The 2015 Awards will be held on the evening of the first day of the conference, the 24th. A new Award “Continuous Quality Improvement” has been introduced to promote the work of quality improvement across the Territory. The 2015 Award categories will be:1 Chronic Disease Health Promotion / Program Delivery Award (Team / organisation)

This award recognises a team or an organisation that has implemented a program that: Is based on the key principles of health promotion and that actively addresses chronic conditions

risk factors or the impact of chronic condition Has achieved significant improvement and / or innovation in health service delivery (quality

2 Aboriginal and Torres Strait Islander Health & Leadership Award – Male/Female

This award recognises an individual who currently works in the field of chronic conditions and has made an outstanding contribution to Aboriginal chronic conditions / health in the NT.

The person may work in an urban, rural or remote setting and across a range of roles such as Liaison Officer, Health Promotion Officer, Health Worker, Community workers, Women’s / Men’s Health, Child Health, Mental Health, Drug and Alcohol, Nursing, Trainees, planning, policy or research.

3 Outstanding Contribution to the prevention and management of Chronic Conditions in the Northern Territory (Individual)

This is awarded to an individual who has made outstanding contributions in the field of chronic conditions considering factors such as best practice, innovation, leadership, mentoring, advocacy roles and contributions to strategic planning, policy and research.

The recipient of this award will be invited to present a 20minute oration at the 2015 CDN Conference

4 Continuous Quality Improvement (Individual or organisation)

This is awarded to an organisation or individual that has achieved significant innovation and / or contribution in the area of continuous quality improvement with the aim of improving the prevention and management of chronic conditions

Having well trained teams can support our collective role in reducing smoking rates in the Aboriginal and Torres Strait Islander community and we would love for your team to bepart of our national effort.The 3-day Quitskills training provides participants with the knowledge, skills and confidence to support their community in reducing the burden of smoking. The training is nationally recognised, with three smoking cessation competencies awarded at a Certificate III or IV level. Since 2012, we have trained more than 600 people across Australia and participants recognise the importance of this training in supporting the work they do. Cancer Australia Cancer Australia is working with Menzies School of Health Research to develop a National Aboriginal and Torres Strait Islander Cancer Framework.The Framework aims to define evidence based priorities that can be used to inform the national cancer control agenda, and ultimately contribute to improved health and wellbeing for Aboriginal and Torres Strait Islander people with cancer and their families.To inform the Framework, Cancer Australia is aiming to capture a wide range of perspectives from Aboriginal and Torres Strait Islander people who have been affected by cancer, including their families and community. We also want to hear from all people who have a role in cancer control, including Aboriginal and Torres Strait Islander health workers, GPs, nurses, specialists, allied health practitioners and people who work in screening, prevention and palliative care.There are a number of ways you can get involved and provide your important feedback:
