2. Why is CHI offering these additional voluntary benefit choices to me?

We provide employee benefits so that you can take care of yourself and your family: physically, mentally, financially, socially and spiritually. The Supplemental Health plans and the Identity Theft Protection plan can complement your other benefit choices.

3. Are the voluntary benefit options medical insurance?

No. It is very important to understand that these benefit choices, especially the two Supplemental Health plans, are not medical insurance. The Supplemental Health plans provide financial protection to help you pay for medical and household expenses in the event of a serious health issue.

4. How do the voluntary benefit options complement my medical insurance?

5. Am I eligible for these voluntary benefits?

All active benefits-eligible employees who are at least 18 years old may enroll in voluntary benefits. You do not need to be enrolled in a CHI medical plan or other CHI benefits to enroll in the Supplemental Health plans or the Identity Theft Protection plan.

6. When can I enroll in these voluntary benefits?

You can enroll during our Annual Enrollment period in Fall 2018. You can also enroll if you are a new hire, become newly eligible for benefits as a result of a job change, or if you experience a Qualified Status Change.

7. How do I enroll in these voluntary benefits?

When Annual Enrollment begins, please log on to the well-being pages found on InsideCHI and click the link for the Annual Enrollment page. Once you reach the Annual Enrollment page, click the “Enroll Now” button to begin enrollment. You can enroll yourself and eligible dependent(s) in each benefit separately, so you can choose the specific benefits that best meet the needs of you and your family.

8. What if I am on leave of absence during the Annual Enrollment period?

Even if you’re on leave of absence during Annual Enrollment, you can enroll in benefits via the well-being pages found on InsideCHI.

9. How do I pay for these voluntary benefits?

Premiums are paid through payroll deductions. If you choose to enroll in any or all of the three benefit plans, the premiums will be added to your overall payroll deductions which are collected each pay date for a total of 26 pay deductions in a calendar year.

10. When may I cancel voluntary benefits coverage?

You may cancel voluntary benefits coverage at any time. Call the HR/Payroll Connection Support Center at 1-844-450-9450 (8 a.m. – 8 p.m., EST, Monday-Friday) for specific information on how to cancel coverage.

Group Voluntary 24 Hour Accident Coverage 11. What is Group Voluntary 24 Hour Accident Coverage?

This plan pays a specified amount if you or a covered family member is injured as a result of a covered accident. It provides 24-hour coverage for on- and off-the-job injuries.

12. How does it protect me?

Accidents can happen anytime, anywhere. If you or a covered family member are injured in an accident and require medical care, even with medical coverage, your out-of-pocket costs can add up. Group Voluntary 24 Hour Accident Coverage can help with these costs.

13. How are Group Voluntary 24 Hour Accident benefits paid?

If you or a covered family member are injured in an accident, you will need to file a claim. If the claim is approved, the benefit payment is mailed directly to you. It is not mailed to a doctor, hospital or medical insurer unless you request that. The benefit can be used for any expenses you have, from deductibles, copays and coinsurance, to everyday costs such as housing, utilities, food, or child care.

14. Whom can I protect by enrolling in Group Voluntary 24 Hour Accident Coverage?

You can choose one of the following options:

Employee

Employee + Spouse/Legally Domiciled Adult (LDA)

Employee + Child(ren)*

Employee + Family

*All children under age 26, regardless of marital status or financial dependence, can be covered under the plan. Disabled dependents can remain on the plan after age 26 and may also join the plan at any time as long as they are approved as a disabled dependent per the plan rules.

15. How does Group Voluntary 24 Hour Accident Coverage complement my medical plan?

It offers additional financial protection against the costs associated with being injured in an accident. Your medical plan offers coverage for medical care, but you’ll have some out-of-pocket costs in the form of deductibles, copays and coinsurance. Group Voluntary 24 Hour Accident provides a benefit that you can use to pay for these expenses.

16. What injuries and services from a covered accident result in a benefit payment?

A partial list of injuries and services from a covered accident includes:

Initial hospital confinement

Intensive care

Dislocation/fracture

X-ray

Lacerations

Brain injury diagnosis

Paralysis: Paraplegia/Quadriplegia

Coma with respiratory assistance

Blood and plasma

17. How much does coverage cost?

To see a list of premium rates, visit the well-being pages found on InsideCHI. Cost is not determined by age or tobacco use.

18. How does the plan define accident?

An accident is any sudden unforeseen and unexpected event which occurs without the person’s intent which results in an injury to be covered independent of disease, infirmity, or any other cause. The plan may even cover things like allergic reactions, food poisoning, and bee stings.

19. When does coverage for me and/or my dependents begin under the accident plan?

Coverage begins on the first day of the month in which deductions begin unless you are not actively at work. The coverage effective date will be listed on the employee certificate.

20. If I leave the company, can I keep my accident coverage? What is the cost, and how do I go about

keeping the plan?

If you leave the company or if the company chooses to no longer offer the accident plan, you can continue coverage for yourself and your dependents as long as you make premium payments

directly to Allstate Benefits. Payments need to be received by Allstate Benefits within 30 days of your certificate termination date. The certificate termination date is the last day of the period for which any required premium payments were made. For example, if you leave the company on September 8 and your final paycheck with a portion of Allstate Benefits premium deductions is issued on September 15, your certificate termination date would be September 15.

21. I plan on retiring next year. Can I enroll in Accident coverage this year and take it with me when I

retire?

Yes. Your first premium payment must be made through payroll deduction and remitted by CHI to Allstate Benefits. After that happens, you may port your coverage at any time.

22. When can I submit a claim for benefit payment under my accident coverage?

You can submit claims for covered benefits any time after the coverage effective date.

23. How do I submit a claim?

You can obtain a claim form at https://www.allstatebenefits.com/ClaimForms.aspx. Instructions for submitting a claim are included on the claim form.

24. Are there medical questions to answer to enroll in this coverage?

The policy is offered on a guaranteed issue basis. That means you do not need to answer medical questions to enroll in this coverage.

25. Is there a waiting period?

There is no waiting period to receive benefits. For example, if your coverage effective date is January 1 and you are in a covered accident on that date, you are eligible to receive benefits if you submit a claim and the claim is approved.

Group Voluntary Critical Illness Coverage 26. What is Group Voluntary Critical Illness Coverage?

This plan pays a lump-sum benefit if you or a covered family member is diagnosed with a covered critical illness.

27. Doesn’t my medical plan cover critical illnesses?

Your medical plan is different from Group Voluntary Critical Illness. Your medical plan pays for a portion of covered medical expenses if you are diagnosed with a critical illness. Group Voluntary Critical Illness, however, is not medical insurance. It offers financial protection in the form of a cash benefit you can use to help cover medical costs that your medical insurance doesn’t cover or pay other expenses.

28. Whom can I protect by enrolling in Group Voluntary Critical Illness Coverage?

You can choose one of the following options

Employee

Employee + Spouse/Legally Domiciled Adult (LDA)

Employee + Child(ren)*

Employee + Family

*All children under age 26, regardless of marital status or financial dependence, can be covered under the plan. Disabled dependents can remain on the plan after age 26 and may also join the plan at any time as long as they are approved as a disabled dependent per the plan rules. Children are enrolled at no additional cost but you must select either Employee + Child(ren) or Employee + Family and select coverage for each dependent child when electing coverage.

29. What coverage options are offered to me?

You can enroll in one of two coverage options:

$15,000 maximum benefit amount.

$30,000 maximum benefit amount.

Your covered spouse/legally domiciled adult (LDA) and/or dependents receive 50% of the benefit you would receive. For example, you enroll in the critical illness plan at the $30,000 coverage option and your spouse/LDA suffers a heart attack. You submit a claim and if the claim is approved, your spouse/LDA would receive a $15,000 benefit.

30. How does Group Voluntary Critical Illness Coverage protect me?

A critical illness affects your health and your finances. Your expenses may include out-of-pocket costs for medical care, travel to and from treatment, new or additional prescriptions, and follow-up appointments. Group Voluntary Critical Illness Coverage offers a cash benefit directly to you if you or a covered family member is diagnosed with a covered critical illness. You choose how to use the payment.

31. How are Group Voluntary Critical Illness Coverage benefits paid?

If you or a covered family member are diagnosed with a covered critical illness, you will need to file a claim. If the claim is approved, the benefit payment is mailed directly to you. It is not mailed to a doctor, hospital or medical insurer unless you request that. The benefit can be used for any expenses you have, from deductibles, copays and coinsurance, to everyday costs such as housing, utilities, food, or child care.

32. What types of illnesses are included in Group Voluntary Critical Illness Coverage?

Here’s a partial list of covered critical illnesses:

Heart attack

Stroke

Coronary artery bypass surgery

Major organ transplant

End-stage renal failure

Invasive cancer

Carcinoma in Situ

Advanced Alzheimer’s disease

Advanced Parkinson’s disease

Benign brain tumor

Complete loss of hearing, sight or speech

33. What is the Pre-Existing Condition Limitation?

If you are diagnosed with a critical illness within 12 months after your effective date of coverage, benefits are not payable for a critical illness that is -- or is caused by, contributed by, or results from -- a pre-existing condition. A pre-existing condition is a sickness, injury or other condition, whether diagnosed or not, for which symptoms existed or medical advice or treatment was recommended by or received from a physician or other medical professional during the 12 months prior to your effective date of coverage. 34. What if I have a reoccurrence of a critical illness?

You’ll receive the same benefit amount if you or a covered family member is diagnosed for a second time with a critical illness for which a benefit was previously paid if:

The second date of diagnosis is more than 12 months after the first date of diagnosis for the initial critical illness; and

The second date of diagnosis is while the covered person is insured under the policy.

A covered person can receive the reoccurrence benefit only once for each initial critical illness.

35. How much does this plan cost?

Premium amounts are determined by age group, specifically by your age on the date your coverage takes effect. Note: When you enter the next age group, your premium will increase on the following January 1. For example, if you enter the next age group on July 21, 2019, your premium will increase beginning January 1, 2020.

36. Will I pay a different premium amount if anyone in my family uses tobacco?

There is no difference in the premium amount for tobacco users.

37. What is the Wellness Benefit and who is eligible for it?

The Wellness Benefit pays an annual cash benefit when you and your eligible dependents have a covered examination. Your eligible dependents are your family members that are covered under your Group Voluntary Critical Illness Coverage.

38. How much is the Wellness Benefit payment?

The Critical Illness Wellness Benefit pays $75 per covered person per year when you complete the wellness activity and then complete and submit the appropriate form (and any required documentation). Please note that the Critical Illness Wellness Benefit is separate from any earned wellness dollars from participation in the CHI wellness program.

39. What examinations are included in the Critical Illness Wellness Benefit?

Your annual Wellness Benefit covers specific examinations performed by a physician or physician’s assistant. Here’s a partial list of covered examinations:

Biopsy for skin cancer

Blood tests for triglycerides

Chest X-ray

Colonoscopy

Echocardiogram

EKG

HPV (Human Papillomavirus) Vaccination

Lipid panel (total cholesterol count)

Mammography, including breast ultrasound

Pap Smear, including ThinPrep Pap Test

40. Does my annual wellness visit with my primary care physician qualify for the Critical Illness

Wellness Benefit?

Annual wellness visits with a primary care physician do not qualify for the Critical Illness Wellness Benefit.

41. How do I file my Critical Illness Wellness Benefit claim?

You may file your Critical Illness Wellness Benefit claim online or submit it to Allstate Benefits via mail or fax. For complete instructions, visit www.AllstateBenefits.com/MyBenefits.

42. When does Critical Illness coverage begin for me and/or my covered dependents?

The first day of the month in which deductions begin unless you are not actively at work. The coverage effective date will be listed on your certificate.

47. What happens if I have a heart attack while covered under the Critical Illness plan and undergo

Coronary Artery Bypass Surgery 60 days later?

Each diagnosis must be separated by at least 30 days for both benefits to be payable. In order for any benefit to be paid, the diagnosis must take place after the effective date of coverage. Additionally, the conditions cannot be defined as pre-existing or excluded from coverage by the certificate provisions. 48. I am a cancer survivor; will I ever be eligible to receive the benefit for Invasive Cancer?

Yes, as long as the new diagnosis of cancer occurs after the coverage effective date and you have been treatment and symptom free for 12 months. Maintenance medications are not considered treatment.

49. What happens if a covered person dies while covered under the Critical Illness plan?

Allstate Benefits may make benefit payments for any unpaid and approved claims directly to the named beneficiary or estate.

50. What happens if I or a covered family member is diagnosed with a covered condition and do not

seek further treatment?

Benefits are paid at time of diagnosis regardless of whether you or your dependents seek further treatment.

51. Do benefits decrease as I get older?

Benefits will not decrease due to age.

52. My spouse/LDA and I are in different age brackets? Which premium amount do I pay?

Premiums are determined by the employee’s age, not by the spouse/ LDA’s age.

53. Are there medical questions to answer to enroll in this coverage?

Group Critical Illness coverage will be Guaranteed Issue during open enrollment up to $30,000. There are no medical questions to answer when you enroll in coverage.

PrivacyArmor 54. What is PrivacyArmor?

PrivacyArmor is a portfolio of products, services and support to help protect your identity and assist in fraud remediation and identity restoration. It is offered through InfoArmor, an award-winning leader in identity, financial and privacy protection.

55. How does PrivacyArmor protect me?

PrivacyArmor proactively monitors fraud by looking for any signs that someone is attempting to misuse your personal data. PrivacyArmor detects fraud in many scenarios, including wireless account activity, auto and mortgage loan applications, compromised login credentials, or anyone trying to gain access to your financial accounts, make fund transfers, or reset your passwords.

56. Does InfoArmor monitor the Dark Web if I enroll in PrivacyArmor?

Yes, InfoArmor uses state-of-the-art internet surveillance to search for any instances of your personal information being bought or sold on the dark web.

57. If I enroll in PrivacyArmor, what happens next?

If you enroll, you will receive a PrivacyArmor online account with the following resources automatically activated:

Identity monitoring and alerts

Credit monitoring and general alerts

Compromised credentials monitoring

High risk transaction alerts

Financial transaction monitoring

$1,000,000 identity theft insurance policy

Your PrivacyArmor online account will include additional features that you can choose to activate. These features include:

CreditArmor®. This feature goes beyond “free” credit monitoring services, providing you

with proactive alerts on any changes to your credit. It also provides access to monthly credit

scores and annual credit reports.

WalletArmor®. This feature helps you quickly replace the contents of a lost or stolen wallet.

Your credit card numbers, driver’s license, passport, professional license numbers and

insurance card data are stored in a secure online vault. Your wallet contents are monitored

for use in the underground internet and can be accessed on-the-go in the event of loss or

misuse.

SocialArmor®. This feature monitors some of the most common social accounts for

everyone in your family. It provides alerts on vulgarity, explicit content, violence, threats

and other potentially damaging content.

58. Is it safe to provide my personal data to InfoArmor if I enroll in PrivacyArmor?

Yes. InfoArmor has a comprehensive information security policy. Your data will be stored in a state-of-the-art data center. It is only accessible via secure, encrypted connections.

59. How do I know my identity is secure?

PrivacyArmor will send updates with your identity risk level and any active identity alerts. PrivacyArmor will alert you as soon as it detects an issue or suspicious activity. You’ll be able to discuss next steps with a Privacy Advocate if the activity is fraudulent.

60. If I leave the company, can I keep my PrivacyArmor coverage?

Yes, PrivacyArmor coverage is portable. If you leave the company, you will receive a notification from InfoArmor with instructions on how to continue your coverage on a direct-bill basis. This notification will be sent via postal mail to the address you provided InfoArmor. It will also be sent via email to the email address you provided InfoArmor.

61. How does PrivacyArmor compare to other services, like credit monitoring?

PrivacyArmor detects a different type of identity theft that is not related to credit accounts. PrivacyArmor’s identity monitoring provides a broader range of protection by monitoring for misuse of not only credit, but a variety of data sources. Credit is an important component in identity protection, but it represents a piece of the whole issue of identity protection. Privacy Armor’s service includes (free of charge) an annual credit report, monthly credit scores and credit monitoring.

62. When does my PrivacyArmor coverage become effective?

For employees enrolling in PrivacyArmor during Annual Enrollment (Fall 2018), coverage is effective January 1, 2018.

For new hires who enroll in PrivacyArmor, coverage is effective the 1st of the month following 30 days of employment.

For employees that are newly benefits-eligible and enroll in PrivacyArmor, coverage is effective the first of the month following the benefits eligibility date.

For Qualified Status Changes that enroll in PrivacyArmor, coverage is effective on the date of the Qualified Status Change.

63. How do I activate additional features of my plan like CreditArmor®, SocialArmor®, and

WalletArmor®?

Once your plan is effective, you will need to login to your online account and activate additional features of your protection in your online account such as CreditArmor, SocialArmor, and WalletArmor. Login to your account and click on the corresponding tabs for each feature for more directions. For additional questions about these features please contact the CHI HR/Payroll Connection Support Center at 1-844-450-9450 (8 a.m. – 8 p.m., EST, Monday-Friday).

64. When I activate the CreditArmor feature, will my credit score go down?

Activating CreditArmor will not negatively affect your credit. Since you are not applying for credit, a hard inquiry will not be placed on your file. Once activated you will receive credit monitoring, an annual credit report and monthly credit scores.

65. What should I do if my identity is stolen?

experts in identity restoration and are committed to doing the work necessary to restore your identity for you.

66. What if my Privacy Advocate can’t get a hold of me if they find out I have been a victim?

If your account is up to date with contact information, InfoArmor will alert you via email or text message (depending on your account settings) as soon as an issue is detected. In addition, a monthly email is sent to your email address with your identity risk level and any alerts that you have not taken action for. All active alerts are viewable in your online portal. If your contact information was not provided when you initially enrolled, a welcome letter was sent to your home with instructions on how to login to your account and input your contact information. It is highly recommended that you keep your account updated with some form of communication method, so InfoArmor can quickly alert you of suspicious activity. You can call InfoArmor at 800-789-2720 to review your account if you do not want to provide an email address or phone number associated with text messaging or do not have access to these communication methods.

67. Does PrivacyArmor provide a credit report?

Because InfoArmor realizes that credit is an important component to financial security, it also offers a free credit report each year, monthly credit scores, and credit monitoring to PrivacyArmor participants. Adding credit reports, scores, and monitoring will make the process even easier through InfoArmor’s one-stop shop portal. InfoArmor believes that credit is one component of your identity that should be periodically reviewed for accuracy as a check to ensure fraud has not occurred. Currently, InfoArmor reminds PrivacyArmor participants to pull their free credit reports through annualcreditreport.com three times a year to see a complete snapshot of their credit. This is a good complement to InfoArmor’s more robust identity monitoring solution.

68. Is the free annual credit report separate from the free report that can be obtained through credit

bureaus?

Yes, this is a separate benefit from what is provided as a free service through credit bureaus.

69. Is the credit score my FICO score?

The score provided by the CreditArmor feature is not a FICO score. The score displayed on your CreditArmor tab comes directly from TransUnion and is referred to in the industry as your TRANSRISK SCORE. While the FICO score is most commonly used in financial sectors to determine credit worthiness, the TRANSRISK score shares the same scoring range as FICO. Both scores have a

floor of 350 and an 850 ceiling. Scoring fluctuations with your TRANSRISK score follow similar rules to that of FICO but when a FICO score is accessed it also takes into account an Equifax and Experian score.

70. Why is the CreditArmor score so much lower than the score my bank provided?

Depending on the reason your bank was pulling your credit score, it’s possible they may be pulling a different type of score – with a different low and high – than the one InfoArmor provides. If your bank pulled your score to determine eligibility for a financial product or loan, it’s possible they pulled your VANTAGE credit score. While all scoring systems share a similar objective, the VANTAGE score utilizes a different range. A VANTAGE score can go as high as 990. The credit score we provide through CreditArmor will top out at 850.

71. Should I place a fraud alert on my credit bureau files?

InfoArmor only recommends the use of fraud alerts if you have a reason to believe that your identity has been compromised or our systems show that your identity is at a high risk of identity theft. InfoArmor utilizes industry-leading technology to monitor your identity.

72. What is Internet Surveillance?

The Underground Internet, also referred to as the Deep Web, is a place for cyber criminals to store and sell Personal Identifiable Information illegally. Internet Surveillance scans the Underground Internet for your personal information. Internet Surveillance scours an ever-evolving complex of over 30,000 compromised machines, networks and web services identified by InfoArmor and leading cyber security firms. Whether it is personal identifying information (i.e. name, address or Social Security number), a medical insurance card or an email address, Internet Surveillance is designed to find breached data and alerts you.

73. What is a Digital Exposure report?

This interactive, easy-to-read report not only summarizes what a real-time deep Internet search finds out about you, but also offers you a Privacy Grade with tips to better secure your information. Digital Exposure goes far beyond a typical Internet search and showcases your exposure on the Internet.

74. What is covered under your Identity Theft Insurance Policy?*

75. Who is included in the “Family” price?

InfoArmor’s PrivacyArmor benefit is available to those that have a valid Social Security Number. All ages are welcome and there is no age limit for children to enroll (i.e., infants all the way through adult children supported by their parents may be enrolled). Family coverage is available for individuals that are supported by you financially or live under your roof.

76. What if I currently reside in another country? Can I still enroll and receive protection?

One of the components of our monitoring service is your social security number. If you are residing in another country and are a US citizen or are from a US territory, InfoArmor can monitor your information for instances of fraud. InfoArmor will still be able to alert you if it finds suspicious activity associated with your identity or credit, however, the company is unable at this time to monitor for foreign bank accounts.

77. Is there an age limit for children to enroll?

There is no age limit for children to enroll. You can enroll your small children all the way through older children off at college (age 18+) or still living with you at home with InfoArmor’s “under roof, under wallet” definition of family. There is no age requirement for PrivacyArmor. The CreditArmor feature can only be activated for children that are 18 years or older.
