•Select from 3 or 6 month benefit period (provides financial protection before your LTD starts paying)
•Waiting period starting at 0 days for Accident and 7 days for sickness
•Partial/residual disability benefit
Key features of AccidentThis plan provides lump sum benefits to help offset unexpected medical expenses due to an on or off job accident. Optional spouse disability rider available up to $1,500 per month.

“Use it or loose it” rule:
IRS rule requires you to forfeit any unused funds remaining in your FSA account at end of eligible claim year.
