Healthy Savings ProgramHealthy Savings Program• Members residing within 30 miles of a
participating grocery store enrolled automatically
• All others may request enrollment by contacting Medica member service• After you receive your card you will receive weekly
emails with featured Healthy Savings items automatically loaded on your card.
• Shop for the healthy food items offered at participating grocery stores
• Have the cashier scan your Healthy Savings card at checkout to get your instant savings

Open Enrollment ProcessOpen Enrollment Process• Review the 2014 Benefit Decision Guide• Contact JA Counter for one on one assistance• Complete a Medica enrollment/change form to change your current
medical plan election• Current election will remain in effect if no form received
• If participating in the medical insurance, complete a HSA payroll contribution form
• Complete a 2014 Flex election form • Contract the District office if you wish to change any other plan elections

Changes outside of open enrollment will only be allowed if they are consistent with a Qualifying Family Status change & the District Office is notified within 31 days of the event
