It is already three quarters of the term passed since I took over the office as the President, there are many activities happening so far, I shall highlight a few salient areas in this message.

Wisma MDA ProjectWhen the idea of Wisma MDA was first mooted, it was for the purpose of preserving MDA reserved fund against the sharp rise in inflationary pressure. We witnessed the crude oil price rose from USD30 a barrel to USD147 recently. It is a whopping 4.5 times increase in a span of 7 years and this has brought about worldwide sharp inflation. The hard earned MDA reserved would have been wharfed to less than half if we keep them in the bank. Hence the decision to launch the Wisma MDA project to acquire a property that would preserve the value of our money.

However when we got to purchase the property, we discovered that our cash reserve of two over millions hardly can buy any decent property. As a responsible Council, we would not borrow and subject the next Council to a heavy debt. We therefore took the plunge to raise fund to build our own Wisma MDA. This is a big dream as the amount involved is enormous. And to ensure the success of this project as well as not to burden future Councils, we therefore decided to set up the Wisma MDA Board tasked with the duty of raising fund, seeking government assistance as well as constructing the building.

Despite the noble intention, we faced sharp criticisms; Some members are both sceptical and critical. We took on the task, bite the bitter pill and work ourselves up step by step. It was in August when the Council met the Minister of Health YB Dato’ Sri Liow Tiong Lai at Putrajaya that we begin the see the light at the end of the tunnel. To our great surprise, our Honorable Minister of Health was very supportive of MDA’s noble intention and has agreed to help MDA to achieve our dream. My tears filled the eyes when the Minister told the MDA Council that he and his Deputy Minister YB Datuk Rosnah both had agreed to serve as Advisors for the Wisma MDA Board to assist the Dental profession in building our own home.

Indeed, mishaps are like double edge sword, if we grasp them by the handle, it helps us to solve the problem; However, if we view it negatively, it cuts painfully. With the prospect of potential government assistance in tax exemption as well as government land allocation, we decided to raise fund to meet our end needs. I wish to thank the Past Presidents Dato’ Dr Ratnanesan, Dato’ Dr Lim Chee Shin, Dr Wong Foot Meow, Dr Lee Soon Boon for their unwavering support. From my Council, I wish to thank all of them in particularly, Dr Muzafar, Dr Neoh Gim Bok and Dr Ng Woan Tyng for solidly behind me despite many criticisms and personal attacks.

Running an Association is like managing our own character, personality and temperament, we must be honest and truthful to ourself and only with that we could look the world straight in the eye and hold our head high. There are millions of things happening everyday and tonnes of decisions to be made, we planned each chapter course, each careful steps along the byway, sometimes we make mistakes but we excel by overcoming each obstacles.

Setting up Wisma MDA BoardWe have set up the Wisma MDA Board and invited senior prominent members to sit in the Board to oversee the fund raising, land acquisition, design and build as well as management of the building. This highly specialised task require specific knowledge and long term commitment. Ideally, there should be a constitutional provision to empower the Board to carry out this special mission.

The Council would consult our legal advisor to set up proper framework and guidelines into this aspect and we would table this for the House approval during the next AGM.

Why do we want to bid for the World Dental Congress 2015?Bidding for the World Dental Congress is not a simple task, it involves enormous preparatory works and coordination and most important of all, the courage to face it all. Why do we want to bid for the World Dental Congress? The reasons are as follows:

1. We need to raise fund the Wisma MDA, through the organization of this Congress, we are hoping to make RM2 million to meet the end needs of building the Wisma MDA.

2. We need to support the government’s policy of internationalization of Malaysia as a major convention centre in the world.

3. We need to groom future leaders for the Association, it is through organizing conferences that offer a good training ground to breed our future leaders.

4. We have strong government support for our noble objective.

5. We have strong support from the FDI and the international community to support Malaysia.

Capping the Professional Scale of FeesMany of us have mixed feelings about the government led effort to cap the professional fees. This effort form part of the Addendum for the Private Healthcare Facilities Act and is aimed at keeping the healthcare cost affordable and making Malaysia more competitive in our drive towards dental tourism.

Tremendous efforts has been put in getting all stakeholder to come out with proposal and resolving differences. Many meetings have been held, including 2 Emergency Meetings called by the MDA Council to resolve the differences and come to a consensus.

The Ministry of Health conducted a 3 Day Workshop to finalise the Scale of Fees. MDA has been instrumental in getting all specialties concerned to take part. I wish to thank our Honorary General Secretary Dr Neoh Gim Bok for his relentless effort in coordinating this uphill task.

MDA Northern, Southern & Eastern Zones AGMFebruary is a very busy month for the MDA Zones. Each Zone has held their AGM cum Zone Conference successfully. I am glad to see that the Zone AGMs had gone on smoothly and the new Zone Committee had been elected. We would like to welcome the newly elected Northern Zone Chairman Dr Goon Yong Por, Eastern Zone Chairman Dr Roland Chia. We also wish to congratulate the Southern Zone Chairlady Dr Angie Wong

for being re-elected as the Southern Zone Chairlady. MDA Council look forward to work closely with our Zone Committee and is currently looking into amending the Constitution to allow more autonomy for Zone Committee to organise Zone activities.

MDA plan to introduce Digital Voting System to facilitate AGM and ensure fair electionModelled from the FDI Parliamentary procedure, we have been studying carefully about the FDI Parliamentary procedures where hundreds of issues are being discussed without conflicts and personal attacks. FDI also introduce Digital Voting System where all votes are independently computed by the computer and presented instantly without error and dispute. This would be a reliable model for us to adopt to ensure fair, smooth and speedy election procedure without controversy. In addition, Resolutions and Constitution Amendment can be put to vote efficiently to enable smooth conduction of the AGM.

68th MDA AGM/ FDI World Dental Confederation International Convention & Trade Exhibition

I am proud to introduce to you yet another international Convention organized by the MDA. Under the able Chairmanship of Dr Mohd Muzafar Hamirudin as well as the Trade Chairman Dr Neoh Gim Bok. We are going to witness yet another record breaking MDA convention and Trade Exhibition. Ever since the 66th AGM, We have been working very hard to ensure each conference is better than the preceding one. I am very pleased that our conference has enjoyed a very good national and international reputation as quality and valued for money International conference. Many overseas

delegates look forward to attend our conference. Dear friends and colleagues,

Assalamualaikum dan Salam Sejahtera,

At the time I write this message, it reminds me that the next exciting event where we can meet or to be re-united with friends apart than seeking for new skills and knowledge in dentistry is just around the corner. The next 68th MDA AGM Scientific Convention and Trade Exhibition will be held from the 10-12 June 2011 at Kuala Lumpur Convention Centre. The theme for this year’s event

is ‘Current perspectives in Dentistry’. This is inline with the current rapid evolution in the field of dentistry. Delegates will be served with eminent speakers, both local and international. Just to name a few, we have Dr. David Sontag and Dr Jeeraphat to lecture on Endododontics, Professor Ferranti Wong who specializes in paediatric dentistry, Professor Angus Walls to share his expertise on geriatric dentistry and many more.

In my personal opinion, I have seen research in dentistry flourish for the past few years in Malaysia. MDA recognizes that research and scientific presentations have become part of KPI for staffs in MOH, MOHE and MOD. In order to support this development, MDA has taken the initiative to organize a platform for those who want to share their expertise and latest research findings. In line with this, I would like to invite all members to participate in the oral and poster scientific presentation in conjunction with the aforementioned event. Those who showcase their scientific research

will also be given the opportunity to publish their work in the MDJ.

Apart from the exciting scientific programme, the organizing committee is striving very hard to ensure programmes such as MDA Informal Nite and MDA Gala Night will be the most memorable and fun events for all.

Last but not least, I would like to invite all MDA members to register early and join us for the upcoming June event.

Dear Members

It has been six months since I hold the post of Gen. Secretary of MDA. I would like to thank the entire membership for the support given to me.

For the last few months the two issues that cause anxiety among members were the scale of fees and the practice of orthodontics among general dental practitioners. Both issues were amicably resolved during a dialogue session held at KLCC on 16th January this year between members, MDA and the Oral Health Division of MOH.

The 18thFDI/MDA Scientific Convention and Trade has been a tremendous success. Thanks and congratulations are

due to the organizing chairman Dr. Teh Tat Beng and his team for job well done. I would also like to take this opportunity to remind members that CPD points may be mandatory for APC renewal next year. Please try to attend CPD events organized by MDA and its affiliates or associates to collect your points. On that note I would like to urge all members not to miss the 68th MDA AGM/FDI International World Dental Scientific Convention and Trade Exhibition. Dr. Muzafar, our president–elect and the organizing chairman has managed to arrange a number of prominent international and local speaker for this meeting.

Scale of fees. As announced by our honorable Health Minister that all professional fees of medical and dental will be capped on March this year. MDA had been tasked to come up the scales of fees for dental. The schedule was finalized at a workshop organized by MOH at Grand Blue Wave Hotel, Shah Alam from 20th to 22nd of Feb, attended by MDA and most of it’s affiliates and associates. It is now pending approval by the Health Minister. I would like to thank Dr. Chow Kai Foo, Dr. Jaswant Singh for representing MDA and all the affiliates and associates involved for their valuable contributions. I would also like to extend my heartfelt gratitude

to our President Dato Dr. How , Dr. Elise Morasinghe, Assistant Principal Director from the Oral Health Division and Dr. Mohamed Anis from the Amalan division for their guidance.

With regards to the administration of the secretariat, Ms Alice Kaur has left us for greener pastures and I would like to thank her for her contributions during her time with us. We have now employed a new staff, Ms Wennie Kok to take over her duties and she is now undergoing a probationary period.

Once again I would like to thank the membership, the council members and the secretariat staffs for making my job a meaningful one.Thank You

Dear esteemed friends and colleagues,

I hope this issue of MDA News finds all of you in good cheer. Without realizing it, more than half my term as Publication Secretary has elapsed. It has been a great experience so far as I have learnt a lot from my fellow principle office bearers

and MDA council members. I am certainly grateful to have their constant support and encouragement.

I am happy to report that the MDA News and MDJ have gone from strength to strength. We have received many article contributions from both local and international authors. The editorial team, with the guidance of our president, has also managed to diversify and increase readership, besides gaining more support from advertisers. Nonetheless, I hope that MDA members will keep on contributing to the newsletter and journal. We are here to be your voice and be a platform to share knowledge amongst our close-knit dental community.

On a personal note, 2011 has brought on the joys and challenges of motherhood for me. God willing, with the continued

support from the MDA team and improved time management skills, I will be able to carry on serving the association effectively.

Lastly, hope to see all of you at the upcoming 68th MDA AGM Scientific Convention and Trade Exhibition (10-12 June 2011) at KLCC Convention Centre!

Japan tsunami relief efforts!On March 11th Japan was hit by the largest earthquake in its history measuring 9.0 on the Richter scale. The massive earthquake resulted in a number of tsunamis that battered the coast of Japan resulting

in the loss of life of thousands of Japanese and massive amounts of damage to coastal communities. Whole villages have been washed away and vital infrastructure has been severely impacted. In light of these devastating events, we would like to ask for your help in supporting these communities in the relief efforts.

Our President, Dato Dr How Kim Chuan has already extended the MDA’s heartfelt concern and compassion to the good people of Japan in a recent letter to the President of the Japanese Dental Association, Dr Mitsuo Okuba. MDA has also pledged USD1000 to aid in relief efforts via the Japanese Dental Association (JDA). As we continue to pray for the Japanese community, we at MDA would like to call on our esteemed members to respond to this catastrophe by making donations to the JDA. In the spirit of kinship, let us show that MDA cares!

Japan tsunami relief efforts!

18th FDI/MDA International Scientific Convention and Trade Exhibition held in Kuala Lumpur Malaysia

A new year, a new beginning’…Malaysian Dental Association had started off 2011 with a “Big Bang!” with a successfully hosted event in the world class Kuala Lumpur Convention Centre (KLCC) from 13-16 January 2011.

The History of FDI/MDA Conjoint Continuing Dental Education (CDE) Conference

The history of FDI/MDA Conference started off in 1994 under Dr Choo Teck Chuan who was then the FDI CDE Programme Director and Dato’ Dr Ratnanesan who was then the President of the Malaysian Dental Association. Malaysian Dental Association was one of the first organizers as well as initiator of this conjoint CDE programme with the FDI. The success of this conjoint Conference with the FDI subsequently become a role model for many countries to follow as a major CDE activities with respective members of the FDI.

MDA has decided to go international in June 2010 in response to the call by Minister of Health of Malaysia

The Minister of Health Dato’ Sri Liow Tiong Lai in June 2009 posed a challenge to the MDA to elevate the national conference to an international level. The Council has decided to respond to this challenge and elevated the 67th MDA AGM/ FDI Conference to an international conference level on 10-13 June 2010.

In our maiden attempt, the MDA has successfully attracted 2300 local and international delegates from more than 32 countries attending the Conference and attracted more than 4000 visitors and 180 Trade Exhibitors participating in the Trade Exhibition. This Internaitonal Conference was officiated by our Deputy Prime Minister YAB Tan Sri Muhyiddin.

The success of 67th MDA/AGM MDA/FDI Convention has inspired us to elevated the 18th FDI/MDA Conference ,as well, to an international level. We have decided to ride on the high note of our previous success and did achieve, yet another success in our second attempt into the international foray, which was the 18th FDI/MDA International Scientific Convention and Trade Exhibition in 13-16 Jan 2011.

The MDA’s theme for the event this year was “Clinical Dentistry: Today and Beyond”, chosen because Dentistry is a fast changing professional, and we are living in an era of informative technology explosion which is affecting our dental fraternity one way or another.

In the world of Dentistry, Change is inevitable. New material is being invented, new techniques and procedures are being developed every day. By attending Continuing Professional Development Programme (CPD) like 18th FDI/MDA Convention is one of the ways to keep up with the advancement in Dentistry and to provide better care for our patients.

The annual FDI/MDA convention goes from strength to strength and once again in 2011 the number of participants has reached record level.• Over 1500 participants• Over 200 Exhibition Booths

With the fervent promotional efforts by the organizing committee, the convention has managed to attract over 2800 delegates and visitors over the 3 day’s event. This includes over 350 international delegates from USA, Australia, Brunei, China, Cambodia,

France, Hong Kong, India, Indonesia, Japan, Germany, Korea, Pakistan, Portugal, Philippine, Singapore, Thailand, Singapore, UAB, Uruguay and Malaysia. International traders came in full force to market their latest products and services. The MDA has received overwhelming positive feedback from exhibitors, the majority of whom have committed to participating in future MDA conventions.

The Scientific programme was indeed a great success, with 20 internationally renowned speakers who shared their expertise at the state of the art, Plenary Hall, which has a capacity of 3000 seat and the Plenary Theatre with the capacity of 500 seats, at Kuala Lumpur Convention Centre.

In line with MDA’s tradition of promoting professional excellence and fellowship amongst dentists, dental auxiliaries and students, there were numerous other programmes organized concurrently: the Oral and Poster Competition, Lunch Symposium, Dialogue with MDA member on the current issue, Dental students performing during the Wisma MDA Night and the Grand Opening Ceremony.

Hands-on Clinical WorkshoA total of 7 Pre-congress Hands-On was held successfully at the state of the art Dental Simulation Laboratory at USIM, Hospital Tuanku Mizan, Dental Division Malaysian Armed Forces, DEMC Specialist Hospital and Osstem Training Centre on the 13th January 2011. All the lectures, hands-on workshops were very well attended.

Wisma MDA Building Fund Raising Dinner The highlight of the conference is the launching of the MDA Building Fund. This noble project of building a home for the Dental Profession is supported strongly by the Minister of Health Dato’ Sri Liow Tiong Lai. The Minister has announced that the government of Malaysia is granting MDA 10 million tax exempted BuildingFund and had personally donated RM20,000 in support of this noble project.

The Minister is very pleased that MDA has taken up his challenge by elevating the national conference to an international level and he has now posed another challenge to the MDA to bid for the FDI World Dental Congress in 2015. The Minister assured MDA the government’s full support for this Congress and appeal to the FDI President Elect Dr Orlando Monteiro Da Silva to support Malaysia’s wish.

Over 520 attendees attended the Wisma MDA Night dinner and we are very proud to announce that we had successfully raised RM 520,000 for MDA Building Fund on that evening under the name of “One Malaysia, One Dental Family”. Many of our non-dental colleagues have also donated generously to this noble cause.The President of Malaysian Dental Association Dato’ Dr How Kim Chuan also announced in the evening about MDA’s plan to offer FDI World Dental Federation as well as Asia Pacific Dental Federation (APDF) the new Wisma MDA Building as their Regional Headquarters once it is completed.

The Secretary General of FDI Dr Oliver Hennedege praised MDA’s effort as significant player in the region as well as one of the important Founder members of the APDF. The Health Minister congratulated MDA for having successfully bid for the Commonwealth Dental Association/FDI Convention 2012 at Kuching and the 35th Asia Pacific Dental Congress (APDC) at Kuala Lumpur. It is time for MDA to bid for the FDI event now. The Health Minister had reiterated the strong support of the Malaysian Government for MDA to bid for the FDI 2015.

He cited that MDA had successfully hosted the FDI World Dental Congress with a record participation of 15,000 delegates in the year 2001.

Report of Oral Health Awareness CampaignMalaysian Dental Association (Northern Zone),working together with dental industry-MorningKiss, had launched Oral Health Awareness Campaign on 30th and 31st October 2010 at Sunshine Farlim Supermarket in Penang. Morningkiss had kindly given away door gifts to all participants. Students from PIDC also helped in the dental screening programme.

The theme of this campaign was “Towards A Lifetime Of Oral Wellness”. The theme was aptly chosen because oral wellness is closely related to one’s overall general health.

This campaign aimed to:• Increase the awareness of the public that quality of life is improved with

oral wellness• Increase awareness that oral wellness required control of risk factors that

contribute to oral diseases• Facilitate early detection and treatment of oral diseases• Reduce the incidence and impact of oral diseases• Inform individuals of their oral health problems• Inculcate habits of effective oral health practices, including regular professional care

This campaign was officiall launched by Penang State Exco of Health and Welfare, Caring Society and Environment, YB Phee Boon Poh. Various activities were held, including free public dental screening, public talks on oral health awareness, video presentation on oral healthcare technique, talent quest and trade exhibition of oral healthcare products.

Throughout the campaign, the public response was overwhelmingly good. A total of 1380 individuals attended for the free public dental screening and received professional consultation on matters pertaining to oral health and wellness and education on oral hygiene.

In conclusion, the objectives of this campaign had been achieved. Being a national professional body, MDA Northern Zone has a strong commitment to community service and our members will continue to fulfill our social responsibility to serve the public and improve the oral health condition of the community.

All-ceramic crowns for incisors with discolored margins

All-ceramic materials are increasingly utilised by dental professionals, improving aesthetic dentistry and demonstrating truly life like restorations. While restorations with discolored substrate in anterior region still remains agreat challenge. They are often caused by microscopic leakage due to open margins, tetracycline stains, Non precious metal restorations, posts and so on. Nasser Shademan (CDT) explains how to achieve the goal of masking the dark shadow without causing unnatural opaque looking margins.

Preparation of laboratory working models and collection of adequate information concerning aesthetic and occlusion including digital dental photos assists the lab ceramist to produce appropriate provisional restoration that offers a great opportunity for dentist, dental patient and ceramist to study and visualize possible problems that is likely to threaten the success of aesthetic all ceramic restorations.

Achieving beautiful all ceramic restorations begin with discovering what is simply not beautiful in window of oral aesthetics. Here the author utilizes water color techniques to demonstrate a Helpful method in planning aesthetical ceramic build-up guide, a map to assist ceramist with additional knowledge, to apply the right portion of ceramic with appropriate chroma, hue or, value at right parts of each tooth in a similar way that nature does! So they appear as if they actually belong there. Remember that often to beautify the nature is to apply modification without completely changing its biological, natural character!

Preparation of pressed ceramic caps is done considering all aspects of aesthetics such as tooth position, smile line, midline, gingival position in aesthetic zone, facial structure elements relevant to color –form and texture of teeth in oral window, discoloration and its impact on prepared tooth, etc.

The case presented here demonstrates inadequate preparation (on margins) of patient’s natural tooth that results in insufficient room for conventional pressed all ceramic layering technique and at the same time there are sever discoloration on margins of prepared teeth. Failure in understanding the optical behavior of different all ceramic materials will also limit the ability of dental ceramist to choose the right material for right case.

Masking the dark influence of underlying tooth is planned by measuring the room available for layering technique according to final contour of aesthetic restorations, after which ceramist could better decide on the sufficient amount of pressed core thickness to accommodate space for alternate layers of ceramic.

The desired level of chroma in opaque dentine will be achieved adding mixture of white, orange, ochre, brown stains into it , note that the nature of original shade should not be disturbed. Knowing roles and science of ceramic color to replicate individual shade is important and needs not to be compromised.

Selecting the right brush move to closely mimic the unique proprieties of natural tooth structureminimizes failure and provides wonderful experience as ceramic build up is completed.

Dentine color effects needs to be placed as planned based on information collected formexisting neighboring teeth. Careful study of overall composition, feminine or masculine indications, tooth texture & body anatomy could assist ceramist to reproduce natural aesthetics in harmony with oral environment.

Aesthetic and Conservative Dentistry

Porcelain has been used for veneers with greater frequency than direct composite resin restorations due to the lack of understanding by clinicians relative to translucency, shade, and the value of composite resin materials.

Quality aesthetic restorations require not only perfect hue, chroma, and value to match their vital counterparts, but also fluorescence, opalescence, and translucency.1 Obtaining an optical parallel with nature can be elusive as well as frustrating. Due to the complexity of shade selection, increasing numbers of dentists rely on their ceramists to replicate the natural tooth using porcelain rather than direct techniques.

The use of high-strength, noncompromising bonding agents has, however, enabled long-term retention and direct composite restoration with larger Class IV lesions. Using this type of bonding agent in conjunction with a

quality composite can maintain the integrity of margins, even superseding the strength of cement and luting resinsin indirect restorations. The sacrifice of conservative dental treatments and healthy tooth structure for more aggressive procedures in the name of aesthetics has improved in a relative sense. For some time, veneers facilitated a less invasive solution when compared to the full-coverage crown. It is, however,more common for clinicians to provide less invasive restorations using microhybrid composites to offer a level of beauty, strength, and ease-of-handling never before achieved. Novel restorative systems, in conjunction with high-strength, noncompromising bonding agents, facilitate the most conservative approach to aesthetic tooth enhancement. The following case presentation demonstrates a minimally invasive, direct, aesthetic approach to the restoration of a Class IV fracture in the anterior region, using a microhybrid composite (eg, Vit-l-escence, Ultradent Products, South Jordan, UT) that exhibits favorable optical properties, adequate shade selection, strength, and optimal handling characteristics.

Case Presentation A 12-year-old male patient presented with Class IV fractures in the mesial aspects of both maxillary central incisors (Figure 1).

The patient was experiencing discomfort due to a recreational accident and was dissatisfied with the resultant aesthetics. Neither pulp exposure nor root fracture was evident. The fractured teeth were quickly restored with composite resin to facilitate tooth whitening prior to definitive restoration. The maxillary teethwere whitened for seven days using an at-home 10% carbamide peroxide gel in a sustained-release formula (ie, Opalescence, Ultradent Products, South

Jordan, UT). The patient returned for definitive treatment two weeks following completion of the bleaching procedure, which enabled the adverse effects of bleaching-on-bonding to dissipate and allowed the shade to stabilize.

A light-cured gingival barrier (ie, Opal Dam, Ultradent Products, South Jordan, UT) was applied to the adjacent soft tissue (Figure 2), and a diamond bur was used to remove superficial defective enamel prior to the application of a microabrasive slurry (ie, Opalustre, Ultradent Products, South Jordan, UT) (Figure 3).

The provisional composite restorations were removed, and a long bevel was placed (Figure 4). An alginate impression was made to facilitate a chairside mock-up of the restorations using a quick-setting stone (ie, Snap Stone, Whip Mix, Louisville, KY). A heavy-bodied compositewas used to establish proper lingual contour (Figure 5), while a flowable composite (ie, PermaFlo, Ultradent Products, South Jordan, UT) was used to augment the marginal ridges and refine the incisal corners. The lingual anatomy was finessed using a football-shaped, fluted finishing bur followed by a polishing point (Figure 6). A silicone template (ie, Exafast, GC America, Alsip, IL) was fabricated from the mock-up and trimmed facially to the incisal facial edge. The use of composite resin instead of wax in the mock-up allowed the clinician to trim the material quickly prior to light curing to increase durability. In this case,

a light-bodied impression material was used in the set putty to capture finite lingual anatomy (Figure 7). The putty template allowed accurate developmentof lingual contour, incisal length, and proper width.

The template also provided the clinician with a stable surface to apply the lingual layers. A filled bonding resin (ie, Composite Wetting Resin, Ultradent Products, South Jordan, UT) was then

placed in the template to facilitate finer detail pickup.

To prepare the teeth and surrounding tissue for the bonding procedure, sulcular fluid control was obtained with a pH-protected ferric sulfate (ie, Viscostat, Ultradent Products, South Jordan, UT) and a retraction cord (ie, UltraPak, Ultradent Products, South Jordan, UT). The teeth were etched, and a bonding agent (ie, PQ1, Ultradent Products, South Jordan, UT) was applied, air thinned, andlight cured for 20 seconds (Figure 8).

The lingual and mesial aspects were established by placing the composite resin in the template prior to curing for 20 seconds. The thickness of the enamel and dentin layers was determined by following the pattern exhibited by the natural tooth. A translucent composite material was subsequently applied to the incisal edge to provide a more natural effect (Figure 9). Each layer was light cured for 20 seconds, and the matrix was removed. The dentin layer was established with shade A1 at the middle third of the tooth and B1 at the incisal one-third of the tooth.The final facial enamel layer was then added. Mamelons and developmental lobes were established using an interproximal carver prior to polymerization.

A flame-shaped finishing bur (ie, 49L, Brasseler USA, Savannah, GA) was used to refine the anatomy (Figure 10). A green polishing point (ie, Ultradent Products,

South Jordan, UT) was used to soften thefacial mamelon depressions (Figure 11). Final finishing was achieved with a medium-abrasive polishing cup (ie, Jiffy Yellow Cup, Ultradent Products, South Jordan, UT) followed by a fine-abrasive polishing cup (Figure 12). A superior polish was added with a polishing brush (ie, Jiffy Brush, Ultradent Products, South Jordan, UT), using a slow-speed handpiece and light pressure on a dried surface. The restorations and the adjacent enamel were subsequently etched for five seconds, rinsed, air dried, and sealed with a light-cured composite sealant (ie, PermaSeal, Ultradent Products, South Jordan, UT). Once the definitive restorations were reviewed, it was determined that the incisal aspect of the restorations appeared overly opaque, and insufficient surface texture remained following polishing (Figure 13).

Using a parallel-shaped finishing bur, a thin layerof facial composite was removed. The 35% phosphoric acid etchant was placed on the freshly cut composite and adjacent enamel for 15 seconds to clean any contaminants from the surface of the composite as well as to etch the enamel. The etchant was rinsed and excess moisture was removed. A bonding agent (ie, PQ1, Ultradent Products, South Jordan, UT) was then applied to the composite and tooth surfaces, air thinned, and light cured for 20 seconds. A translucent layer of composite was applied to provide improved light transmission in the incisal region.

The use of coarse- and medium-grit abrasives was omitted during the retreatment phase to enable retention of the microanatomy and promote a more natural appearance. The anatomy was refined with a finishing bur (ie, 49L, Brasseler USA, Savannah, GA), and the interproximal areas were polished with a fine polishing strip (ie, Epitex, GC America, Alsip, IL) prior to etching and final curing (Figure 14).

ConclusionThe use of a microhybrid composite material allows the clinician to use a minimally invasive approach for the three-dimensional restoration of compromised structures, simulating the anatomical and visual requirements of hue, value, chroma, translucency, and opalescence. New technologies and repairability together provide economic, caring, and patient-centered directions for operative dentistry. With a few adjustments to traditional approaches using resin materials, a tremendous improvement in the final result will become feasible: conservative dentistry with beautiful outcomes.
