Happy Summer!

Welcome to Caledonia Recreation! My name is Ashley Cummings and I am the Recreation Director for the Village of Caledonia. This will be my seventh year working with the Recreation Program and my fifth year working as Recreation Director. I was born and raised in Caledonia, graduated from CMCS in 2007, and still live in Caledonia today! :) In July 2012, I graduated from Nazareth College of Rochester with my Master’s degree in Inclusive Childhood Education and outside of the Recreation Program, I teach third grade at Letchworth Elementary School.

Growing up, I loved attending Hamilton and Washburn Parks. While we still play many of the favorite traditional games, we also engage in various other activities, hold multiple special events, and take numerous exciting field trips! Many events, excluding field trips, are provided free of charge to youth! All events and activities are explained in detail within this booklet.

The phenomenal Recreation Staff work hard to provide our youth with engaging, exciting activities Monday through Friday, from 9 am - 4 pm! In order to maintain a safe and fun environment, my Staff and I expect that all children act safely, respectfully, and responsibly during their time at the Caledonia Recreation Program. In this way, Staff can assure that youth have the best experiences possible while at the Park. Please note that Staff cannot monitor which children come and go through the Park entrances. During Park hours youth are free to come and go as they please. As the Parks close due to severe weather, please ensure that your child is picked up if it is thundering or lightening.

Please feel free to contact me with any questions. I can be reached at 538-4181, or many days you can find me at the parks! My Staff and I look forward to helping your children make some wonderful memories this summer!

Sincerely,

Ashley Cummings

any and all claims, demands, or causes of action, which are in any way connected with my participation in this activity or  my  use  of  Endeavor  Entertainment’s  equipment  or  facilities,  including  any such claims which allege negligent acts or omissions of Endeavor Entertainment.

4. Should  Endeavor  Entertainment,  or  anyone  acting  on  their  behalf,  be  required  to  incur  attorney’s  fees  and  costs  to

enforce this agreement, I agree to indemnify and hold them harmless for all such fees and costs.

5. I certify that I have adequate insurance to cover any injury or damage I may cause or suffer while participating, or else I agree to bear the cost of such injury or damage. I further certify that I am willing to assume the risk of any medical or physical condition I may have.

Wheeled Rodeo Waiver

“2015 Annual Summer Recreation Bike Rodeo” Sponsored by: Cornell Cooperative Extension

Date: July 21, 2015 Time: 10:00 am Place: Washburn Park

Dear Parents,

Compliments of Local Law Enforcement, Livingston County Health Department and the Cornell Cooperative Extension, we able to hold our “Annual Summer Recreation Bike Rodeo”. This year the children are allowed to use any of the following items for the rodeo: bikes, skateboards, scooters, or rollerblades.

• Children are allowed to bring only “1” item (bike, skateboard, scooter, or rollerblades)• Parents please check your child’s equipment the night before the event. We will not be doing any

repairs, if there is a problem with their bike, etc., unfortunately they will not be able to ride.• Children are required to bring and wear a helmet to participate in the rodeo.• Parents- please review the enclosed quick fit check to determine if your child’s helmet is safe.

This year’s rodeo will feature:Helmet Inspection: helmets will be checked for cracks, holes or if it is too big or too small. If the child’s helmet does not pass inspection their helmet will be taken and not returned to them, Cornell will provide them with a new helmet to keep at no charge. There is a limited number of helmets so only children participating in the rodeo will be allowed to obtain a helmet if needed.

Wheel Inspection: the wheels on the item that they are riding will be inspected for safety purposes. We will be teaching the children what they should look for each time they get ready to ride. They will learn how to check their air pressure in the tires, how to check their brakes and they will learn how to check for any loose parts.

Rodeo: Once participants have completed all the inspection stations they will have an opportunity to ride through the rodeos set up by our local police department.

---------------------Permission Slip-----------------Please sign and return----------------------------

I understand and agree that the sole purpose of this program is to help increase the number of children wearing helmets that are the correct size, and are free from cracks/holes; that this inspection is being provided to as a free service to me; that this program cannot fully evaluate the quality, safety, or condition of my helmet or any component of my wheeled device; including the tires and the brakes, and that this program cannot guarantee my child’s safety in a wheeled collision. However, I understand that a properly fitted helmet can prevent or reduce fatal injury. For these reasons, I hereby release Cornell Cooperative Extension of Livingston County and any program participants, from any present or future liability for any injuries or damages that may result from a wheeled sports injury or otherwise. I hereby give my permission for my child to participate in the wheeled rodeo at the above location and I understand that if my child’s helmet is found to be unsafe, it will be replaced.
