Getting Started
There are a few things that I will need from you prior to our initial consultation so we can make the most out of our time together.
1. Sign disclaimer form
Please make sure you have signed the disclaimer form after purchasing your ses-sion. The form can be found at www.dianarosekottle.com/order-success. Your electronic signature will serve as your written consent to all terms and agreements.
2. Schedule our session
Please send me as many open days/time windows (based on Pacific time - Califor-nia, US) that you have available over the next 3 weeks so that I can get you into my soonest opening that matches your availability. You may list your time prefer-ences and I will do my best to accommodate those. Please also let me know what time zone you are in - but be sure to list availability based on my time zone (Pa-cific Time - California, US) so there will be no confusion.
Most sessions are done over Skype, allowing us to see each other, however please let me know if you prefer a phone session.
3. Connect with me on Skype
Please send me a Skype request to: diana.kottle
Please also include your phone number (in case we encounter technical difficulties, or if you prefer a phone session) and your Skype name on the intake form, as sometimes Skype requires both parties to send requests in order to connect.
I will video call you on Skype, or phone you, at our scheduled session time.
4. Complete Intake From
Please complete the intake from to the best of your ability
This information will help provide me with a snapshot of you and what’s going on, prior to our session together.
• Please complete the intake form in a word document (not in the body of the email) and email as an attachment to [email protected] at least 3 days prior to our scheduled session. (This will ensure that I have time to review it before we meet)
