Athlete Nominee Profile Form
Instructions:
· The Team Manager will complete all details of this form (other than the Selector’s notes)
· The Team Manager will provide one Profile Details Form per athlete to each Selector
· This form must be completed as a hard copy, hand written by the Team Selector(s) at the time of trials (team sports) or qualification (individual sports)
· The Team Manager will collate the completed Athlete Nominee Profile Details Forms and submit these to your Regional Manager and copy in Laurence Miller, Sport Operations Coordinator, E: [email protected] within seven days of the completion of the trials
