What is included in Ray’s Coaching/Personal Training

As a seperate product, I offer Ray’s Coaching and Personal Training for those clients not particularly interested in the My Evolution system. Below I outline some of the differences between Ray’s Coaching and the My Evolution system.

Ray’s Coaching and Personal Training is not a system, it is a customised approach to training. As your coach, I do focus on your current needs and requirements, goal guides, target training and results.

I don’t push results and you have the flexibility to recieve smaller training packs, single sessions and lesser commitments to travel at a more relaxed pace. As an incentive you can purchase programs and diets customised to your goal.

The clients that may be interested in this product may be

- Fitness enthiusiats looking to gain new techniques and adjust their current form with multiple exer-cises .- Individuals not looking for a long term commitment but a more flexible approach to personal train-ing.- Clients seeking the latest training techniques, diets and philosophies that I am currently learning with my new coaches.- Clients seeking a monthly program with minimal training sessions due to their work and lifestyle. - Clients that wish to train in pairs on a combined goal, program and diet .

Unique products for clients of Ray’s coaching

Max Muscle Training - Train alongside a friend or stranger, group sizes vary, never more than 4 people. You will perform hard grueling workouts with equivalently strong athletes, work with a variety of new equipement, exercises and techniques that the professional bodybuilders use. Mentally challenge yourself and your friends!

Fit Therapy - Women’s only combination weights and conditioning session, the objective is to slim and tone those muscles along with recovery from a long week with fitness fun that is challenging. You begin the workout with a bit of motivation then run an awesome weight circuit on the gym floor, once completed we go back into the group studio room and perform a mix of exercises from military style obstacles and challenging partner events. This is an upbeat workout that we leave you smiling.

The My Evolution Starter Pack is a break down of the entire client-journey process, with help guides, fit-ness tools and more. Just a little background behind the My Evolution training system before you decide on which option you would prefer. I designed the My Evolution training system that took every aspect of my own personal althletic development, it contains influences of training science that I have been subjected to, from the best coaches in Australia. Most of all I designed this system to forge a brother and sisterhood of fitness enthusiasts who wish to remain focused, goal orientated in life and seek change in every aspect of themselves.

Who is The My Evolution training system for?

- Individuals who are goal driven and who hope to one day to become self-sufficent in designing their own diets and programs for what ever goal they wish

- Fitness enthusiasts/ beginners and seasoned athletes who seek a process where they challenge themselves to be better each day

- Individuals who want new and innovative programs that change on a monthly basis

- Clients who seek a greater personal experience with their trainer

- Clients who are devoted to their goals, long term - Clients who want greater rewards for there achievements while recognising the challenges they have accom-plished.

- Clients who want a training experience that will help to change their psychology, build a positive mind set and ultimately deliver on the promise of a full body transformation

In consideration of Raymond Neto agreeing to provide training and fitness activities and programs, I release Raymond Neto, its employees and representatives from any and all responsibilities or liability from injuries or damages resulting from or ancillary to my participation in any activities or my use of the equipment.

I understand and am aware that strength, flexibility and aerobic exercise, including the use of equipment, is a potentially hazardous activity. I also understand that fitness activities involve a risk of injury or death, and that I am voluntarily participating in these activities and using equipment with knowledge of the dangers involved. I agree to expressly assume and accept any and all risk of injury or death.

I further declare myself to be physically sound and suffering from no condition, impairment, disease, or other illness that would prevent my participation in exercise programs or use of equipment.I acknowledge that I have been informed of the need for a physician’s approval for my participation in and ex-ercise of fitness activity or in the use of exercise equipment. I also acknowledge that it has been recommended that I have a yearly or more frequent physical examination and consultation with my physician as to physical activity exercise and use of exercise and training equipment, so that I might have his/her recommendations concerning these fitness activities and equipment use.

I acknowledge that I have either had a physical examination and been given my physician’s permission to participate, or that I have decided to participate in the activity and use of the equipment without the approval of my physician and assume all responsibility for my participation in activities, and utilisation of equipment in my activities.Finally, I acknowledge and agree that no warranties or representatives have been made to my by any represent-ative of Raymond Neto regarding the results I will or may achieve from any program conducted by Raymond Neto. AGREEMENT & RELEASE OF LIABILITY

My Evolution PT and Ray’s Coaching and Personal Training has a goal to provide a successful experience and professional on-time services to all of my clients. For my business to be efficient in this way I have established the following policies to help all my clients benefit from my services:

1) 24 hour cancellation policy: My Evolution PT/Ray’s Personal Training and Coaching works on a scheduled appointment basis. Because your appointment time has been reserved especially for you, clients are required to give a minimum of 24 hours notice when cancelling appointments and I can immediately reschedule for another time. If an appointment is changed or cancelled within 24 hours (or missed) then a full session rate will be charged. This does not apply only to the following situations:- Serious illness or serious medical emergency- Accidents- Serious accident or illness of a client’s family, relatives, close friends

2) Medical release: If you have any medical conditions or have had a recent surgery, you will be required to get a medical release form from your doctor in order to begin an exercise program 3) It is a part of the My Evolution PT policy/ Ray’s Coaching and Personal Training that the client must give 3 weeks notice before canciling personal training sessions. This is to ensure the trainer has enough time to fill the position left vaccant. 4) Scheduling appointments: When booking appointments with My Evolution PT/Ray’s Coaching and Personal Training, contact me via email, phone or in person, ensure you are on time to the allocated area as delayed sessions will impede on other sessions that clients have arranged.

CANCELLATION POLICYMy Evolution PT/Rays Coaching and Personal Training understands there will be times when you are unable to keep your scheduled training session. If you need to cancel or reschedule a session, please call us on 0435 893 846. Please leave a message on the answering machine if we are busy with another client.Similarly, there may be a time when your trainer may be unavailable for a scheduled appointment due to un-foreseen circumstances. Should we be unable to provide you with another trainer at this time we will provide you with a free session for your inconvenience.In order to receive full credit for the session, 24 hours notice is required for cancellation. Failure to cancel within this time frame or failure to show up for a session will result in full payment. Exceptions will only be made in the case of a medical emergency accompanied by a doctors’ note.

TARDINESS POLICYClients are expected to begin working out at the start time of the scheduled appointment. A late start time does not entitle a client to a session longer than the scheduled appointment.

EXPIRATION POLICYEvery package purchased has a limited time to completion a minimum of 1 session a week. 4 session pack-ages retains a maximum time frame of 4 weeks unless notice of preceeding issues have been identified and related to my cancelation policy . 6 session packs retains a mximum of 6 weeks etc . Please note these packges are specials there by making it affordable for a diverse range of financial commitments but in no way is to be drawn out from the client without justification, if the client continues to press the exstent of the pack-age maximum aloted time frame as the coach and personal trainer the client may be told they will no longer recieve packages on a further bases. My Evolution PT/Ray’s Coaching & Personal Training Policy

My Evolution PT/ Ray’s Coaching and Personal Training needs a wide range of ‘personal information’ in or-der to carry out its business. ‘Personal information’ is essentially information from which you can be identi-fied. This Privacy Policy explains My Evolution PT’s/ Ray’s Coaching and Personal Training approach to the handling of personal information, and how we comply with our privacy obligations.

My Evolution PT and Rays coaching and personal training is bound by the National Privacy Principles con-tained in the Privacy Act 1988. If you provide us with information about your health, we also strive to ensure that our handling of that information complies with applicable State and Territory health records legislation.

What personal information does My Evolution PT and Ray’s Coaching and Personal Training collect and why?

My Evolution PT and Ray’s Coaching and Personal Training collects personal information from its members and from membership applicants, contractors, employees and people we do business with. My Evolution PT and Ray’s Coaching and Personal Training needs personal information:- to provide its members with personalized fitness services (incl. assessments of physical fitness, health status and goals)- To conduct our business (ie. to administer accounts, charge fees and to communicate with our members and business contacts)

To illustrate, the personal information we collect about our members includes:- Identification information including each member’s name, address, contact details (including telephone numbers and email address), date of birth and drivers license number- Health information including information provided on the pre-exercise questionnaire (that members com-plete upon becoming a member of My Evolution PT/ Ray’s Coaching & Personal Training and information we collect in the course of memberships. Health information is a sensitive form of personal information, so we only collect it with your consent, and we only collect what we need- Financial information including bank account details and credit card details, as well as information about payment or non-payment of membership feesWe need this personal information to provide health and fitness services and it is collected from application forms and when members communicate with us through our website. We also collect:- Business contact information from and about our suppliers and other people we do business with (ie. our landlords).

How does My Evolution PT use personal information?

We use personal information to provide fitness and health services, to manage and administer memberships and to operate our business. For example, if you are a member, will use your personal information to process your membership application,charge you membership fees and administer your account. We will also use your personal information to communicate with you, including answering any enquires you may have.Unless you tell us otherwise, we may also use members personal information to mail out special promotions, newsletters and other marketing material. For instance, My Evolution PT may mail its members information about its own promotions. If you don’t want us to do this, please advise us in writing.

Does My Evolution PT and Rays coaching /personal training share my personal information with anyone else?

My Evolution PT and Ray’s Coaching & Personal Training will disclose your personal information when working with certain contractors. For example,t we may engage IT contractors to maintain our databases. All our contractors are subject to strict confidentiality obligations, meaning that they are authorised only to use personal information in order to provide the services or to perform the functions required by us. We will also disclose personal information if we are under a legal requirement to do so (i.e. under a court order, or if required under legislation), or if an authorised request is made from a law enforcement agency. Otherwise, we will only disclose personal information with the consent of the relevant individuals.

How can I access my personal information?

Privacy laws give you rights to seek access to the personal information we hold about you. You can seek ac-cess by requesting it formally in writing. Your personal information will be provided to you within 2 working days of your request, although we may need more time if your request is complicated.We try to ensure that all the personal information that we hold about you is accurate, complete and up-to-date. However, we rely on you to help us with this. If you wish to update or correct the personal information we hold about you, please put the changes in writing.

Is my personal information kept secure?

Evolution PT and Ray’s Coaching & Personal Training takes reasonable steps to ensure that the personal information we hold is stored in a secure environment protected from unauthorised access, modification or disclosure. Your personal information is treated like confidential information.

Anonymity

My Evolution PT and Ray’s Coaching Personal Training only offers its health and fitness services to mem-bers, and it is not possible to become a member of Evolution PT and Rays coaching /personal training with-out providing us with the personal information that is requested in our application form and in our pre-exer-cise questionnaire.

MY EVOLUTION CLIENT POLICY

As a client with Raymond Neto you have decided to begin your fitness journey by utilising the services and products of My Evolution Personal Training, though there are some unique policy updates I would like to inform you about:

When introduced with the starter pack and product descriptions, including client transitions, payment packs and client incentives. You now are about to undertake a independent process with personal trainer Raymond Neto. To begin the client transition, the client must first understand the following below and sign off permis-sion to both withdraw and accept the product:

What are my steps

1) Client must have a clear understanding about the product and if it is right for them.2) Client must understand if they wish to accept the starter pack, the fee is $50 for production, discounted from $250.3) Once the starter pack has been created the client is to sign the non disclaimer form and understand the confidentiality clause and respect the intellectual material they now have.4) The trainer is to spend the first session sitting with the client laying out goals, forecasting the fitness jour-ney and selecting the appropriate programs required for the clients goals. 5) Mapping out the journey is the first part and the client now must update the trainer at each 3.5 week mark with what the next program is, and at the minum re visit the starter pack ever 3-4 weeks with the trainer in order to update progression details/ fitness tracking measurements/photos and more.6) Tier incentives are what you have earned and in all confidents not to be abused but utilsed for your goals currently.

IMPORTANT NOTE

-Clients that have been with Raymond Neto for a length of time are permissible to fast track the tier progres-sion only with Trainer Raymond Neto’s approval.

-If the client however after acceptabling and requesting the starter pack, revokes the product the client is requested to pay the cost of production as the item was produced for the needs of the client.

- Clients that are currently utilisng Ray’s Coaching and Personal Training for their fitness journey and wish to transition must still purchase a starter pack and understand the above points. NO exemptions . Though they can be approved for tier progression only with Raymond Neto’s approval.

- Clients can be on more flexible attending sessions at a max of 1 seession every fortnight with these payment packs , though can increase or decrease sessions per month at there request giving 1 week notice. Note the payment pack still apply for casual attendance .

MY EVOLUTION CANCELLATION POLICY

As per the general policy please notify me on both termination of personal training 3 weeks prior including when you want to discontinue the My Evolution Personal Training system. Understand that if you leave the system and personal training service on a given tier you will not be able to return to that same tier again but have to work from tier 1. If you wish to transition to Ray’s Coaching and Personal Training you can retain the starter pack/products and tier number, though will have to pay the rate required for this product.

PAYMENT CHANGES AND ITEM PRODUCTIONS

Please give some time for product delivery and payment changes, if you are paying fortnightly, monthly, weekly amendments will be made upon the next withdrawl after recieving the starter pack. Shirts and or any other merchandise may take as long as 2 weeks to procure even after purchase and reward, but I will endeavour to update you on progress updates during the process. With the first transaction amendment that includes the program and payments packs and starter pack cost in one , you will see see that price incuded in a single transaction there by after reducing with the fixed amount .

The purpose of the results principle in my business is the understanding that positive results per-petuates positive outcomes. So as my client testing processes, record keeping and transformation changes help to keep you guided on the path that is your fitness journey.

This symbol illustrates the training principle. As my client, you will often receive a personalised training program. It may be designed to gain strength, build mus-cle, lose weight, increase cardiovascular efficiency, work on weaknesses and much more relating to physical training. The purpose of the training principle is to create a mobile tangible training program set to your personalised objectives and level of experience. The personalised training program will aid in achieving your goal and help me to manage my training delivery so that you get the most out of the services I provide.

This symbol illustrates the nutrition principle. The importance of nutrition is undoubtingly the highest priority in any given goal, so as my client, you can be guaranteed a lot of nutritional coaching, diet creations, supplement advice and more. This principle in relation to your fitness journey is symbolic of the road you walk on and signs that lay before you, you can guarentee if you dont stick to the path you will walk off road and away from your goal. I have many methods of dieting, cleansing, detoxification , clean eating and supplement protocols that you will learn in the course of being my client.

This symbol illustrates the education principle. I believe as a personal trainer it is great to be a mentor, coach and teacher in one. Transferring knowledge to you is a great way to gain further value from my services, not just practical but theory based learning is of great importance for any goal. The educational principle means that as your trainer/coach I will provide you with both practical and writ-ten details relating to exercise instruction, diet and supplements.
