Veine porte preduodenale: Apropos d'un cas

RESUME : U ne veine portc preduodcnale fut decouverte a !'operation chcz un enfanr de h uit ans qui resscntait des douleurs epigastriques post prandiales soulagccs par vornisSernenrs depuis dcux ans et qui a presente deux episodes de pancreatitc ayant neccssite !'hospitalisation clans la dernicre annee. L'anomalie de la vesicu lc biliaire a la cholecystographic. le retard a la vidangc gastrique, l'angulation particulierc de la region prepyloriquc et la non rotation incescinalc au rcpas baryce, de memc q uc la position anormale Jc la papille a la pancrcatographie retrograde, auraient pu , en retrospective, pcrmettre Jc suspecter le diagnostic avant !'operation. Une revue de la liueraturc montre en effcr que cette malformation s'accompagnc sou vent d'au tres anomalies. principalernent au niveau du tube digestif. Lorsqu'il ya sym ptomarologic clinique en de hors de la periodc neonatale, ii s'agi r le plus sou vent de subobscruction ala sortie de l'esromac. tive. L'investigation a montre les anomalies
suivan tes: u ne vesicule biliairc allongee et rcpliee sur elle-meme a la cholecystographic; une stase gastrique importance, avec angulation prononcee de la region prepylorique au rcp.:is barytc (Figu re I) ct nonrotation intestinale .:iu transit du grele ( Figure 2). Une papille de Vater
situcc a 180° de sa posi tion norrnalc et par ce fait. impossible a canulcr, a la pancreatographie retrograde.
C'cst a !'operation qu'on decouvre la presence d'unc veine porte passant en avan t de la p remiere portion d u duodenum et faisant obstruction a la vidange gastriq u e ( Figure 3). Le duodenu m quanta lui descend verticalement, se
rccourbc sur lui-rnerne vc rs le h aut, l'arriere et la droite sur une distance de quclques centimetres. avant de se con-

Le duodenum est libcre poscericuremenc pour en defoirc la courburc et unc gas trod uodenos t0rnie est pratiq uce. Mis a part un episode de reflux gastroesophagien traite medicalemcnt, !'evolution postopcracoirc fut sans particulari tc. En fevricr 19~7. l'enfanr nc prescntait plus de douleur. n'avair plus de vomissemcnrs ct route medication l'tait ccssce.
REVUE DE LA LITTERATURE Parmi environ 75 cas de vcinc portc
preduodenale rapportcs a dace, nous en avons rcccnu 41 chez qui nous avons pu c'.!tudicr !'observation originalc ( 1-30). Nou~ avons rcjete les cas decries par Snavely (31 ), OLI ii s'agit en fait d'unc double vcine portc avec stcno~c et hypertension porralc, ct par Grcatrcx (32), oi:1 la vcine porte passc en avant du choledoquc, mais dcrrierc le duodenum.
Nous avons classi fie ccs cas scion l',1gc auq uel le diagno:.tic a etc pose er nous avons aussi relcvc les aucrcs anomalies rencontrecs clans chacun de ces cas (Toblcau I). Com me on le voit, sculcment sept patients ne prcscntaient aucune autrc anomalic ct rous craient des adultcs. Cercains presentaicnt deux, trois ec mcmc quacre anomalies associecs, la p lupart a u ni vcau du tube digestif. parfois au niveau du coeu r ct des gros vaisseaux. De loin , l'anomalie la plus frcquente eta it un dcfaut de rotation OU
de fixation des mcsos.

Vingt de ces patients se sont presences avec un synd rome d'obstruction duodenale ct il y avait obstruction intrinscque par anomalic associec (atresie ou diaphragmc) clans 12 de ccs cas. L'autrc cas fut unc dccouvcrtc forruite !ors d'une auropsie chez un patient dccede d'unc cardiopathic majeure ( 3).

manifesce par une image de subobstruction a la sortie de l'estomac ( 18). Dans les six autres cas, la veine pone prcduodenale fut une dccouverte fortu ite !ors J'explorations pour atrc:;ic des voics biliaircs. Chez l'enfont plus age. ii y avait image de subobstruction gastrique, avcc vomissements repctes, dans lcs trois cas relcvfa, incluant ['observation prfacntc.
Enfin, che: l'adulte, un patient s'est presen ce avec une subohstruction chroniquc ( 26) cc dcux patients avcc des syndromes ulccrcux (22-23). La vcine portc preduodcnalc fut unc dccouverce fortuite clans les huit autres cas. done trois a l'autopsic ct cinq !ors de ch irurgic biliairc, l'anomalie ayant amcnc unc h.cmorragie peropcratoirc massive darn, un cas ( II).

ABSTRACT: A prcJuodcnal portal vein wa~ discovcrcJ at opcracion in an eightyear-old child. The chi ld was complaining of postprandial epigascric pa ins relieved by vomitting, and who presented with cwo attacks of pancrcatitis requ iring hospitalization in the previous year. Retrospectively, gallbladder's anomaly nt cholccystography, gastric ~ta sis with a peculiar aspect of pre pyloric area, intestinal non rotation at barium mea l and the abnormal position of the papi ll a ar endoscopic retrograde cholcdochopancrcatography could have made a preoperative diagnosis possible. Review of the literature reveals th at p reduodcnal portal vein is often accompanied by other anomalies, main ly in rhc d igestive system. When symptoms occur outside the neonatal p~'riod, it i~ usually those of a gastr ic outlet subocclusion. This is the first time that a clinical picture of recurrent pancrcatitis has been reported with th is anomaly.
