Patient Observations
Observations documentation provides details of ward, patientname, DOB, hospital number, consultant, frequency ofobservations.
Patients receive twice daily obs of temp, pulse, blood pressure,resps, level of consciousness and O2 sats, unless otherwisedocumented.
Patients are assessed against MEWS in accordance with Trustpolicy.
24 hour cumulative balances are evident on all fluid balanceobservations.
Evidence of documentation of referrals to medical staff for patientswith MEWS of 4 or more.
Observations repeated within 30 minutes when 4 or more.
