What makes a beautiful and healthy gingiva?

Bone and soft tissue are the most impor-tant factors for successful implant treatment. Bone can be regenerated using GBR and good quality bone materials. It takes some time but we can usually obtain a suitable quantity of bone for implant placement. Good implant design is also important to help over-come poor bone condition.

Soft tissue is also a key factor for esthetics and the longevity of an implant. It is more dif-ficult to create or maintain sound soft tissue than bone. Time is one of the most impor-tant factors for soft tissue. If we lose the chance to keep a sound emergence profile, the soft tissue will begin to deform and it is very challenging to regain its original form.

Większość czasu podczas zabiegów implanto-protetycznych poświę-camy na stworzenie dobrych warunków kostnych wokół implantów, a mniej uwagi poświęcamy procedurom związanym z tkankami mięk-kimi. Tkanki miękkie są jednak niemniej ważne i w strefie estetycznej mogą w niektórych przypadkach decydować o powodzeniu lub poraż-ce, nawet w sytuacjach, gdy sytuacja kostna jest bardzo dobra. Minima-listyczne podejście chirurgiczne pomijało często potrzeby związane z działaniami na tkankach miękkich prowadząc w niektórych przypadkach

do sytuacji, w których wiele wszczepów było wprowadzanych w strefach pozbawionych odpowiedniej ilości dziąsła przyczepionego, co kończyło się niezadawalającą estetyką różową. Na wykładzie omówione zostaną najważniejsze techniki stosowane w codziennej praktyce w celu wytwo-rzenia odpowiedniej ilości dziąsła rogowaciejącego wokół implantów, jak również procedury stomatologii estetycznej związane z tkankami miękkimi, mające na celu stworzenie ich idealnego profilu wokół implan-tów w strefie estetycznej.

Długoterminowe wyniki estetyczne terapii implanto-protetycznej zale-żą od odpowiedniej jakości kości odpowiedzialnej za podparcie tkanek miękkich, które z kolei są odpowiedzialne za ochronę tkanki kostnej. The Holy Grail of Immediate Implant Placement & Restoration in the Aesthetic Zone.

Single tooth implants in the aesthetic zone are one of the most challenging cases to execute well. Attention to detail is paramount for predictable, long-term success….but which method is best? Delayed placement was the norm, later some advocated immediate placement and loading. It is universally accepted however that after tooth extraction loss of soft and hard tissues occurs which can often result in suboptimal aesthetic outcomes. Therefore there are concerns that the buccal plate may still collapse in immediate placement cases. So what’s the safest and most predictable way to immediately place and restore implants in the aesthetic zone. More recently Partial Extraction Therapies has been advocated as a way to prevent the loss of buccal plate contour during implant therapy. So is this the holy grail we have been looking for?

This presentation will outline the concept of PET and socket shield and will showcase cases that I believe to be safe and predictable methods of immediately placing and loading implants for long-term success.

Immediate placement and loading, what’s in it for the patient and practice? 1 Surgery 1 Smile concept.The extraction socket represents a challenge in every day’s clinical practice. Maintenance of the hard and soft tissue envelope and a stable ridge volume are considered important aims and main advantages of immediate implant place-ment and immediate loading, allowing optimal functional and aesthetic outcomes.

There are two main benefits of placing implants immediately and using short implants; maintaining the ridge profile “preserving of the soft tissues contours” and eliminating the need for complicated ridge augmentation procedures.

Molar sites, and sites presenting big bony defects “non standard extraction sockets” still present big challenges because of the deficient ridge dimensions, however, minimally invasive techniques nowadays allow us to reduce the number of surgeries to “1”. Immediate loading protocols, for single implants, partial and full arches on implants will be discussed in detail providing objective guidelines.
