    Thank You To All Our Heroes. We are once again very pleased to report that the UnitedHealthcare

    Children’s Foundation (UHCCF) had an incredible year of growth and

    giving. We surpassed many of our goals and awarded more medical grants

    to deserving children than ever before. In total, UHCCF approved 2,003

    medical grants worth $5.4 million. That brings our total contribution since

    2007 to over 9,000 grants valued at over $27 million! It’s an amazing

    accomplishment. But, we’re just getting started.

    As great as the numbers are, they only tell part of the story. In this Annual

    Report, we’re excited to share the stories of several children who have

    benefited from UHCCF. You will read about our grant recipients and how,

    despite their medical conditions, all live very full lives. You will also get a

    chance to learn more about new strategies we have launched to help the

    Foundation grow.

    2014 was an amazing year and we can’t wait to use that momentum to

    grow our mission in 2015. We want to thank our national and regional board

    members, staff and numerous volunteers, who give selflessly throughout

    the year to UHCCF in support of these wonderful children and their

    families. You truly are heroes.

    Where Are They

    Now?

    IsabellaWhether swimming, swinging or boating, you can find

    Isabella with a smile on her face, especially when she

    gets to drive the boat! In school, you can find her

    enjoying her favorite subjects, reading and math, and

    socializing with her friends.

    Isabella has a genetic metabolic disorder with

    developmental delays of her motor system and

    processing. Doctors recommended speech, physical,

    occupational, metabolic specialist and psychiatrist

    visits. Isabella’s UHCCF grant assisted with the expense

    of these treatments and, ever since, she has shown

    incredible improvement.

    These days, she is able to walk on her own, no longer

    needing a wheelchair at school. Since receiving her

    grant, Isabella has also seen great improvements in her

    speech, allowing her to more confidently join in on the

    conversation with her peers. Way to go, Isabella!

    In 2014, we raised the bar – literally. UHCCF approved 2,003

    medical grants worth $5.4 million. This was made possible through individual donations, corporate

    contributions, our amazing fundraising events and sales of our ever-expanding line of

    merchandise. And, as you can see in the chart, the impact is being

    OUR 10,000TH GRANT!We don’t yet know who the child will be or how

    their life will be impacted. What we do know is

    that it will serve as an exciting benchmark for

    UHCCF and a reason to look back and thank

    all the amazing children (some of whom are

    now actually entering adulthood) and families

    who have welcomed us into their lives. And we

    also thank everyone who has helped make this

    accomplishment possible. That 10,000th grant

    will represent over $30 million in support.

    As a way to mark the occasion, we’ll be sharing

    some success stories – both past and present

    – within the pages of this Annual Report. Get to

    know our grant recipients and be inspired to help

    make more stories like theirs possible.

    HEROES AMONG US

    The UnitedHealth Group Employee Giving

    Campaign activity kicked off in September,

    reaching over 100,000 employees. The

    Giving Campaign allows the Foundation to

    share its story and make a direct connection

    with the greater UnitedHealth Group family.

    The campaign accounts for nearly two-thirds

    of UHCCF grant funds each year. It is a vital

    part of maintaining and growing the Foundation

    – and the 2014 campaign was outstanding. More

    than 9,100 UnitedHealth Group employees

    pledged nearly $3.6 million to UHCCF, which

    includes the UnitedHealth Group match. We are

    incredibly grateful for the ongoing generosity of

    UnitedHealth Group and its employees.

    KATELYN,AGE 8

    A “triple threat” doesn’t even come close to describing this motivated girl. Eight-year-old Katelyn describes herself as a

    dancer, singer, baseball player, gymnast and cheerleader.

    There is no slowing her down – a far cry from when

    she first came to the U.S. from China at the age of

    four and could barely walk.

    Katelyn’s cerebral palsy affects her gait, balance and vision. But, with the help of her first UHCCF

    grant, she has been able to attend weekly physical

    therapy, which helped her learn to walk, run,

    gallop, hop and even balance on one leg.

    Additional grants from UHCCF would help fund

    an adaptive stroller and an eye surgery

    to correct her vision issues.

    Katelyn’s condition doesn’t stop her

    from trying anything she wants and she

    prides herself on the fact that she

    never gives up. We can’t wait to see

    what she takeson next!

    Delivering Grins™ at Sales Pinnacle 2014.More than 230 of UnitedHealthcare’s top sales

    executives and their guests met in Palm Springs,

    California, in April to attend the Pinnacle Award event.

    Attendees assembled and created over 300 Grins to

    Go™ bags, part of the Delivering Grins™ program, for

    children at area hospitals and shelters.

    Each one of the bags would go on to help two different

    children – the child that received the bag and a child

    that would benefit through its proceeds. In total, the

    program immediately benefited 600 children. Each bag

    contained 14 gifts, including UHCCF’s new “Little Book

    - Big Laughs” joke book and other games and toys

    that brought a smile to the children at the Loma Linda

    Children’s Hospital, Ronald McDonald House, Mattel

    Children’s Hospital at UCLA and Shelter from the

    Storm organizations. A special thank you to all of the

    wonderful volunteers who participated in assembling

    the bags and made this happen!

    Another extension of our Grins to Go™ bag has

    been the successful launch of our corporate

    team-building and philanthropic program

    called Delivering Grins™.

    Through fun, collaborative team-building

    exercises, groups work together to fill bags, which

    are then distributed to local children. It’s a great

    way to give companies and other organizations an

    opportunity to do something positive and highly

    rewarding right in their own community.

    The Grins to Go™ gift bag has been a springboard to a number of exciting

    new initiatives – and has brought a lot of joy into the lives of some

    deserving children. Filled with popular toys and activities, each bag actually

    brings smiles to two children. One child receives a dose of laughter and

    inspiration through the gifts they find inside the bag. Another child will be

    impacted through a UHCCF medical grant, made possible by the funds

    raised by those who purchase the bag. One simple gift. Two very big smiles.

    Now This Will Make You Smile

    Growing from the success of the Grins to Go™ bag, the Foundation

    launched the Grins2 program in 2014. Working directly with a growing

    network of healthcare facilities across the U.S., we’ve made it easy for

    individuals or organizations to make a donation of our Grins to Go™ bags

    directly to the children that need it most. Already, over 250 children’s

    hospitals across the country are participating in this program, resulting

    in nearly $9,300 for UHCCF. Learn how you can help us impact even more

    The Fifth Annual UHCCF Golf Classic

    was held at the Hazeltine National Golf

    Club in Chaska, Minnesota, on August 4

    and raised approximately $1.25 million

    in funds for the Foundation. Two UHCCF

    grant families and more than 400 people

    participated in the fun, including two

    flights of golf, silent and live auctions, and

    many other activities. Julie Nelson, news

    anchor from KARE11, and Amy Hockert,

    Editor-in-Chief of BringMeTheNews,

    emceed the event. Two of our grant

    recipients, Jayden and Morgan, were the

    guests of honor and led the way as our

    brave grantee heroes.

    UHCCF Golf Classic Guest of Honor

    Jayden was diagnosed with a sinus venosus ASD and

    partial anomalous pulmonary venous return at birth.

    The UHCCF grant assisted with Jayden’s heart surgery

    and has helped offer him a better quality of life. A little

    boy with a great personality and sense of humor, his

    energy level has soared. His doctor said he will

    “go places.” We think so, too!

    UHCCF Heroes Swing Into ActionIn 2014, several UHCCF events brought out some big golf shots and even bigger fundraising numbers.

    Each year, these events have proven to be some of our most successful ways to raise funds for the

    Foundation, and this year was no exception. Read about a few of the highlights from each location.

    Superstars were on hand at the UHCCF Golf Classic at Manor Country Club in Rockville, Maryland, on

    September 8, hosted by Kevin Ruth and team. The list of participants included Washington Redskins

    Hall of Famer Charles Mann, Baltimore Ravens Cheerleaders and many local and national business

    leaders. But there was no bigger star in attendance than guest of honor, Jayden. When the day was

    done, $85,000 had been raised to support future medical grants for children.

    More than 180 golfers took to the links on September 29 for the Third Annual UHCCF “Day of Golf” at

    the Lake of Isles Golf Course near Foxwoods Resort in Connecticut. The golfers raised $92,000, which

    will go toward medical grants for children in the Northeast area.

    The worlds of art, athletics and business came together on

    October 24 at the IRONMAN® 70.3 Miami UnitedHealthcare

    Corporate Challenge Reception, held at the internationally

    acclaimed Miami pop artist Romero Britto’s gallery. Romero Britto

    and four-time World Triathlon champion Leanda Cave put their

    artistic and athletic talents together to create a one-of-a-kind

    bike. Britto lent his artistic talents and designed the art graphics

    on the frame of the bike that Cave rode in the 2014 IRONMAN®

    World Championship in Kailua-Kona, Hawaii, October 11. The bike was

    auctioned off with the proceeds supporting UHCCF.

    Each year, the UnitedHealthcare Pro Cycling Team selects one

    important race on the schedule to honor a lucky grant recipient in

    a special way. In 2014 that child was 9-year-old Chace, from Illinois.

    He’s a brave young third grader who loves to spend time on his bike.

    Chace’s medical grant from UHCCF has helped make the treatments

    for his hormone deficiency possible.

    As the team’s “Pro for the Day,” Chace received a full new “racing”

    kit, including a bike, helmet, team jersey and other team gear so

    that he could line up for the kids’ race in style! After an on-stage

    interview with his proud mother, Chace received a front row call-up

    in the men’s professional race, rolling out onto the starting line with

    UnitedHealthcare Pro Cycling stars Karl Menzies, Brad White, Hilton

    Clarke, Adrian Hegyvary and Ken Hanson.

    One-of-a-Kind Bike Auctioned on Behalf of UHCCF We Are...

    Heroes On BikesGoing the extra mile for children’s grants.

    In August more than 60 riders from UnitedHealth Group, UnitedHealthcare and

    Optum, along with key business partners and other UHCCF supporters, took park in

    a 100-mile cycling ride in Minnesota, beginning in Eden Prairie. In just the third year

    of this annual UHCCF Century Ride, the goal was to raise more than $175,000. They

    managed to eclipse that by raising $200,000 toward future medical grants.

    The UnitedHealthcare Pro Cycling Team continues to deliver more smiles with

    every mile. In addition to supporting UnitedHealthcare’s mission to help people live

    healthier lives and increasing employee engagement, this internationally-ranked team

    promotes and raises funds for UHCCF throughout the year at numerous races and

    events and contributes a portion of their race winnings to the Foundation.

    Even with the demands of professional cycling, the team continues to be

    UnitedHealthcare’s health and wellness ambassadors to local communities, including

    schools, children’s hospitals and community organizations across the nation. The

    team made over 40 appearances for UnitedHealthcare in 2014, personally reaching

    over 9,000 employees, community members and business partners.

    These Heroes Are Having A Ball

    The annual Rookie Club Bowling Fundraiser held in Atlanta was another great success!

    UnitedHealthcare and the Atlanta Falcons teamed up to score big for UHCCF. Grant

    recipient of honor, Jamese, was on hand to enjoy the festivities.

    The friendly bowling competition was followed by a silent auction, raising money to

    create more medical grants for kids like Jamese, whose UHCCF grant has assisted

    with the expense of medication for her severe chronic asthma. Thanks to all the

    UnitedHealthcare volunteers that made this amazing evening possible.

    This past summer the Mercer Island, Washington, office held their Second Annual

    “Kickin’ It for Kids” Kickball Tournament fundraiser for UHCCF, raising $15,000 for

    the Foundation. UnitedHealthcare Employer & Individual employees along with

    community members came together for this fun event and are looking forward to

    next year’s challenge.

    UnitedHealthcare Children’s Foundation shares insights on non-profit careers and

    children’s medical grant programs.

    Late in 2014, the UnitedHealthcare Children’s Foundation was one of 74 organizations

    that participated in the Great American Teach In program held at Tampa area schools.

    As part of Clark Elementary’s Great American Teach In, UHCCF’s Druanne McGraw

    shared insights about UHCCF and the important programs and services charity

    organizations provide to local communities.

    As part of the event, the third grade and special needs classes at Clark Elementary got

    a chance to get their hands on a lot of the fun new books and toys offered by UHCCF.

    JAMESE,AGE 10

    Marissa Marissa became a UHCCF grant recipient when she was

    fifteen. Born with Spina Bifida – a type of birth defect

    called a neural tube defect, which occurs when the bones

    of the spine (vertebrae) don’t form properly around

    part of the baby’s spinal cord – Marissa never let her

    medical condition stop her from doing what she loves…

    participating in and excelling at sports. The UHCCF grant

    made it possible for Marissa to receive a custom fit

    sports wheelchair, which gave her the speed, agility and

    confidence to play multiple sports.

    Now an 18-year-old high school senior, Marissa is playing

    wheelchair basketball at a college level and holds three

    Minnesota State High School Wheelchair Track and Field

    records, along with three National records. Twice, she

    was selected by the USA Paralympic committee as a High

    School All American for Track and Field. Marissa’s goal is

    to make the USA Women’s Basketball Team and with her

    relentless practice and determination, there is a good

    chance she will make it in 2020.

    HERO SPOTLIGHT

    NOAH,AGE 8

    Noah is a very active eight-year-old who has

    Down syndrome. Two grants from UHCCF

    have helped Noah receive speech and

    occupational therapy, helping him make great progress with his verbal

    skills. He’s now tying his shoelaces and

    buckling his belt on his own, boosting his

    confidence level.

    Friday is Noah’s best day of the week –

    movie night with his mom. He also has a love for basketball.

    LeBron James is his favorite player, and sometimes he

    even introduces himself as Noah LeBron James.

    He’s certainly “The King” in

    our book.

    HERO SPOTLIGHTAVA,AGE 11/2

    Ava is an eighteen-month-old courageous and adventurous little girl who enjoys going out on walks and taking nature hikes with her mom and dad. However, Ava’s true passion is the slide – she lights up with glee whenever she sees one.

    Ava was diagnosed with severe hearing loss and her UHCCF grant made it possible for her to receive hearing aids. Ava loves music and since she began using the hearing aids, she can now be heard singing nursery rhymes and even “Let It Go” from Disney’s Frozen. Keep on sliding and singing, Ava!

    * In 2014, our primary source of revenue, the UnitedHealth Group Employee Giving Campaign, made changes to how employee giving is managed. These changes affected how UHCCF may recognize the revenue under Generally Accepted Accounting Principles (GAAP). Even though overall UnitedHealth Group employee giving to UHCCF has remained consistent, recognized revenue for 2014 did see a one-time realignment. These reporting changes have not affected the cash inflows to UHCCF from UnitedHealth Group employees.

    HERO SPOTLIGHTCHRISTIAN,AGE 2Two-year-old Christian can spend all day outside playing in the dirt, riding his power wheel, or just running around and exploring. He never stops and loves getting into mischief! Christian was diagnosed with Craniosynostosis, a condition in which one or more of the joints between the bones of the skull closes prematurely before the brain is fully formed. UHCCF awarded Christian a grant for cranial vault surgery to give the brain adequate space to grow and develop. Christian’s mom gave us this recent update: “I definitely think that without this surgery it wouldn’t have been too long before we started to see signs of decline. Thankfully, we found doctors that correctly diagnosed Christian and the UHCCF grant made it possible for Christian to receive the surgery he needed.” We thank Christian’s mom for keeping us posted and look forward to a lot more updates in the years ahead!

    Exciting new product launches. 2014 marked an exciting year of new products being introduced

    that inspired kids and families, while helping raise money for

    future medical grants. Toys, books and more found their way

    into the hands of families across the country.

    Oliver & Hope™ returned in their second award-winning

    storybook, “Oliver & Hope’s Adventure Under the Stars.” This

    heartwarming camping tale has already gone on to critical

    acclaim, including a Mom’s Choice Silver Award. And, in 2015, the

    third book in the series. “Oliver & Hope’s Superhero Saturday”

    will continue to inspire by showing us all that anyone can be a

    hero. The Oliver & Hope™ series has inspired a number of other

    products from UHCCF, including several plush toys and a Spot it!

    game. Best of all, proceeds from these products have gone on to

    raise thousands of dollars toward new UHCCF medical grants.

    One of the biggest surprise stories of 2014 was the breakout

    success of our “Little Book – Big Laughs” Joke Book. Featuring

    hundreds of jokes submitted to UHCCF from kids and their

    families, it generated an enormous amount of press across the

    country, with numerous local profiles on the young jokesters

    that contributed to the book.
