we will pay for any medical service or care that you are considering, you have the right to ask us whether we will cover it before you get the service or care.

At times, your PCP may request a service that is unavailable or not provided within your network. Your PCP may need to get approval ahead of time from Molina Medicare Options Plus to authorize this type of specialty referral. This process is called “prior authorization.” If you have questions or concerns about a prior authorization, you may call Member Services.

Out-Of-Network Provider Bills

If you receive a bill from an out-of-network provider do not pay this bill, please submit the bill to Molina Medicare Options Plus for processing and determination of your liability, if any.

Getting care if you have a medical emergency or an urgent need for care

A “medical emergency” is when you believe that your health is in serious danger. A medical emergency includes severe pain, a bad injury, a sudden illness, or a medical condition that is quickly getting much worse. You may get covered emergency medical care whenever you need, anywhere in the United States. Call 911 for help or go to the nearest emergency room, hospital, or urgent care center. You don’t need to get approval or a referral first from your doctor or other network provider.

“Urgent Care” is when you need medical help for an unforeseen illness, injury, or condition, but your health is not in serious danger. If you have a pressing, non-emergency medical need while in the service area call your PCP first. If they are not available, look in this Provider Directory for an urgent care center. You generally must use network providers (refer to the Evidence of Coverage). If, while temporarily outside the Plan’s service area, you require urgently needed care, then you may get this care from any provider. In addition, when out of the service area you can obtain dialysis treatment from any qualified dialysis provider.

Getting care when traveling outside the Plan’s service area

If you need care when you are outside the service area, your health care coverage is limited. The only services covered when you are outside the service area is care for a medical emergency, including urgently needed care, renal dialysis (kidney), and care that Molina Medicare Options

Plus or a network provider has approved in advance. For Molina Medicare Options Plus there is no coverage when traveling outside the country except in limited areas.

If you have questions whether a service is covered when you travel, please call Member Services.

You must use network providers except in emergency or urgent care situations or for out-of-area renal dialysis or other services. If you obtain routine care from out-of-network providers, neither Medicare nor Molina Medicare Options Plus will be responsible for the costs.

Cómo seleccionar su proveedor de atención médica primaria (PCP)

Su proveedor puede ser un médico, enfermero especializado u otro profesional médico que cumple con requisitos estatales y está capacitado para brindarle atención médica básica. Los profesionales médicos son elegibles para proveer servicios como un PCP en nuestro plan de salud cuando practican las especialidades de medicina de familia, medicina general, geriatría, medicina interna, auxiliar de enfermería, enfermera especializada o tocoginecología.

Usted tendrá que elegir uno de nuestros proveedores de la red que aparece en este directorio para ser su Proveedor de Atención Primaria (PCP, por sus siglas en inglés). En general, usted debe recibir servicios médicos de su PCP. Un PCP tiene un contrato con Molina Medicare Options Plus para brindar los servicios de atención médica primaria a los miembros. Todos los proveedores tienen acceso a intérpretes por teléfono.

Su PCP será responsable de todos sus servicios médicos rutinarios. Por lo tanto, si tiene alguna inquietud relacionada con la salud, consulte primero a su proveedor. Si hay un especialista u hospital en particular que usted desea utilizar, primero verifique si su PCP puede ordenar una remisión a ese especialista u hospital. Un especialista es un proveedor que presta servicios de atención médica para una enfermedad particular o parte del cuerpo. Existen varios tipos de especialistas, como oncólogos que tratan a pacientes con cáncer; cardiólogos que atienden a pacientes con condiciones del corazón; y ortopedistas que tratan a pacientes con ciertos tipos de condiciones de los huesos, articulaciones y músculos.

Llame al Departamento de Servicios para Miembros si usted encuentra algún error en este directorio, y así darnos la oportunidad de corregir el problema y ayudarle a seleccionar un nuevo PCP.

¿Cómo puede cambiar su PCP?

Puede cambiar de PCP por cualquier razón y en cualquier momento. Para cambiar su PCP, simplemente llame al Departamento de Servicios para Miembros al (888) 665-1328 los 7 días de la semana, de 8:00 a. m. a 8:00 p. m., hora local. Los usuarios de TTY deben llamar al 711.

Cuando llame, asegúrese de informar al Departamento de Servicios para Miembros si usted está consultando especialistas o si está recibiendo otros servicios cubiertos que necesitan la aprobación de su PCP (como los servicios de atención médica domiciliaria y equipo médico duradero). El Departamento de Servicios para Miembros le puede ayudar a asegurar que usted continúe recibiendo atención especializada y otros servicios que usted ha estado recibiendo cuando cambie a un nuevo PCP. También verificarán que el PCP al que quiere cambiarse está aceptando nuevos pacientes.

En ocasiones un médico, especialista, clínica u otro proveedor de la red que usted está utilizando podría dejar de participar en el plan de salud. Si esto ocurre, usted tendrá que cambiarse a otro proveedor que participe en el plan de Molina Medicare Options Plus.

Si su PCP abandona Molina Medicare Options Plus, nosotros se lo informaremos y le ayudaremos a cambiar a otro PCP para que usted pueda continuar recibiendo los servicios cubiertos. Cómo recibir atención médica de su PCP

Su PCP llegará a conocerlo personalmente, se familiarizará con su historia médica y le ayudará a desarrollar un programa de salud que se adapte a su estilo de vida y sus necesidades médicas. Su PCP le brindará la mayor parte de su atención médica y le ayudará a organizar y coordinar: análisis de laboratorio, radiografías, terapias, atención especializada, ingresos al hospital y atención de seguimiento. Exceptuando los servicios de emergencia o como se indique en otra parte de la Evidencia de Cobertura, los servicios y suministros obtenidos sin una remisión por adelantado de su PCP no estarán cubiertos bajo su plan.

Cómo obtener atención médica de especialistas y otros proveedores dentro de la red

Su PCP le brindará atención médica de rutina. Durante el tratamiento, es posible que su PCP decida que usted necesita un tratamiento especializado y le dé una "remisión" (autorización previa) para consultar con un especialista de la red.

Molina Medicare Options Plus mantiene una red de proveedores especializados para atender a sus miembros. Se necesita una remisión de parte de su PCP (a menos que esté permitido - por ejemplo, servicios de salud femenina, etc.) para recibir servicios especializados; sin embargo, no necesita autorización previa cuando su PCP lo remite a un proveedor dentro la red.

Los proveedores de la red que aparecen en este directorio han acordado brindarle servicios de atención médica. Puede acudir a cualquiera de nuestros proveedores de la red enumerados en este directorio. Sin embargo, algunos servicios pueden requerir una remisión.

Cada PCP tiene ciertos especialistas de la red que utiliza para hacer remisiones. Los especialistas de Molina Medicare Options Plus que usted puede utilizar dependerán del PCP que usted elija. Si hay especialistas específicos que usted quiera utilizar, averigüe si su PCP remite a sus pacientes a dichos especialistas.

Usted puede cambiar su PCP en cualquier momento si quiere consultar a un especialista de la red al cual su PCP actual no pueda remitirlo. Si usted quiere cambiar de PCP, solo tiene que llamar al Departamento de Servicios para Miembros.

Si usted desea utilizar hospitales específicos, verifique si su PCP trabaja con dichos hospitales.

Es muy importante que obtenga una remisión de su PCP antes de visitar a un especialista de la red o ciertos otros proveedores (existen algunas excepciones, pero sin limitarse a las siguientes: exámenes físicos y atención médica preventiva para la mujer). En ocasiones, su PCP puede solicitar un servicio que no está disponible o que no se proporciona dentro de su red. Es posible que su PCP necesite obtener una aprobación por adelantado de Molina Medicare Options Plus para autorizar este tipo de remisión a una especialidad. Este proceso se denomina "autorización previa". Si usted tiene preguntas o preocupaciones acerca de una autorización previa, puede comunicarse al Departamento de Servicios para Miembros.

Facturas si usted usa proveedores fuera de la red

Si usted recibe una factura de un proveedor fuera de la red, no pague esta factura. Envíe la factura a Molina Medicare Options Plus para procesamiento y determinación de su responsabilidad, si existe alguna.

Cómo obtener atención médica si tiene una emergencia médica o una necesidad de atención urgente

Una "emergencia médica" existe cuando usted cree que su salud está en serio peligro. Una emergencia médica incluye un dolor intenso, lesión grave, una enfermedad repentina o una condición médica que empeora rápidamente. Usted puede recibir cobertura para atención médica de emergencia cuando la necesite y en cualquier parte de los Estados Unidos. Llame al 911 para obtener ayuda o acuda a la sala de emergencias, hospital o centro de atención urgente más cercano. No es necesario que primero obtenga una autorización o remisión de su médico u otro proveedor de la red.

La "atención médica urgente" es necesaria cuando necesita ayuda médica con una enfermedad, lesión o condición inesperada, pero su salud no está en serio peligro. Si tiene una necesidad médica apremiante pero no de emergencia, mientras que está en el área de servicio, primero llame a su PCP. Si este no está disponible, consulte este Directorio de proveedores para localizar un centro de cuidado urgente. Generalmente debe recurrir a los proveedores de la red (consulte su "Evidencia de cobertura").
