o If you are scheduled for the Full External Evaluation (EEC), do not fill out this form but instead pre-populate the External Evaluation Committee Report (EEC-R). If you are not scheduled for an EEC this year, then you are required to fill out this Program Evaluation Report and submit no later than August 20, 2018. The Ministry will contact you about scheduling a Program Evaluation during September or October. If the school is due for a monitoring inspection, it will be combined with the Program Evaluation.

o Please attach a copy of the last Municipal Compliance letter from the local government/regional district to this application confirming there are no compliance-related concerns regarding the school facility and property. (Note: the Compliance Letter only needs to be updated prior to a Full External Evaluation (EEC); for all other inspections, the current Municipal Compliance Letter suffices. However, if the school program expansion also involves facility expansion/construction, then the School is required to obtain a new Municipal Compliance Letter and submit the same with this Program Evaluation report.
