Hounslow Social Prescribing Programme
Overview
September- October 2018
During this period, volunteers have interacted with 190 patients to support the social prescription service and three volunteers have been recruited to assist with call-backs and information research. The purpose of office-based volunteers is to follow up with patients the impact of their social prescription, if further support is required or if an urgent case requires signposting to statutory service provision. The service follows up with patients after one week and then three months or where required after 24hours.
Increasing capacity for the service is an on-going target and for this period a new intern has joined the service and more volunteers have been recruited. There has been a focus on recruitment through a volunteer day at west Middlesex University for students to discuss social prescription and opportunities to volunteer within the project. New adverts have been placed on indeed and total jobs for social prescription volunteers.
To promote the service the Self Care, Prevention and Volunteer Coordinator (SPVC) has attended all Locality meetings, a health day event at Chiswick to promote the project and interact with other stake holders, meetings with voluntary sector partners, including Age Uk, to discuss social prescriptions and how we can refer patients to their services and attendance at university fairs to promote volunteer opportunities (Westminster University, University of West London).
In total volunteers have interacted with over 3381 patients and dedicated 1446 hours since the project’s inception

This is the general information obtained during our initial conversation with the patients at the GP practice. The majority of patients spoken to have long term conditions, most of these patients require more information on how to manage their condition or community services they can get help from. 15% of patients spoken to wanted more information on healthy eating, diet and exercise, a majority of these being women. 12% of patients wanted more information on social activities within the borough, the majority of these being patients over 50 years of age. We have seen a rise in patients with mental health problems, most of these patients wanted information on depression as opposed to severe and enduring mental health. Patients with LTC seem to require more information on mental health services and social activities within the borough.

Summary for September-October
We have interacted with a total number of 190 patients A total of 380 leaflet/information and sheets were given out to patients We have 3 volunteers fluent in Punjabi, Hindi, Urdu, Somali and Arabic who have successfully engaged with patients within the
Hounslow and Great west Road Locality 30% of patients spoken to required more information on long term health conditions 6% spoken to required information on mental health services within this 4% were female 12% of Patients spoken to have diabetes, most had no knowledge of community services and classes available to them We have found 12% of patients interacted with wanted more information on social activities within the borough 6% of patients interacted with have lower back pain with no information on how to self-manage this We found 3% of patients have high blood pressure, and 3% with high cholesterol problems 4% of patients spoken to wanted more information on housing advice We have found the majority of mothers engaged with required more help emotionally. Volunteers found most mothers expressed
being generally healthy however found they required support regarding their mental wellbeing We have found 30% of patients have long term conditions Most patients with long term conditions had not heard of PAMS Most Patients consented to giving their contact details so the volunteers can follow up Volunteers interacted with 2 patients regarding domestic violence, one linked with mental health Most mental health advice given was linked to stress and depression Volunteers interacted with 2 patients in relation to refugee stress due to war trauma and possible PTSD

Most have knowledge on how to self-care but wanted additional advice on services available to them. Great west Road had one of the lowest percentages for mental health help, most patients spoken to regarding this, refused help but were still given information. Majority of carers spoken to within this locality were well informed on financial help for careers but none were aware of any emotional support or services within the borough. The lowest percentage in this locality was for Housing advice and mental health

Majority of patients spoken to in Chiswick are confident about their health and don’t require much information however 16% of patients spoken to required information on maternity and family support, most being mothers. Most mothers spoken to in Chiswick have a good support system of friends however of the 13% who required mental health information, most of these were mothers with young children .14% of patients spoken to wanted more information on diet and exercise however these were mostly women. 15% of patients had a form of long term condition, most of these had completed the PAMS questionnaire. volunteers found 6 patients who needed to complete the questionnaire. The lowest percentage in this locality was for careers. 11% of patients required more information on social activities most of these being over 60 years of age.

Patients with long term conditions have the highest percentage in this locality,10% of these are diabetic patients. Most diabetic patients spoken to attend diabetes services within the Feltham health centre. Patients who wanted social activities, wanted ones within their ethnic groups and culture. We found more people who had various types of addiction support, mostly smoking who wanted help to stop. Mothers who required help and advice regarding maternity and family ,wanted advice on housing and finance. Volunteers recorded a domestic violence situation within this locality. Volunteers completed 3 PAMS questionnaires in this locality.

The highest percentage in this Locality was for maternity and family support for mostly women. The number of Diabetic patients interacted with were quite low in this locality. The patients volunteers spoke to mostly wanted self-care information on type 2 diabetes. Volunteers interacted with patients from all ethnic backgrounds who needed help/advice with housing and finances. The lowest percentage was advice and help for carers who needed support. Most patients spoken to who were carers didn’t realise or acknowledge that they were classed as a carer. The majority were carers for family relations. 11% of patients spoken had long term conditions most wanted advice on how to self-care with these conditions.

The lowest percentage in this locality was support for carers. Most patients volunteers spoke to were caring for family members. More patients wanted information on specific LTC. Localities Comparison
Diet and exerciseGreat west road locality has the highest number of patients who required information and advice on diet and exercise, the majority of these were women and mothers from diverse ethnic backgrounds. Felltham however had he lowest percentage at 18%. Chiswick, Brentford and Hounslow had equal percentages on this subject.
DiabetesGreat west road had the highest percentage of patients with diabetes within all the localities. Diabetic patients in Feltham seem more aware of community services within the borough. Compared to the other localities Feltham diabetic patients have more knowledge on how to self-care with the condition. Chiswick, Brentford and Hounslow equally had the lowest percentage of patients
Social ActivitiesGreat west road has the highest number of patients in need of social activities. Most of these patients are over 60 or have long term conditions. Chiswick had the least percentage of patients in need of social activities, this may be due to the lack of activities in the area. We have found that Chiswick has the least options for social activities in the borough. Most activities found in Chiswick are not free and are usually run privately. Most Chiswick patients have expressed that Hounslow would be too far of a distance to travel for social activity.
Long term conditions supportFeltham has the highest percentage of patients with long term condition support. Hounslow, Chiswick, great west road and Brentford equally had the same at 15%. Most patients spoken to in general wanted more advice on managing a variety of conditions and additional support.
Mental Health SupportChiswick, Hounslow and Brentford had the highest percentage of patients who required more information on mental health support. Most patients we spoke to were mothers, women or patients with long term health conditions. Great west road had the lowest percentage in this subject area

Maternity and familyHounslow, Brentford and Chiswick had the highest percentage for Maternity and family support. The majority of patients asking for this support are women and mothers. Volunteers have found that this subject will then lead to housing and financial advice.
Carers SupportGreat west Road had the highest percentage in regards to information for carers. Chiswick, Brentford and Hounslow had the lowest percentage on this subject. We have found that most patients who are carers do not acknowledge this role as they are caring for a family relations. Volunteers have also found that most carers have financial support but have not received any emotional support for reasons hat were not mentioned to the volunteers
Addictions supportFeltham had the highest number of patients needing support and help with addiction. Most of these were related to smoking. A majority of these patients had tried to quit on their own without any additional help. Brentford had the lowest percentage in this subject area.
Housing AdviceHounslow, Brentford and Chiswick had the highest percentage for housing advice, the lowest being Great West road and Feltham. Most housing questions are asked during conversations regarding family and long term health conditions

Call Backs
Volunteers aim to call back patients within a week of initial conversation at GP practice. Where cases are more severe, volunteers call back within 24 hours.
We have seen a significant discrepancy between information required at initial contact with patients compared to when volunteers call back, for example:
36% of patients require more information on finance and Housing. 12% have required more information on adaptations to property due to long term conditions 8% required more information on emotional help for carers, most being young carers We have had 3 cases of Learning disability support Volunteers have referred a total of 12 Patients to Hounslow social services and the my independence team in Hounslow Volunteers have had to research social activities within different borough as some patients interacted with live in different boroughs
but have continued to stay with their GP in the Hounslow borough We have had to signpost patients to mobility help such as dial a ride as we have found the majority of patients spoken to do not
know of some of the mobility services available in the borough Volunteers are finding services that are no longer taking referrals therefore having to find other services outside the borough, for
example the befriending service in Age uk, Citizens advice bureau locations, One you Houslow park run dates and times, Domestic violence emergency numbers that are not working provided by Hounslow community safety partnership.
