How Do I Complete The 2014 Application
For Services?
Make sure you use blue or black ink only.
Title 18, Section 1001 of the U.S. Code makes it a criminal offense to make willful false
statements or misrepresentation to any
department or agency in the United States as to any
matter within it jurisdiction.
Please ensure you have COPIES of all documents listed on page 2 of 8
from the 2014 application for services. GCCSA does not make
copies.
Please keep in mind GCCSA will make no more than THREE (3)
attempts to contact you via phone.
