Indicators of Success Toward Long-Term Goals

How will the success of the project be measured toward long-term goals? What measurable long-term outcomes would be considered successes? Indicators could include measures of water quality improvement, wildlife habitat re-establishment, social and economic benefits, scientific studies developed, air pollutants captured and amount of carbon sequestered.

5. Grant Narrative

The grant narrative is your opportunity to tell us about the project in your own words. Please use the Project Criteria as a reference for responding to the narrative questions. We encourage you to respond to the questions as clearly and succinctly as possible. We recommend that this narrative be 2-5 pages.

1. Project AbstractPlease provide an abstract that is concise, covering key project information (no more than 200 words).

2. Restoration Issues and StrategiesPlease discuss the key environmental and social issues your project is addressing and your strategies for restoring the site with tree planting. (See Project Criteria examples, such as site location and ownership; site conditions; project design; implementation and supervision; and long-term maintenance.)

3. Proposed Activities Please describe the key activities for implementing your overall project. (Criteria examples: project design and approaches; planting of diverse and native tree species; volunteer strategies and involvement; public education and outreach; and timeline or schedule of project activities.) NOTE: special consideration will be given to organizations that engage Alcoa locations in volunteer activities.

4. Restoration goals or desired outcomesPlease discuss environmental and social benefits you hope to achieve with your project and how you plan to monitor and measure them (e.g.; water quality and quantity, soil quality, wildlife diversity and habitat improvements, recreation, non-timber forest products.)

6. Additional Information

Please include the following attachments with your grant application.

1. Qualifications of Project Lead and Financial Officer: Please provide a brief description of A) the Project Lead’s experience and training for leading this restoration tree planting project and B) the Financial Officer’s experience and training for overseeing this grant.

2. Alcoa letter of support: Please provide a letter of support from the local Alcoa office serving as a partner for your project (see Sample Letter)

3. Project Budget: Please complete the budget form for your project (see below). Include revenue by type, expenditures by type and surplus or deficit.

4. Legal status of organization: Please attach your organization’s IRS Letter of Determination recognizing nonprofit status or equivalent country documentation.

5. Tax Form: W-9 tax form with employer identification number (EIN) (for U.S.-based organizations). www.irs.gov/pub/irs-pdf/fw9.pdf

6. Organizational Chart: Please attach a one-page organizational chart or staff list. Be sure that your Project Lead and Financial Officer are on your organizational chart.

7. Board List: Please attach a list of your organization’s board members with terms, occupations and places of employment.

8. Annual Board-approved organizational budget: Please include annual revenue, expense, and change to net assets. Indicate the dates that your budget begins and ends.

9. Annual Report: Please attach your organization’s most recent annual report (if available).

10. Audit Report: Please attach your organization’s audit report with two years’ comparative data. If you do not have an audit, please provide a statement of financial position (balance sheet) and a statement of activities (income statement) for the last two complete fiscal years. Also, please

submit your management letter. If you did not receive a management letter, write us a memo indicating this.

11. Interim Financial Statements: Please provide a copy of the most recent set of unaudited financial statements for your organization. At a minimum, please provide a statement of financial position (balance sheet) and a statement of activities (income statement). Feel free to submit one document that shows the statement of activities compared to the board-approved budget.1
