Page 1 Velos Clinical Laboratory Services Intake Process (Study Team) This tip sheet will guide you through the Clinical Laboratory Services Intake process in Velos. The Study Team will submit the intake form and the study status UTSW Ancillary – Clinical Laboratory Services – Submitted in Velos. After submission of the status by the Study Team, the Clinical Laboratory Services Specialists manage the addition of the remainder of UTSW Ancillary Clinical Laboratory Services statuses. Step 1: Log into Velos 1. Type this link in your web browser: https://velos.swmed.edu 2. Type your login ID in the Username field and password in the Password field. 3. Click on the Login button. 4. To logout, click on the Logout button on the top right. Step 2: Search for the Study 1. Enter the study number in the Seach a Study field and click on the Search button. You must enter your Username in all lowercase letters, but your password as you would normally enter it. The Velos default e-Signature is 1234. You should allow for pop-ups from the Velos system in your internet browser. Velos – Clinical Laboratory Services Intake Process (Study Team)

This tip sheet will guide you through the Clinical Laboratory Services Intake process in Velos. The Study

Team will submit the intake form and the study status UTSW Ancillary – Clinical Laboratory Services –

Submitted in Velos. After submission of the status by the Study Team, the Clinical Laboratory Services

Specialists manage the addition of the remainder of UTSW Ancillary Clinical Laboratory Services statuses.

Step 1: Log into Velos

1. Type this link in your web browser: https://velos.swmed.edu

2. Type your login ID in the Username field and password in the Password field.

3. Click on the Login button.

4. To logout, click on the Logout button on the top right.

Step 2: Search for the Study

1. Enter the study number in the Seach a Study field and click on the Search button.

You must enter your Username in all lowercase

letters, but your password as you would

normally enter it.

The Velos default e-Signature is 1234.

You should allow for pop-ups from the Velos

system in your internet browser.

2. Click on (Study Administration) button located under Quick Access on the left to pull up the

study.

Or

3. Right click on the (Study Administration) button located under Quick Access and click on

the Forms menu item.

Step 3: Locate and Submit the Clinical

Laboratory Services Intake Form

1. Navigate to the Forms tab.

2. From the Jump to Form dropdown select the form Clinical Laboratory Services Intake Form and click

on the Go button.

3. Click on the New button to create a new form.

4. Enter the information in SECTION 1: TO BE FILLED BY STUDY TEAM.

5. After the form has been filled, scroll down to the bottom and select the Form Status as Completed.

6. To submit the form, enter your e-Signature and click on the Submit button.

Step 4: Enter Study Status

1. After Submitting the Clinical Laboratory Services Intake form the study team must enter the status

UTSW Ancillary – Clinical Laboratory Services – Submitted.

2. Click on Study Status tab.

3. Click on the link ADD NEW STATUS located on the right.

a. Organization: Select ‘UT Southwestern Medical Center and Affiliates’.

b. Status Type: Select ‘Ancillary Service’.

c. Study Status: Select ‘UTSW Ancillary – Clinical Laboratory Services – Submitted’.

d. Documented By: It will be defaulted with your name.

e. Status Valid From: Today’s date.

4. Enter your e-Signature and click on the Submit button.

NOTE: If the Clinical Laboratory Services Team has indicated a study status UTSW Ancillary – Clinical Laboratory

Services – On Hold, the Study team must correct the information as indicated in the Study Status notes and

resubmit the study for review. Follow steps 3 and 4 to resubmit the study to the Clinical Laboratory Services

team.
