World Institute of Pain (“WIP”) was conceived in 1993 for the purpose of alleviating disparate standards of pain practice through better physician educational and practical training programs specifically oriented towards interventional pain practice. Today, WIP is recognized as a global symbol of excellence that is reflected in how WIP executes its mission. WIP efforts are realized through the unification of physicians, scientists, and industry specialists who believe that working together in the global arena more can be achieved for the greater good of all mankind.

Through WIP, member and non-member physicians, laypersons and industry supporters of WIP programs, collaborate to optimize the exchange of cutting-edge scientific evidence and clinical practice experience in interventional pain practice.

The WIP official magazine of Pain Practice fulfills a need for scholarly, evidenced-based articles written by global experts in the field of pain medicine.

Through the WIP Fellow of Interventional Pain Practice certification program, pain physicians are choosing to demonstrate via the competency-based curriculum of rigorous oral, written, and practical examination exercises that pain medicine is a specialized field that warrants dedicated training and recognition to ensure the best practice of interventional pain medicine and the best outcomes for patients worldwide.

The advent of WIP’s Excellence in Pain Practice (EPP) Award for pain centers is elevating the scope of FIPP certification from an individual measurement of quality and expertise in pain medical practice to a global symbol of excellence. FIPP directors of EPP pain centers and their associates are assuming the roles of mentors and educators in their local regions. Through the WIP network of EPP pain centers, global advancements of pain medicine can be optimally achieved in more regions of the world.

The World Institute of Pain truly brings together the most recognized experts in the field of pain medicine throughout the world for the advancement and standardization of interventional pain practice. Through this guiding mission, physicians collaborating throughout the world are achieving higher standards of care for pain patients.

The World Institute of Pain (“WIP”) was founded in 1993 and incorporated in 1995 in the State of California of the United States of America. WIP is a non-profit corporation, exempt under Internal Revenue Code section 501(c )(3). Since 2007, the WIP Secretariat was established in Winston-Salem, North Carolina, USA.

Mission Statement

WIP’s mission is to bring together the most recognized experts in pain medicine throughout the world for the advancement and standardization of interventional pain practice and the achievement of improved standards of care for pain patients.

WIP fulfills its mission through its organization, sponsorship, and endorsement of global educational and training programs for pain physicians. These program are aimed at facilitating the development of practice guide-lines and standards of examining and assessing competency in physicians who specialize in interventional pain practice.

Aims and Objectives

To educate and train personnel of member pain centers via local hands-on training, international seminars, and exchange of clinicians. Update pain centers with state-of-the-art pain information, including a newsletter, scientific seminars, interlined telecommunications, and publication of a journal and books.

To encourage members of pain centers to develop an informational database and protocols for efficacy and outcome studies.

To communicate administrative and patient-related matters on a regular basis by way of newsletter, telephone hook-up, world directory of pain centers by region, and video teleconferencing (including patient consultation).

To categorize and credential pain centers by mail correspondence, local information and the industry’s medical representatives.

To develop an examination process for pain centers in testing trainees, and provide technical expertise in the examination process.

To encourage interested industries to provide information on pain medicine in each region of the world; to bring together local pain physicians and industry for education in new technology and training; and to formulate an advanced fellowship program in pain medicine.

Dear Members and Supporters of WIP,

As a professional society organized for the purpose of bringing people together to promote the best practice of pain medicine, the aims of WIP are both high and attainable. High, because we have chosen a global forum to channel our goals for excellence in pain medical education, training, and clinical practice. Attainable, because as a society, the strength of WIP is in its network of thousands of physicians worldwide who share our belief that working together we can achieve more to pro-mote standardization and advancements in interventional pain practice.

Through WIP’s global network, real progress is being made through-out the world to advance the standards and recognition of physician edu-cation and practice in pain medicine. WIP’s multidisciplinary network of professionals encompasses associate members; full members who have attained the professional status of Fellow of Interventional Pain Practice, many of whom serve as leaders and mentors to current and next genera-tion pain specialists; pain centers of Excellence in Pain Practice; scholars who contribute to the official journal, Pain Practice; and participants and supporters of WIP pain conferences, workshops, and World Congresses.

Through the WIP Sections, the diffusion of our networking efforts is cultivating at local and international levels the true power of our ability to serve the pain medicine profession and achieve real outcomes improve-ments for patients throughout the world who suffer daily with the silent syndromes of chronic or acute pain.

The World Institute of Pain is a society of professionals who join hands to fulfill a common purpose of realizing better pain education for physicians and better outcomes for our pain patients. The World Institute of Pain (WIP) was founded 23 August 1993 in Paris, France at the Palais du Congres during the International Association for the Study of Pain World Congress. Drs. Prithvi Raj (USA), David Niv (Israel), Ricardo Ruiz-López (Spain), and Serdar Erdine (Turkey) held the first meeting about the creation of WIP. On 13 October 1994, Dr. Gabor Racz (USA) met with Drs. Raj, Nov, Ruiz-López, and Erdine during the American Pain Society meeting in Washington, DC. With a $1,000 contribution from each, WIP was officially chartered in 1995. WIP operates as an autonomous entity, independent of any other association, society, or academy. The first WIP secretariat was established in 2007 in Winston-Salem, North Carolina, USA.

World Congresses

Since its founding, WIP has organized five World Congresses: Eilat, Israel (1998); Istanbul, Turkey (2001); Barcelona, Spain (2004); Budapest, Hungary (2007); and New York, USA (2009). The sixth World Congress will be conducted in February 2012 in Miami Beach, Florida, USA.

Regional Symposia and Workshops

As of December 2011, WIP has sponsored 16 cadaver workshops in Budapest; four international symposia and eight cadaver workshops in London, UK; and sponsored or endorsed pain conferences and workshops in Sharm El Sheikh and Cairo, Egypt; Athens, Myconos Island, and Santorini, Greece; San Juan, Puerto Rico; Brussels, Belgium; Maastricht, The Netherlands; Kolkata and New Delhi, India; and in the USA in New York, New York; Cleveland, Ohio; Memphis, Tennessee; and Lubbock, Texas.

Fellow of Interventional Pain Practice

Since 2001, the FIPP Board of Examination has conducted ten examinations in Budapest, Hungary; three in Memphis, Tennessee, USA; one in New York, USA; and one in Maastricht, The Netherlands. A total of physicians representing 38 countries have earned certification as a Fellow of Interventional Pain Practice (FIPP). eFIPP™, the electronic application for the FIPP examination was introduced in 2010 to better facilitate the application, payment processing, and administrative processes associated with the FIPP program.

Pain Practice Journal

Pain Practice, the official journal of WIP, was first pub-lished in 2001. It has been indexed in Medline since 2006, and is in its twelfth year of publication by Wiley-Blackwell.

WIP Sections

In 2007, WIP established 17 official Sections representing regions where the FIPP population is greatest. In 2010, four more countries qualified for and granted Section status in 2011: Canada, Colombia, Hungary, and Iran. Through these 21 Sections, WIP focuses its core activities and growth strategies to optimize fulfillment of its mission.

EPP Award

In 2010, WIP introduced the Excellence in Pain Practice (EPP) Award program for pain centers. In 2011, EPP Awards were conferred upon 14 pain centers representing 11 countries. The EPP Award recognizes pain centers that demonstrate the highest standards of pain practice. Varying levels of recognition are attainable, including specializa-tion in a particular area of clinical pain practice, multidisciplinary clinical pain practice, and comprehensive recogni-tion that encompasses all dimensions of pain management including cutting-edge research.

Communication and Media

WIP’s newsletter and website are WIP’s windows to the world, and showcase the many ways that WIP, through its global network of pain physicians, is fulfilling its mission. Through WIP’s media outlets, WIP aims to demonstrate that those who affiliated with WIP share a common vision to provide better education for pain physicians and deliver better relief for pain patients around the world.

Committees of the WIP are aligned with specific organizational activities to facilitate fulfillment of WIP’s aims and objectives. At least one member of the Executive Board serves on each committee. The President is an ex-officio member of all committees.

6th World Congress (2012)

WIP is delighted to have chosen Miami Beach, Florida, USA as the venue for its 6th World Congress to be held February 4-6, 2012. Since its founding, WIP has held five successful World Congresses in Eilat, Israel; Istanbul, Turkey; Barcelona, Spain; Budapest, Hungary; and New York, USA.

WIP’s combined focus on participant-friendly activities together with hands-on courses has led to the development of novel interventional techniques for the management of pain. At the same time, WIP has fostered consensus building amongst pain experts on the effectiveness of existing techniques and on avenues for further improvements in therapeutic performances. The 6th World Congress will be an opportunity for participants to hear and meet renowned lecturers and share their experiences from all over the world.

The Congress chairs, scientific and local organizing committees, are collaborating to prepare a comprehensive program representing the very latest evidence-based and best-practice-based pain medicine in all areas and disciplines in the field. WIP is anticipating a highly successful and exiting Congress. Participants will have the opportunity to exchange ideas and expertise in an international framework, meet friends and colleagues from all over the world, and listen to lectures to be delivered by prominent leaders in pain medicine.

In addition to participating in a stimulating scientific congress, participants have opportunities to see a little of Miami. Although best known for its sun, surf, and sand, Miami Beach is also a vibrant cultural and entertainment center renowned for its Art Deco architecture and designer fashions.

WIP looks forward to welcoming the international pain physician community to Miami in 2012!

Introduction

Established March 2008, members of the Advisory Board are selected by the Executive Board based on experience in the pain field and contribution to WIP activities. Advisory Board members serve for a two-year term and can be reappointed for only one additional term.

The Advisory Board Chairman serves as an at-large member of the Executive Board.

Responsibilities

The responsibilities of the Advisor Board are to:

Promote membership of WIP in their region;

Promote WIP activities in their Section;

Promote publications of their Section’s members’ work in Pain Practice;

Plan and organize WIP educational programs;

Introduction

WIP Sections were established in 2007. In 2011, four new Sections were added to the original 17, bringing the total number of Sections and associated Council Chairs to 21. The four new Sections are: Canada, Colombia, Hungary and Iran.

Each Section has a Chairman, Vice-Chair (optional), Secretary, Treasurer, and individual members. Individual members may be Regular (FIPP) members or Associate members. Only FIPP members may serve as Chairman or any other Section officer.

The Chairman of each Section serves as a WIP Council member. One of these Section Chairmen also serves as a Member-at-Large of the Executive Board.

Responsibilities

Promote membership of WIP in their region;

Promote WIP activities in their Section;

Promote publications of their Section’s members’ work in Pain Practice;

Plan and organize WIP educational programs;

Encourage members to attend WIP educational programs; and

Solicit appropriate members for the FIPP Board of Examination, the Editorial

WIP encourages the establishment of regional Sections when such Sections would further the objectives of the WIP. The Executive Board has the authority to control the establishment of Sections and to establish regulations for this purpose. Individual Section members are required to have common membership with WIP.

Sections may only be formed by FIPP certificate holders and a minimum of five (5) FIPPs are required in the country or region desiring to form a Section.

The WIP Section of Africa held its annual WIP Cadaver Workshop from 18-20 March 2011 at the University of Stellenbosch Anatomy Department.

Guest lecturers were Dr. Andrea Trescot (USA) and Dr. Leland Lou (USA).

Topics covered included the present syllabus for FIPP examinations. Three C-arm workstations and one ultrasound workstation with four wet cadavers were used to teach 18 students over a 20-hour period. Both Drs. Trescot and Lou were struck by the high propor-tion of orthopedic surgeons (five) and neurosurgeons (six) present on the course. Further-more, we had an interventionalist radiologist present.

Four physicians earned FIPP certification in 2011, more than doubling the number of FIPPs in the Section of Africa.

Discussions are underway between Drs. Trescot, Lou, Eric Wilson, and I to develop a WIP fellowship for Africa since such a fellowship does not exist in Africa. The idea is to have a prerequisite of three WIP workshops as an entry level to the one-year modular course. The course will consist of six modules of which three will be done in Belgium, Holland, and the UK, respectively; with three modules in the Republic of South Africa at three accredited pain clinics. This will ensure that WIP Africa Section candidates are well prepared prior to sitting for the FIPP examination.

Furthermore, a Pain Training Academy is being established at the University of Stellen-bosch Medical School with its own C-arm as a permanent ongoing training facility to train under- and post-graduate students.

Prospective pain practitioners in neighboring countries of Nambia, DRC, and Ghana have been identified to establish outreach WIP pain clinics in MBBch (Wits) FRCS (Glas)

FIPP (USA), FRCS (Edin) these to ensure adequate pain services in these third-world countries.

The second cadaveric workshop performed in the anatomy lab of Ain Shams University on the 18th June 2011 was a big success.

Under the flag of the Egyptian Society for the Management of Pain all FIPP certified Middle East Section physicians conducted a discrete hands-on training on basic spine interventions to 40 young pain clinicians from all over Egypt governorates. At the end of this workshop, every participant had a CD explaining the techniques they learned. Candidates requested clearly their demands for regular repeated training .

The WIP Section of India is directed by its elected officers: Drs. Gautam Das, Chair; Subrata Ray, Vice-Chair; Pankaj Surange, Secretary; and Karthic Babu Natarajan, Treasurer.

The International Conference on Interventional Pain Management 2011, organized by the WIP Indian Section and conducted at Chettinad University near Chennai on September 8-11, 2011 attracted an international audience of approximately 500 pain physicians representing the USA, Europe, Latin America, and Asia. The theme of the conference, Safety in Interventional Pain Practice, delivered an evidence-based curriculum aimed at maximizing patient safety and efficacy of pain management.

As Organizing Chairman, Dr. Ravi Krishna Kalathur, aptly stated, “Pain medicine is a young and rapidly-evolving science. It is rapidly gaining acceptability as an essential and independent specialty in the West and in India. In keeping with these rapid strides, it is essential to translate the scientific advances to our day-to-day practice of interventional pain management.”

“It is also important to keep interventional pain management an absolute evidence-based science with highest standards in safety during clinical

practice,” said Organizing Secretary, Dr. Karthic Babu Natarajan. Dr. P. Prithvi Raj, Founder and Past Presi-dent of WIP was invited to inaugurate the event and delivered an inspiration-al talk aimed at inspiring young pain physicians to become engaged in the study, practice, and scholarly exchange of interventional pain techniques. Dr. Raj was also honored with “The Prithvi Raj Oration,” delivered by a pio-neering pain physician. Dr. Raj stated that, “The conference was divided into scientific sessions with a parallel review course, ‘Ask the Experts’ session, and a cadaver train-

ing course. The auditorium was ultra-modern with all tech-nical facilities for a major conference.

The cadaver workshops were extremely well located, the sessions were well controlled, and the meeting was always on time. The cadavers were excellent for teaching purposes and the faculty taught the registrants all interventional procedures in accordance with WIP standards. The interna-tional faculty represented WIP well and I was told that they were happy to participate.” Raj also characterized the social activities as well organized for the international faculty and their wives, with the highlight of the meeting being the Gala Dinner with a grand concert of Indian dancers.

Hospitality was excellent and sincere, and the attendees were very keen on learning about pain management.

WIP President, Dr. Ricardo Ruiz-López declared, “WIP’s Indian Section has emerged as an integral educational center of excellence, eroding barriers to access to the best pain education for physicians not only in south Asia, but globally as well.” “It was the biggest conference on pain in more than 25 years of history of pain medicine in India and also in this

part of Asia,” said Dr. Gautam Das, WIP Chair for the Indian Section. Das added, “It was truly international and biggest in terms of total numbers of participants, international faculties, international participants, quality of cadaver workshops and number of workstations, and ultimately truly satisfied feedback from participants and faculties. The term ‘excellent’ has been used so many times by almost all participants and all this was possible, because of a team of young and dynamic pain physicians of India, inspired by the WIP International team and senior Indian pain physicians lead by Prithvi Raj himself. The country of Iran experienced significant growth in 2011, qualifying it for independent Section status. Dr. Mahdi Panah Khahi was elected Section Chairman. Dr. Farnad Imani and Fasihi Harandi were elected Section Secretary and Treasurer, respectively.

The 2nd International and 4th annual ISRAPM (Iran Society of Regional Anesthesia and Pain Medicine) Congress was held September 28-30, 2011 in Tehran, Iran.

A wide spectrum of topics on the captioned subject were covered by an international faculty. A group of experts and opinion leaders, from overseas and from Iran, delivered and exchanged experience and scientific knowledge.

The 2011 Congress was designed to reinforce the basic and clinical practices in the field of interventional pain treatment, and included lectures, panel discussions, workshops, and poster presentations.

The WIP Section of Australia experienced significant growth in FIPPs within its small

section, with four new FIPPs bringing the total to 15.

One pain center, Metro Spinal Clinic in Victoria, Australia applied for and received and EPP Award for Clinical Pain Practice Specializing in Musculoskeletal Pain Syndromes. The Section acknowledges the efforts of its FIPP colleagues Bruce Mitchell, MBBS, FIPP; Paul Verrills, MBBS, FIPP; and Robert Gassin, MBBS, FIPP.

Although there have been some informal discussions with individual members, there have been no official meetings or sessions at local meetings.

WIP Benelux proves successful in promoting quality in pain management Increasing number of FIPP’s

WIP Benelux has now 58 FIPPs. Each year pain physicians are motivated to participate in the FIPP examination. The alumni are always prepared to help the candidates with the preparation, which has proven to be successful because the success rate is very high. The third year in a row the Samuel Hassenbusch Prize was won by a WIP Benelux member, with Dr. Jan Willem Kallewaard who had the highest composite score in 2011.

Already two centers with the EPP award

Multidisciplinair Pijncentrum Lievensberg Ziekenhuis, Halsteren, The Netherlands, Direc-tor: Kenneth van Tilburg and David van den Tol received the excellence award for multi-disciplinary clinical practice.

Multidisciplinair Pijncentrum, Ziekenhuis Oost-Limburg, Genk, Belgium, Director Jan Van Zundert and the team: Martine Puylaert, Pascal Vanelderen, Pieter De Vooght and Roel Mestrum received the award for comprehensive multidisciplinary pain practice.

Continuous efforts for training and education

Besides the ad hoc training sessions mostly organized in the multidisciplinary pain center of the Ziekenhuis Oost-Limburg, Genk, Belgium, WIP Benelux organizes again an interventional symposium and hands-on workshop. During the 2011 edition of the symposium and workshop, the scientific presentations ran parallel with the hands-on workshop. Several participants

expressed the regret that they had to miss part of the congress; therefore the scheme is changed for the 2012 meeting. For 2012, the theoretical session will be elaborated to provide participants with an update on the anatomy, diagnosis and management of cervical facet, discogenic, and lumbosacral radicular pain. Thanks to the cooperation with the neurologist, Dr. J. Patijn, the physical examination of the cervical and lumbar area will be developed. The anatomists, A.

Lataster and A. Herrler make sure that relevant preparation are available to allow participants to study the different important structures for correct and safe performance of interventional pain management techniques. Furthermore several stations with fresh-frozen cadaver specimen equipped with fluoroscopy will be available for demonstra-tion and practice of the techniques. Six participants per station ensure everyone gets the opportunity to practice. The WIP examination for FIPP will be organized in Maastricht in June 2012.

Evidence-Based interventional pain medicine according to clinical diagnoses

The chair and vice chair of WIP Benelux are respectively first and last editor of the book: Evidence-Based Interventional Pain Medicine, according to Clinical Diagnoses. Several WIP Benelux members contributed to this work that has the objective to assemble the up to date information relative to diagnosis, additional investigations, differential diagnosis, conventional and interventional management for the clinical diagnoses most prevalent in a pain clinic. This work is based on the original book that was pub-lished in Dutch. Thanks to the cooperation with Nagy Mekhail and Craig Hartrick the information of

each chapter was adapted by at least two experts for the US, thus giving the content an international aspect. The majority of the 61 authors have acquired the FIPP certificate. The hand book is recommended literature for the preparation of the FIPP examination.

A good representation and activity planned in Miami

During the 6th World Congress of WIP in Miami Beach, several WIP Benelux members will act as speaker, trainer, and examiner. Several posters have been submitted from WIP Benelux members. On Saturday evening the members and the editors of the handbook will host a get together, where the publication of the book will be celebrated and members will have the opportunity to exchange impressions of the congress and formulate ideas for the organization of the 7th World Congress in May 2014 in Maastricht.

Preparing for the organization of the 7th World Congress of WIP in 2014

The 7th World Congress of WIP will be organized in Maastricht. The congress centre MEC has gained an excellent reputation also in the health care sector. It is situated within walking distance of the Maastricht University Medical Centre and the University department of anatomy and embryology. The section officers outlined the action plan from now until D-day. Several WIP Benelux members already offered their support for making this meeting a success.

From the section committee

Frank Wille, treasurer, has been appointed as Director of the Department of Pain Medicine of the Amsterdam Medical Cen-tre. Kris Vissers, secretary, continues his activities in the educational board of WIP and EFIC. He is also involved in the program on “Societal Impact of Pain” (SIP) whereby pain is brought under the attention of government and policy makers. The SIP group outlined a roadmap that will be implemented in the European countries to increase the awareness of pain as a

disease in its own right. Maarten van Kleef, MD, PhD, FIPP serves as vice-chairman of the WIP Benelux Section and co-chairman of the FIPP Board of Examination.

Participation in the workgroup “complications”

WIP Benelux section will be represented by Frank Wille in the workgroup on the registration of complications of interven-tional pain management procedures.

Summary of Section’s efforts in 2011

Project Nablus

A team consisting of various specialists traveled on a mission to Nablus, a city in the Palestinian Territories. The trip was accompanied by a German film grew and the report was broadcasted in the German TV ARD in September 2011.

By deep and emotional experiences and the medical crash course will be won for humanitarian missions, have been sensitized and are committed to continue to pursue similar projects in future

The goal: Building up the first pain center in the Palestinian Territories.

Prospective outlook of 2012 initiatives

Symposium and Cadaver Workshop

For end of the year 2012 we plan for the Section Central and Eastern Europe a symposium in Munich including a Cadaver workshop for 100-200 participants.

Pain physicians from 34 countries gathered in Budapest to participate in The 16th Annual Advanced Pain Conference and Practical Workshop. Organized by WIP Founder and Past President, Dr. Gabor Racz, Dr. James Heavner and the Local Arrangement Committee of Drs. Edit Racz, Agnes Stogicza (Section Secretary), and Lorand Eross (Section Treasurer) participants were stimulated by the advanced curriculum of practical and clinically-relevant top-ics pertinent to today’s pain specialists.

Maintaining WIP’s customary formula of delivering new and effective pain treatment modali-ties under the guidance of world-renowned scholars, participants spent their mornings at the Kempinski Hotel Corvinus engaging in topical lectures and afternoons at Semmelweis Universi-ty Medical School in cadaver workshop sessions. This formula of didactics promoted uniform standards of excellence in pain practice while also allowing for focused instruction on proce-dures most relevant to each participant’s practice. Participants benefitted from this tradition of excellence that is synonymous with WIP’s compre-hensive curriculum that optimizes both knowledge and performance in interventional techniques involving head and neck, thorax, lumbar, and pelvic anatomy.

“The conference and cadaver workshops which I attended in Budapest were among the best I ever at-

tended,” said Dr. Samir Desai of India.

Dr. Arman Taheri of Iran agreed, “It was (a) help-ful conference and workshop, and it was great to meet pain authorities from all of the world…(and) to upgrade my knowledge in pain practice.”

The impact of this annual conference on the global nexus of pain physicians is evident in the support demonstrated toward organized social activities as well as to the sponsors and exhibitors. Exhibitors enjoyed prime space adjacent to the lecture halls that optimized the exchange between in-dustry representatives and conference participants.

The Mediterranean Section experienced growth in memberships with a surge in FIPP certifications in the country of Italy.

Preliminary plans are underway to organize another international symposium in Greece, as reported by Prof. Dr. Athina Vadalouca, Section Chair for the Mediterranean Section. The year 2013 is projected.

The Iberian Section welcomed a new FIPP in 2011. Efforts are underway to organize the first WIP-sponsored symposium in the Iberian Section. This symposium is anticipated for 2012 and will be directed by Dr. María Teresa Bovaira, WIP Section Chair for the Iberian Section, with organizers: Drs. David Abejón González, Carmen Pichot, Alejandro Jiménez Jericó, and Ricardo Ruiz-López.

The WIP Switzerland Section was quite active, with three of the 9 FIPPs also being members of the Swiss Society for Interventional Pain Management (SSIPM). Dr. Philippe Mavrocordatos is the President of SSIPM . The group participated in the creation of an official sub-specialty in interventional pain, recognized by the Swiss Medical Federation. They also participated in the re-engineering of the reimbursement of all interventional procedures at a national level. This was very important, as no physician would specialize in pain if interventions were not reim-bursed. This guarantees the survival of the specialty of pain medicine.

The yearly Congress was also co-organized by FIPPs and takes place in Montreux every year. This year’s meeting was organized with the Swiss Chapter of IASP and the Swiss Chapter of INS.

The Cecil Pain Center, directed by Dr. Mavrocordatos, received in 2011 the WIP Excellence in Pain Practice Award for Multi-disciplinary Pain Practice.

The UK-Éire Section of WIP is developing well with good interest. The new officers have taken up post and had a fruitful meeting. They are Dr. Patrick McGowan (Chair), Dr. Adnan Al-Kaisy (Vice-Chair), Dr. Dalvina Hanu-Cernat (Treasurer), and Dr. Lourdes Gaspar (Secretary). We wish to extend our thanks to Dr. Muhammad Ather and Dr. Charles Gauci, the previous officers who have made significant contributions and whose advice is still valuable.

The economic climate is causing change in the delivery of healthcare in the UK. A large amount of healthcare is publicly funded and as a result of this is sensitive to political decisions. Pain Medicine is sensitive to this. Pain Medicine has been enhanced by the creation of the Facul-ty of Pain Medicine at the Royal College of Anaesthetists a few years ago in the UK, and the Faculty of Pain Medicine of the College of Anaesthetists of Ireland.

The Section particularly promotes the value of the FIPP diploma. The Irish Faculty of Pain Medicine records the FIPP one of the entry criteria (equivalent to DAPAPM and FPMANZA) for Foundation and Ad Eundem entry to the Faculty of Pain Medicine of the College of Anaes-thetists of Ireland.

In May 2012 a UK-Éire FIPP dinner is due to take place in London and it will be opened to others who wish to attend. We hope to see further WIP-sponsored meetings in the coming year.

The current membership of the WIP UK-Éire Section is 84 with 35 FIPPs and we hope this will continue to grow.

I extend my thanks to all those who have worked so hard, including past officers of the Section, to further the aims of WIP and who have given of their value time and expertise.

The UK-Éire Section has been particularly thankful to Di-anne Willard and Mark Tolliver at the WIP Secretariat for their unfailing assistance.

Latin American Section Establishes Advanced Training Program in Brazil

     For practicing physicians, taking a year (or two) off to pursue a new career in pain medi-cine is a huge barrier to entry. And yet, a week-end course does not allow for adequate training. Dr. Fabricio Dias Assis (FIPP) and his col-leagues at Singular—Centro de Controle Da Dor (an EPP Award Pain Center) in Campinas,

Brazil, have developed a unique training program, one weekend per month for seven months, to train anesthesiologists, orthopedic and neuro-surgeons, as well as neurolo-gists and primary care physicians, in the diagnosis and treatment of common pain problems. Students receive didactic training from international WIP members and national experts as well as hands-on fluoroscopic training on a spine phantom. In addition, they are educated on the evidence-based data regarding each procedure, observe live procedures, are trained on physical exam, and receive more than 300 seminal articles on pain topics. Fifteen students from all over Brazil started in the course in September 2011, and another course is already scheduled to begin in April. With the support of WIP and its members, the quality of pain care in Brazil will increase exponentially.

The USA Section served as a co-sponsor of the 28th Annual Pain Symposium held in Lubbock, Texas, USA in June 2011.

Section Chair, Dr. Andrea Trescot, served as an instructor and program co-director of the new advanced interventional pain training program in Brazil, organized by the Latin American Section. Dr. Trescot is in discussions with FIPPs in other sections to develop similar training programs in other countries.

Dr. Trescot also served on the Excellence in Pain Practice (EPP) Award inspectors’ panel.

Pain Practice, the official journal of WIP, publishes international multidisciplinary articles on pain and analgesia that provide its readership with up-to-date research, evaluation methods, and techniques for pain management. Special sections including the Consult-ant’s Corner, Images in Pain Practice, Tutorials, and the Evidence-Based Medicine series combine to give pain

researchers, pain clinicians, and pain fellows-in-training a systematic approach to continuing education in pain medicine.

In 2011, total circulation including consortia, increased by 17% from 2009. In 2011, institutional subscriptions remained stable; member subscriptions grew steadily as renewals were processed throughout the year and overall membership subscriptions increased 5% from 2010. Non-member personal subscriptions increased slightly in 2011.

In 2011, Pain Practice articles were downloaded 94,341 times on Wiley Online Library, excluding third-party (EBSCO and Ingenta). This compares with 69,691 in 2010; an increase of 35%.

Readership for Pain Practice on Wiley Online Library rose from 69,691 (2010) to 94,341 in 2011, an increase of approximately 35%. This increase is slightly attributed to improvements on Google indexing and Wiley Online Library search functionality.

Regional analysis of readership (full text accesses): 37% USA; 29% Other; 8% UK; 5% Australia; 4% Canada, India; 3% Italy, Netherlands, Spain; and 2% Germany, South Korea.

e The idea of developing a competency measure for interventional pain practice was inspired by WIP Founder P. Prithvi Raj when he witnessed the first cadaver courses held in Lubbock, Texas, USA and Budapest, Hungary during the early years of WIP in early to mid 1990. The experts who spoke at these conferences embraced different standards and techniques for interventional procedures. One could not say that one expert was not as good as another, yet the concepts and techniques promoted by each physician varied widely in quality and standard.

These early workshops witnessed by Dr. Raj influenced his belief that better and closer contact between the experts would result in standardization of techniques in pain practice. He presented his thoughts and concept to the other WIP founders. They were hesitant to proceed, based on the fact that it was believed people would not come together, it would be too difficult to organize, the concept might not be accepted by major institutions, and it was considered to be very expensive to initiate. Another consideration was that if we conceived of a competency examina-tion, would there be barriers imposed by the official examination boards in each country or respective governments?

Dr. Raj consulted specialists in creating and conducting examinations to obtain their practical and legal advice. Knapp and Associates Inc. was chosen for this advice, because Dr. Raj had worked with them when he served on the Board of the American Board of Pain Medicine (ABPM). The agency had done good work in creating the ABPM examination and its results were well received in the USA. The agency advised that a body such as WIP could create a competency examination and provide a certificate for passing the examination. It was determined that the examination had to cover the area of common practice in the field of pain medicine. Furthermore, eligibility criteria had to be uni-form, nondiscriminatory, and transparent. The Information Bulletin was prepared for eligible candidates to describe how the examination was to be conducted, scored, and results communicated and in what time period for each candidate.

The next question raised was how the interventional examination would be formatted. Was it going to be just theoretical, just practical, or a combination of both? It was Dr. Raj’s belief that the fellowship examination he took in England in Anesthesiology (FFARCS) was an extremely thorough and fair assessment. Based on that examination, the proposed interventional examination for WIP was decided to be formatted in three parts: theoretical, identification, and procedural. That is how the examination was conceived and initiated. The examination cost WIP $70,000 to initi-ate, including consultation fees. Initially, in the first two or three beta examination periods physicians of excellence in interventional pain practice were invited to participate. WIP began with the first group of nine and later with another 13 physicians who administered the beta examination and created an examiner’s pool. WIP had a sufficient number of examiners by 2001 and that is when the first official FIPP examination was conducted. Since then, modifications have been implemented to the examination based on necessity and growth of the examinees. The initial certificate was given as a Diplomat of Interventional Pain Practice and soon was changed to Fellow of Interventional Pain Practice (FIPP).

As of 2011, 684 physicians from 38 countries have earned FIPP certification.

As the Fellow in Interventional Pain Practice exam celebrates its 10th year anniversary, I look back and marvel at the successes and accomplishments. In the beginning, the exam was the lofty ideal of a few for-ward-thinking practitioners and thought leaders. Precedents for such an ambitious exam were few. As the field of interventional pain medicine continued its rapid expansion, existing written exams proved insuffi-cient in testing competency. The exam was born from these inadequacies. Effort and sacrifice have been made by examiners and members of the examination board as the process has developed and expanded. More importantly, I thank the examinees who put significant amounts of time, money, and trust that the FIPP exam was meritorious and of value to their practice. They studied hard and many were rewarded with an FIPP diploma. While there remains much work to be done, the first decade of the FIPP has enjoyed remarkable success. The international community of pain practitioners has clearly acknowledged the value of an FIPP diploma. As one of the stewards of the World Institute of Pain, I am extremely proud of the FIPP exam and look forward to helping it expand to new heights. (FIPP) is the only certification on pain intervention so that when you have the FIPP qualification, you can say you reach certain standards in pain management. Although there are other qualifications in pain in different countries...they do not focus on the pain interventions. However, when the pain interventions are not properly done, they cause more harm and suffering to the patient. The Excellence in Pain Practice (EPP) Award program was introduced by WIP in

2010 to promote the highest standards of pain practice. By recognizing pain centers that demonstrate clinical and scientific excellence, as well as dedication to teaching and mentoring physicians-in-training and young physicians in newly established practices. WIP aims to bolster the exchange of clinical experience and scientific exploration in interventional pain practice. Through EPP, WIP encourages networking between FIPPs and associate pain physicians in FIPP-member pain centers to stimulate medical research and continuous learning in the most up-to-date technical procedures. In 2011, WIP presented EPP Awards to 14 pain centers. Istanbul Pain Center was founded in 2000 by Prof. Dr. Serdar Erdine, MD, FIPP. From using scientific and technological possibili-ties, the more positive aspects of medicine provide new therapeutic opportunities. Imaging techniques, based on laboratory tests, are utilized to avert unnecessary treatments and surgeries. Imaging techniques are important, as are the evaluation of laboratory findings, and other tests and clinical findings with the clinical need to adapt methods of investigation. That is exactly the evaluation of the patient to be examined and the methods to be utilized. Because there is a feature that separates the branches of medical science, a simple positive science of medicine itself has emerged as an art. Istanbul Pain Center continues to work in the light of these princi-ples. Patients admitted to our center are met by the staff. Then, the patients are evaluated by a team of professors and other faculty members under the direction of Prof. Dr. Erdine. After the patient has been diagnosed, the methods of treatment of the disease are described in detail to both patients and relatives.

Prof. Dr. Serdar Erdine graduated from Cerrahpasa Medical Faculty of Istanbul University in 1978. In 1982 he completed residency in the Department of Anesthesiology and Reanimation of Medical Faculty of Istanbul, Istanbul University. In 1986 he became Associate Professor in Anesthesiology, and in 1991 became Professor of Anesthesiology and Algology. The University of Washington Division of Pain Medicine provides inpatient and outpatient services in 9 programs at 5 hospitals: UW Medical Center; Harborview Medical Center; Seattle Children's Hospital; VA Puget Sound Healthcare System; and the Seattle Cancer Care Alliance. In each of these programs, multispecialty medical evaluation is provided with an emphasis on diagnosis, goal-setting, effective treatment and follow-up. UW is involved in pain education of primary care providers locally, statewide, and nationally. For pain therapy means the diagnostic approach, therapeutic and scientific to pain, which is a disease and should be treated as such. Chronic pain makes incapacitated person who suffers both from a physical point of view that emotional and therapy in this area aims at an improvement of the quality of life.

Dr. Yong Chul Kim is director of the Pain Management Center, Seoul National University Hospital. He performs various interventional pain procedures, such as kyphoplasty, spinal cord stimulator implantation, and intrathecal pump implantation. Dr. Kim and his colleagues are also studying on the efficacy and neurotoxicity of spinoaxially-administered drugs as preclinical trials.

The pain center consists of 3 professors, 2 clinical assistant professor, 4 fellow-ships, 3 residents, 6 nurses, 2 aid-nurses, 2 clinical and research assistants, and 2 specialized radiographers. Only for pain procedures, two theatres are operated and equipped with C-arms and radiofrequency machines. Also, there are 3USGs, 2 RF machines, an ESWT machine, an infrared thermography and etc. to manage patients in outpatient clinic. Over 110 patients visit the pain center a day, and 60-70 proce-dures are performed under fluoroscopy-guided or USG-guided. They are able to perform almost of present minimal invasive procedures. The first case of many procedures in Korea or in Asia was performed here, including radiofrequency, spinal cord stimulation, intrathecal drug administration system and etc. They have a lot of experiences in spine intervention such as vertebroplasty, kyphoplasty, sacroplasty, IDET, nucleoplasty, dekompressor and epidural adhesiolysis using a Racz catheter or an epiduroscopy. They use dry needling and CAM, indeed. For education, the pain center offers five fellowships; 12-month positions each year (2 starting in our hospital for 6 months, another 3 more in branch hospital). The fellowship program is devoted to the evaluation and management of complex pain problems. The program trains candidates to be qualified in pain management by the Korean Board of Pain Medicine. Fellows must have completed an anesthesiology residency at an accredited residency training program. Fellows completing a residency in a specialty other than anes-thesiology will be considered on an individual basis. Fellowship training is available in com-mon and advanced interventional techniques. Cancer pain management based on the inpa-tient consult service is also encompassed in the program. Fellows are also required to partic-ipate in teaching and research activities. We also train 3 residents. We review text books and journals every Monday, Tuesday, and Friday from 7 to 7:45 am. And on Wednesday from 7 to 8:45 am. In particular, four hospitals participating conference is held on Thursday morn-ing in semester. Until now, many Korean and foreign doctors have visited our pain center to learn the procedures. We are willing to share our experiences with anyone recommended by WIP, and provide consultation.

Patients who suffer from pain for weeks, months or even years require special attention. That is why hospitals often have a special department dedicated to the therapy of chronic pain. The field of pain therapy comprises doctors from a variety of branches of medicine – rheumatology, anesthetics, general surgery and physical medicine working togeth-er in an interdisciplinary regime. They select methods of pain relief tailored to the patient that either completely eliminate the pain or at least alleviate it. Various drugs and surgical techniques can be used to achieve this. These days, use is often made of the latest methods, such as transcutaneous electrical nerve stimulation (TENS) – where the nerves in the spinal cord are stimulated using an electrical cur-rent – or implanting pumps that drip-feed pain-relieving drugs into the body. Pain therapy also makes use of techniques from the Far East, such as acupuncture and traditional Chinese medicine. Hypnotherapy (hypnosis used for therapeutic purposes) can also help to alleviate chronic pain. As pain can also have a pronounced impact on the spiritual well-being of the patient, psychiatrists and psychotherapists are often involved in pain therapy.

He is also a certified Fellow in Interventional Pain Practice. A highly active author, editor, and pain management specialist, Dr. Patel has managed and practiced at medical centers throughout Illinois, including Loyola University Medical Center near Chicago where he served as Program Director for the Pain Management Fellowship program and as an associate professor of the pain management section in the Department of Anesthesiology. The World Institute of Pain Foundation was founded in 2010 to engage global advo-cates of world population health and function in the field of pain medicine. The WIP Foundation is dedicated to the creation of projects that will enable scholarships for train-ing and certification of eligible pain physicians, protocols for global clinical and scien-tific research in pain medicine, and greater collaboration among pain physicians world-wide. By partnering with the WIP Foundation, the public can help make a difference in the quality of life for pain patients throughout the world.

Through the generosity of those who believe in our mission, the WIP Foundation aims to train more physicians in more countries by eliminating geographic and economic bar-riers that impede access to and opportunity for advanced physician education and im-proved patient care outcomes.

The Samuel J. Hassenbusch Prize

Awarded annually to the highest-scoring candidate for the FIPP

The Samuel J. Hassenbusch Prize, established in 2008, rec-ognizes the candidate who earned the highest composite score on the examination for Fellow of Interventional Pain Practice (FIPP).

In 2011, the Hassenbusch Prize was awarded to Dr. Jan W. Kallewaard, MD, FIPP of The Netherlands. Dr. Kallewaard is affiliated with the Rijnstate Hospital in Arnhem, the Netherlands. He has served as chairman of the Dutch Society of Anesthesiology.
