1 (1-3 marks) Simple points made in relation to the question and to the bullet points provided.

Medical Knowledge Role of the church

E.g there was a lack of an understanding in how to treat illness during the Middle Ages.

2 (4 - 6 marks)

Maximum 5 marks for Level 2 answers that do not go beyond aspects prompted by the stimulus points

Evidence used to back up the points made.

Medical KnowledgeE.g. herbal remedies were used during the Middle Ages. These would be provided in the home or in the churches.

Role of the ChurchE.g. the church provided care during the Middle Ages. Within the church monks and nuns would look after the patients by giving them food and rest.

Lack of better ideas/ technologyE.g. people did not have better ideas about how to treat disease. People did not challenge the work of Galen or Hippocrates as people could not prove their theories wrong.

3 (7 –9 marks)

Maximum 8 marks for Level 3 answers that do not go beyond aspects prompted by the stimulus points.

Because is used to clearly explain why the importance of the evidence presented.

Students explain why each of the factors was important in explaining why medicine remained ineffective during the Middle Ages.

E.g the church provided care during the Middle Ages. This is because Christianity became the dominant religion. Within the church, monks and nuns would look after the patients by giving them food and allowing them to rest. The church provided ‘care not cure’. This meant that patients would not be treated or cured because the monks and nuns lacked medical knowledge and relied on the work of Galen. This meant that the church refused to adopt any ideas.

4 (10-12 marks)No access to Level 4 for answers which do not go beyond aspects prompted by the stimulus points.

Analysis of the importance of each factor related to the question.

Students analyse the importance of each factor in the explaining why medicine remained ineffective during the Middle Ages:

E.g. the dominance of Christianity and the role of the church was influential in the treatment of illness during the Middle Ages. The church advocated prayer, rest and herbal remedies as treatments for illness. Patients relied on God to cure them and relied on remedies which often did not work. The crucial problem was that there was a lack of knowledge about what caused illnesses, therefore it was difficult to cure them.

Here’s a good way to look at a 12 mark answer – which one of these is closest to describing yours?

Superficial answer – 1-3 marks

I have written an answer which has a few sentences or pieces of information. These are quite general. E.g. There was a lack of an understanding in how to treat illness during the Middle Ages.

A shallow answer – 4-6 marks

I have used examples to back up or make the sentences or points I have written. Even if I’ve added information of my own it’s only evidence or examples NOT explanations. E.g. the church provided care during the Middle Ages. Within the church monks and nuns would look after the patients by giving them food and rest.

A deep answer – 7-9 marks

You can only get 6 if you only use the 2 bullets. I’ve explained the points and examples and linked these to the question. E.g the church provided care during the Middle Ages. This is because Christianity became the dominant religion. Within the church, monks and nuns would look after the patients by giving them food and allowing them to rest. Superficial answer 1-4 marks

You have written a basic summary – maybe with generalised comment that is accurate but with limited knowledge / examples. No conclusion reached.

People in the Middle Ages liked to be treated at home. Hospitals were not very good because treatments weren’t good. Physicians were expensive. You could be cared for by your family at home.

A shallow answer 5-8 marks

You begin to explain why people preferred to be treated at home rather than hospitals. You support this with some good examples. A conclusion is reached but it is not supported.

I agree because people at home were respected for their cures and you would get treated the same at home as you would in a hospital. Hospitals focused on care not cure and people could get this at home anyway. There were more hospitals in the Middle Ages and a lot of people did use them because they were very religious. The care provided could comfort patients.

2. “Hospital treatment in England in the period from 1250 to 1500 was very rare.” How far do you agree? Explain your answer. [20]

Skills checklist – self assessment – Structure (S), Focus (F), Depth (D), Knowledge (K)…

(S) Did you include 2 clear paragraphs, one arguing in favour of the statement and one arguing against it? _____

(S) Did you write a conclusion giving your final judgement? _____ (F) Did you use the keywords in the question in the first sentence of each paragraph? _____ (F) Did you link back to these keywords in the last sentence of your first two paragraphs? _____ (D) Did you explain how each example supported the point to your paragraph? _____ (D) Did you develop your answer (approx. 10-15 lines per paragraph)? _____ (K & D) How many key historical terms did you use? ____

(K) Did you refer to the following in your answer:

Hospital care Y/N Care in the home Y/N

Number of hospitals did increase in the Middle Ages – by 1500 an estimated 1100 hospitals run by the Church.(especially good for the poor)

The vast majority of people in the Middle Ages were cared for at home. Women would make the patient comfortable and prepare food and herbal remedies.

Hospitals were run by monks and nuns who did use herbs, poultices and prayer – but mostly focused on care for the soul not curing illness.

Herbs could be grown in the garden of the home. Therefore easier and cheaper to be treated at home.

Hospitals in the Middle Ages were good places to rest and recuperate as the monks and nuns kept things clean – change linen and clothes.

1400s – letters by Lady Margaret Paston and her husband suggest that they did not trust physicians – women at home were well respected and trusted.

What could be your 3 rd bullet point?

Humoural treatments – how could that fit with the question?

Herbs, bathing, diet – how could this be done at home – why go to hospitals when this could be done at home?

OR could use Medieval physicians – expensive – humoural – similar to home remedies.

A deep answer 9-12 marks – the detail you use is specific and clear – your answer is well structured and focused clearly on the question.

You examine both sides of the argument, beginning to explain why hospitals were important and also why a lot of people simply used home for remedies and care. This is supported by accurate and relevant information. Your conclusion is partially supported and argued.I mostly agree with the statement because…

Although some people did use hospitals, this was not as common as care in the home because…

The deepest answer 13-16 marks

You examine both sides of the argument, evaluating how strong each is, before reaching a final, detailed and justified conclusion. You support your argument with precisely selected details.

Links to the end of every paragraph plus final conclusion:

Many people preferred to be treated at home when they were ill not just because they would be cared for by their family, but because the remedies and treatments used by women at home could be home grown and because the women were often well respected for their ability to cure illness. Many cures were linked to the humours and to diet, therefore treatment at home was practical and often just as good, or better than that in hospitals which cared rather than cured patients. Therefore I mostly…

Simple or generalised comment to explain a similarity.

General information included showing limited knowledge and understanding of the period.

One similarity between people’s beliefs about the cause of disease in the Middle Ages and the Renaissance was that they continued to believe in the Four Humours.

2 (3-4 marks)

Features of the period are analysed to explain a similarity.

Specific and detailed information used to add support showing good knowledge and understanding of the period.

This can be seen in the way that patients were treated. For example, in the Middle Ages doctors looked, tasted and smelt people’s urine. Different colours were linked to different humours being out of balance and the treatments were aimed at balancing the humours back out again. Similarly, when Charles II was treated he was bled and purged with laxatives, both treatments used to balance his humours. Consequently, this shows how beliefs about the cause of disease were similar across the two periods.

Here’s another example of a 4 mark question. Notice how it is asking for a difference between 2 periods? It still requires the same skill of using specific information or examples to support the idea which is being used.

Skills checklist – self and peer assessment – focusing on the question (F) – identifying a similarity (S) – using clear examples to support (E)…

(S) Have you identified the key similarity between the two periods? (S) Give an example of how a similar belief was used in each period? (F) Did you use the keywords in the question in the first sentence of your paragraph? (E F S) Did you make sure that any examples used matched the similarity you were writing about? (E) Did you develop your answer (approx. 6-8 lines)? (S) Did you make sure you only focused on one similarity only? (E) Did you back up your answer with specific examples of the similarity?

William Harvey discovered circulation of blood. Renaissance medicine was not very good. There were new ideas. New ideas were not to do with medicine.

A shallow answer 5-8 marks

You begin to explain whether the ideas in the Renaissance made medicine better or did not lead to medical progress. You support this with some good examples. A conclusion is reached but it is not supported.

Harvey’s ideas did not help with medicine. Discovering the circulation of blood didn’t lead to progress in treatments even though it was a step forward in anatomy. This was the same with people like Vesalius who understood the body better but not medicine.

Unsupported conc – e.g. I agree quite a lot with the statement.

A deep answer 9-12 marks

You examine both sides of the argument, beginning to explain why new ideas which developed during the period had significant or limited impact on developments in medicine. This is supported by accurate and relevant information. Your conclusion is partially supported and argued.

At least two examples per argument.

Both sides of the argument covered.

The deepest answer 13-16 marks

You examine both sides of the argument, evaluating how strong each is, before reaching a final, detailed and justified conclusion. You support your argument with precisely selected details.

Links to the end of every paragraph plus final conclusion:

The impact on medicine of the work of Renaissance thinkers such as Harvey and Vesalius was limited at the time because their discoveries were focused on anatomy and not medicine. Discovering the circulation of the blood would help doctors to understand and develop ideas in the future, but from 1500 to 1700 technology was so limited that it had no impact on patients, treatments and medical understanding. This leads me to conclude that the statement is mostly true, especially when we link this to the treatments used by the very best doctors at the time on patients such as Charles II.

(K) Did you refer to the following in your answer:

Major Breakthrough Y/N Limited development Y/N

The work of Vesalius improved knowledge of anatomy but had little relevance to illness and treatment, therefore there was little change.

The authority of the Church was declining and therefore people were more willing to challenge Galen’s ideas.

The work of Harvey improved knowledge of physiology but had little impact on knowledge of disease and treatment therefore there was little change.

The printing press meant that ideas could spread more rapidly and medical students/ doctors could see accurate drawings of the body even if they didn’t conduct dissections. This undermined Galen’s authority and made challenges to accepted ideas more possible.

Medical training was still dominated by the ideas of Galen, therefore little change was likely.

The Royal Society was set up to encourage scientific investigation and communication of new ideas.

Religion remained a key factor in people’s ideas about disease, prevention and treatment, for example

Charles II continued the ceremony of ‘touching’ to cure scrofula.

Thomas Sydenham stressed the importance of studying the symptoms of different diseases, leading to a new method of classification and differentiation of disease.

12 mark answer – which one of these is closest to describing yours?

Superficial answer – 1-3 marks

I have written an answer which has a few sentences or pieces of information. These are quite general. E.g. Florence Nightingale improved conditions in hospitals and the role of nurses.

A shallow answer – 4-6 marksI have used examples to back up or make the sentences or points I have written. Even if I’ve added information of my own it’s only evidence or examples NOT explanations. E.g.Florence NightingaleE.g. work in Scutari, impact of this on British hospitals, role in improving nursing.Development of antiseptic techniquesE.g. better engineering led to better hospitals including ventilation.

Explain why there was significant improvement in the treatment of illness in hospitals during the industrial period. [12]

The deepest answer – 10-12 marks

I have analysed the importance of each of the 3 points linked to the question…E.g. However, had doctors continued to work with unsterilised equipment and without effective antiseptics then the infections would have continued from surgery, despite the work Nightingale did, making Nightingale’s work less important a reason for this improvement.

A deep answer – 7-9 marks

You can only get 6 if you only use the 2 bullets.I’ve explained the points and examples and linked these to the question. E.g. Florence Nightingale made developments in the cleanliness of hospitals by…this improved treatment and hospital care because patients were less likely to pick up an infection through dirty sheets and they were less likely to catch worse illnesses because of the bad ventilation.

A deep answer 9-12 marks

You examine both sides of the argument, beginning to explain why Jenner’s vaccination could be seen as a breakthrough, and also why it took so long to have an impact on medicine. This is supported by accurate and relevant information. Your conclusion is partially supported and argued.

At least two examples per argument that match the above.

Both sides of the argument covered.

The deepest answer 13-16 marks

You examine both sides of the argument, evaluating how strong each is, before reaching a final, detailed and justified conclusion. You support your argument with precisely selected details.

Links to the end of every paragraph plus final conclusion:

Jenner’s vaccine can be seen as a breakthrough because…This suggests to me that Jenner’s vaccine was a breakthrough because…Although the impact of the vaccine would be slow because of opposition to it which even ran into the 1900s, it can be said that it was a major breakthrough because…In spite of strong opposition to Jenner’s vaccine for Smallpox, support from the government eventually meant that by 1900 most people…

(K) Did you refer to the following in your answer:Major Breakthrough Y/N Limited development Y/N

Jenner’s vaccination was the first time a specific disease could be effectively prevented.

Many people resisted Jenner’s vaccination because they disliked the idea of using a disease linked to animals or because vaccination was sometimes incorrectly applied and seemed to fail; it therefore had limited effect until it was made compulsory and enforced by the government in 1853 and 1871.

The only development in the prevention of disease before Jenner’s vaccination was during the eighteenth century, when inoculation was used, but this had many problems, so it was not a major breakthrough.

Preventive measures against disease did not change – during the cholera epidemics of the nineteenth century, local authorities ordered barrels of tar to be burned, based on the idea of miasma.

Jenner’s vaccination succeeded in preventing one of the major killer diseases of the period, so that by 1900 it was no longer the threat it had been in 1700.

No one understood how or why the vaccination worked, and so this technique could not be applied to other major killer diseases such as cholera which appeared as a new threat in the nineteenth century.

Jenner was willing to offer free vaccinations so that all groups within society could receive protection from smallpox.

The application of Jenner’s vaccination depended on the chance link between smallpox and cowpox; even if the link was understood, it could not be replicated for other diseases. Only after the work of Pasteur and Koch in the late nineteenth century could vaccination be understood and others developed.

Teacher comment

1 (1-2 marks) Simple or generalised comment to explain a similarity.

General information included showing limited knowledge and understanding of the period.

One similarity between people’s beliefs about the cause of disease in the Middle Ages and the Renaissance was that they continued to believe in the Four Humours.

2 (3-4 marks) Features of the period are analysed to explain a similarity.

12 mark answer – which one of these is closest to describing yours?

Superficial answer – 1-3 marks

I have written an answer which has a few sentences or pieces of information. These are quite general. E.g. the development of magic bullets allowed certain illnesses to be targeted.

A shallow answer – 4-5 marksI have used examples to back up or make the sentences or points I have written. Even if I’ve added information of my own it’s only evidence or examples NOT explanations. E.g.Magic BulletsE.g. Paul Ehrlich set up a private laboratory to investigate the treatment of diseases. He found that several magic bullets had a specific attraction to disease causing micro – organisms in the body. These magic bullets could then target and kill the disease. High tech treatment E.g. High-tech treatment allowed for improvements in the treatment of illness. For example, Florey and Chain helped to develop the antibiotic penicillin by using an ice cream freezer. This allowed penicillin to be stored. DNAE.g. Francis Crick and James Watson discovered DNA. By using new technology they could then see the different genes within the body. The Human Genome Project mapped out the different genes within the 23 chromosomes of the body.

Explain why there was significant improvement in the treatment of illness in hospitals during the industrial period. [12]

The development of high tech treatment was essential in treating illness as penicillin could be distributed around the world. Penicillin was used to treat soldiers during WW2.

The deepest answer – 10-12 marks

I have analysed the importance of each of the 3 points linked to the question…E.g. high tech treatment was the most important factor in the treatment of illness during the Twentieth Century. The development of high tech treatment was essential in treating illness as penicillin could be distributed around the world. Penicillin was used to treat soldiers during WW2. Additionally, DNA could not have been discovered without technology as x rays and photography was used to discover genes.
