Introduction

This document provides clinicians and technical support personnel with phone scripts and visual aids that can help guide patients through performing specific tasks that may arise while using the VitalConnect Vista Solution platform. The following tasks are covered:

• Replacing a VitalPatch • Recalibrating VitalPatch Posture (and Patch Off) • Recalibrating Body Temperature • Applying a VitalPatch Adhesive Overlay

When referring to documentation, ensure that you are using the correct release and product version. Procedures vary between releases.

Clinicians and support personnel using these scripts must be familiar with the relevant procedures in the above documents. For outpatient care, patients will require instruction on the maintenance and operation of the patient-facing aspects of the Vista Solution. The amount of education required will be dictated by the level of clinical assistance provided (less clinical assistance will require more patient education).

Replacing a VitalPatch

When to Use Use when the patient needs help replacing a VitalPatch biosensor. This is necessary under the following scenarios:

• VitalPatch may be at or near expiration (messages on screen for low or expired patch life)

• VitalPatch no longer adhering to skin properly (message on screen for patch not adhered)

• VitalPatch removed by patient without disconnecting from VistaPoint (message on screen for patch not adhered or patch disconnected)

Assistance Script Guidance cues are shown in red. Note that the screens shown in this procedure may vary depending on available features and release version.

Guidance What the Patient Sees

Step 1: Prepare for procedure

Before we begin this procedure, do you have what you need?

Guide patient through checking that they have the following items: • Documentation (IFU and/or QuickStart)

• Replacement VitalPatch in sealed pouch

• Adhesive removal pads and alcohol pads

• Shaving equipment (if necessary)

• Thermometer (not required for VitalPatch 1.5)

• VistaTablet nearby and powered-on

Step 2: View VistaTablet dashboard

Are you currently viewing the VistaTablet dashboard? Patient should see dashboard (says “VitalConnect VistaPoint” in upper-left corner), with or without patch expiration messages:

If you do not see the dashboard, press the Home button on the tablet, which is on the right side of the screen. Then tap the VistaPoint icon on the upper-left corner of the tablet screen.

Step 3: Disconnect Patch

Guide patient through selecting the Disconnect Patch option (if necessary) and arriving at a state where they can see the Connect VitalPatch button. The first step is to disconnect the patch on the VistaTablet.

Do you see a blue “Connect VitalPatch” button in the upper-right corner of the screen? • If user sees button, go to Step 4.

• If user does not see button (and there are no messages on the screen, but vital signs are active), say: Tap the Menu button in the upper-right corner and then tap Disconnect VitalPatch.

• If there are messages, see next guidance.

User sees “Connect VitalPatch”:

Is there a message in the middle of the screen? If yes, please tell me what the message says. • Listen to what user describes and refer

to messages on the right.

• If the message is not shown here, refer to VistaPoint IFU for guidance.

• After disconnecting, the user will be presented with the option to “Disconnect and Keep Monitoring” or “Discharge Patient”. Go to next.

If one of these two messages, select OK and then select Disconnect VitalPatch from the VitalPatch Menu.

Do you see a message asking you to Disconnect VitalPatch or Discharge Patient? • Instruct user to select the “Disconnect

VitalPatch Option

You should now see the blue “Connect VitalPatch” button in the upper right of the screen.

Step 4: Remove Old VitalPatch

Refer to the “Removing Previous VitalPatch” steps in the QuickStart Guide. If you do not have the guide, I will walk you through it.

Is there a clear plastic overlay over the patch? If there is, grab one corner of the overlay and stretch it horizontally to remove it from the skin. The patch might begin to come off while you do this.

Open an adhesive removal pad. Wipe the pad all the way around the edge of the patch. Grab one corner of the patch and pull it off the skin while sweeping the adhesive removal pad underneath. Once the patch is off your chest, clean the skin with an alcohol pad and allow to dry.

Step 5: Apply New VitalPatch

Refer to the “Applying the VitalPatch” steps in the QuickStart Guide. If you do not have the guide, I will walk you through it. • Is there body hair on the site where you

removed the old patch? If yes, shave the site using a disposable razor.

• If you haven’t already, clean the site with an alcohol wipe and allow to dry.

• Open the pouch with the new VitalPatch and place the patch on a flat surface near you. DO NOT remove the backings until I tell you to.

• Press and release the power button. Look for a green light to flash under the word “VitalConnect”.

Hold the VitalPatch against your chest in the same site as the patch you just removed. • If you are using the upper chest, the

side of the patch with three curved lines should be angled down to your left side, and side with the word “VitalConnect” should be towards the center of the body, about an inch below the notch of your collarbone.

• If you are using the site on the edge of your left rib cage, the side of the patch with three curved lines should be angled down to your left side, and side with the word “VitalConnect” should be towards the center of the body, about an inch below the notch of your collarbone.

• Hold one side of the patch against your chest while you flip the other side up and peel off the backing

• Repeat for the other side of the patch. Press down on both sides to make sure the patch is stuck to your skin.

• Keep the backing that has the ID number on it. You will need this for the next step.

Step 6: Pairing VitalPatch with VistaPoint

We are now going to pair the VitalPatch with your tablet. Tap the blue Connect VitalPatch button at the top-right corner of the screen.

When it asks you how you want to connect, tap the button on the left, Search for VitalPatch ID.

The tablet will say “Scanning for Devices”. Look at your backing label from the new patch and match it to the what appears in the list. Tap the matching ID when you see it. If the ID is not displayed in the list, press the power button on the VitalPatch. On the screen, select Yes to repeat the scan. The patch ID should now appear in the list.

After the patch is paired, the patient will be walked through calibrating the posture and temperature. Refer to the next step.

Step 7: Calibrate VitalPatch Posture The tablet should now show Posture Calibration with two pictures of a chest. Tap the picture that matches where you applied the patch and then tap the blue Next button.

The screen should now show either two or three choices for posture.

The best position is standing or sitting. If you can hold that position, tap the Standing button on the left.

If you are in bed, make sure you are flat in the bed and tap the Supine button on the right.

You should see a message asking you to confirm your choices. Are they correct? If they are, tap Yes.

After the patient taps Yes, they will see a message showing that calibration is in process.

Step 8: Calibrate Body Temperature

What do you see on the screen now? • If the patient sees no additional

messages, the pairing process is now complete and vital signs should be displayed on the tablet. Congratulations! You’ve put on a new patch and should be good for the next 5 days or 7 days. Don’t shower for the next 30 minutes.

• If the patient sees a message saying Temperature Calibration Required, continue with the temperature calibration process. Tap the blue Calibrate button.

This step assumes Fahrenheit units Use your thermometer to take your temperature. When you have your temperature, tap the Fahrenheit button and then enter your temperature and tap Calibrate.

Temperature calibration is now complete and patch has been replaced. Patient should see the VistaPoint dashboard. Congratulations! You’ve put on a new patch and should be good for the next 5 days or 7 days. Don’t shower for the next 30 minutes.

Recalibrating VitalPatch Posture (and Patch Off)

When to Use Use when the patient needs help recalibrating VitalPatch posture. This is necessary under the following scenarios:

• A message on the screen says “Calibration Required”

• VitalPatch has partially lifted off the skin and patient has pressed it back in place. Note that if this scenario occurs, patient should be guided through procedure for applying an Adhesive Overlay. Refer to IFU-09 and “Applying an Adhesive Overlay” later in this document.

• The posture on the dashboard says “Posture Unknown”

• The patient notices that the posture displayed does not match their actual posture

Guidance What the Patient Sees

Step 1: Determine the Current State of the VitalPatch

What do you see on the tablet screen right now?

• Listen to what user describes and refer to messages below.

• If the screen is off, guide user through waking up tablet.

• If the message is not shown here, refer to VistaPoint IFU for guidance.

If patient sees this on the screen, go to Step 2 below to help patient calibrate.

If the patient sees this on the screen, ask the following questions: Is the patch currently on your chest, or has it partly fallen off? If patch is not on chest or if it has partly fallen off, walk patient through replacing patch. Is the patch beginning to peel off, but still mostly attached? Press the patch down all the way around the edges and see if that fixes the problem. Wait 60 seconds for message to go away. Go to Step 2 below to

help patient calibrate. Decide whether applying an Adhesive Overlay is appropriate.

If the patient sees “Posture Unknown” or if their posture does not match what is displayed: Tap the Menu button in the upper-right corner of the screen and then tap Calibrate Posture.

Step 2: Calibrate VitalPatch Posture The tablet should now show Posture Calibration with two pictures of a chest. Tap the picture that matches where you applied the patch and then tap the blue Next button.

What do you see on the screen now? • If the patient sees no additional

messages, the calibration process is now complete and vital signs should be displayed on the tablet.

• If the patient sees a message saying Temperature Calibration Required, continue with the temperature calibration process. Tap the blue Calibrate button.

Recalibrating VitalPatch Body Temperature

When to Use Use when the patient needs help recalibrating VitalPatch body temperature (VitalPatch 5D only). This is necessary under the following scenarios:

• A message on the screen says “Temperature Calibration Required”

• 30 minutes after VitalPatch 5D has been applied

• Once per day after VitalPatch 5D has been applied

• The patient notices that their displayed body temperature does not match their actual body temperature

Applying an Adhesive Overlay

When to Use Use when the patient needs help applying an adhesive. This is necessary under the following scenarios:

• Patient has been instructed to apply the overlay, or asks to have an overlay put on

• VitalPatch has begun to peel up from the skin, but has not come off the skin

When NOT to Use • If the patch has fallen off the skin

• If an overlay has already been applied with the current VitalPatch

• If the patient has a known sensitivity to Tegaderm adhesive.

Assistance Script Guide patient through the steps presented in IFU-09, reading steps aloud if necessary.
