Health Interface Service – working as part of the multi-disciplinary team based in The Bungalow with
integrated discharge team at Musgrove Park Hospital, you will work with people and their families
discharged from the hospital on a reablement/discharge to assess pathway back to their own home.
Getting people out of hospital and assessed in their own homes instead of on a hospital ward provides
a much better understanding of their abilities and needs. You will be responsible for devising
reablement plans to maximise their independence, setting personalised goals with them, and for the
reablement providers to work on. You will advise on creative solutions and utilise assistive technology
to help people return and remain at home. You will work closely with community and voluntary
organisations, as well as health and social care professionals to provide better outcomes for people.
Community Occupational Therapist - As a strong team player you will be dedicated to supporting
people at home to maximise their independence and live fulfilling lives. Working in the local community
team alongside social care workers you will find creative solutions for a range of client groups. We have
a formal programme of induction and CPD get you up and running in your new post, with ongoing

West Somerset. The work of the employment hubs are well-positioned to assist local people to be

employability skills development and training through utilising the hubs as venues for provision and
