travel BenefitsTravel Emergency Medical
Provides emergency medical coverage anywhere in the world and isautomatically included whether or not you purchase travel using youreligible Capital One card. As the primary Cardholder, you, your spouseand your dependent children, when travelling with either parent, arecovered for trips up to 22 days, if under 65 years of age, and for tripsup to 8 days, if 65 years of age or older. The date of departure iscounted as the rst day. This benet is subject to an exclusion forpre-existing medical conditions.
Trip Cancellation
If an illness, injury or other covered cause prevents you from startingyour trip, youll be covered for up to $1,000 of eligible cancellationexpenses per insured person, to a maximum of $5,000 for all insuredpersons on the same trip. Trip Cancellation coverage is available to you,your spouse and your dependent children, when travelling with eitherparent, and up to two companions travelling with you and/or your
spouse when the full cost of your trip expenses are charged to yourCapital One card. Eligible expenses include the cost of unused travelarrangements that were charged to your Capital One card.
Trip Interruption
If an illness, injury or other covered cause prevents you from continuingyour trip, youll be covered for up to $5,000 of eligible trip interruptionexpenses, per insured person, to a maximum of $25,000 for all insuredpersons on the same trip. Trip Interruption coverage is available to you,your spouse and your dependent children, when travelling with either
parent, and up to two travelling companions when the full cost of yourtrip expenses are charged to your Capital One card.
Flight Delay
You may be reimbursed for eligible reasonable living expenses incurredduring the period of a delayed ight or missed connection which lastsfour hours or more to a limit of $250 per day and $1,000 per trip forall insured travellers. Simply charge the full cost of your tickets to yourCapital One card.
Baggage DelayIf your checked baggage is delayed for more than four hours at youraway-from-home destination, you, your spouse and your dependentchildren may be eligible for reimbursement of up to $100 per day forup to three days to purchase essential items. Simply charge the fullcost of your tickets to your Capital One card.

definitions
Accident means a sudden,unexpected and unforeseeable
cause of injury from an
external source.
Accidental Bodily Injury meansbodily injury caused directly by an
Accident occurring while the
insurance evidenced by this
Certicate of Insurance is in force,which results, within 365 days
after the date of the Accident,
directly in any of the losses to
which the insurance applies, and
is sudden, unforeseen, unexpected
and independent of any disease,
bodily inrmity, bodily malfunction
or any other cause.
Account means the PrimaryCardholders Capital One World
MasterCard Account or an
upgraded Capital One Platinum
MasterCard Account designated
by the Policyholder and which
is in Good Standing with
the Policyholder.
Administrator means the service
provider arranged by the Insurerto provide claims payment and
administrative services under
the Policy.
Capital One MasterCardmeans a Capital One World
MasterCard or an upgraded
Capital One Platinum MasterCard
designated by the Policyholder.
Car Sharing means a car rentalclub that provides its members with
24-hour access to a eet of cars
parked in a convenient location.
Cardholder means the PrimaryCardholder and any supplemental
Cardholder, who is a natural Person,
resident in Canada and who is also
issued a Capital One MasterCard, as
dened herein, and whose name is
embossed on the card.
Cardholder may also be referred
to herein using You and Your.
Common Carrier means anyland, air or water conveyance
operated, which is licensed to carry
passengers without discrimination
and for hire, excluding courtesy
transportation provided without
a specic charge.
Dependent Child means Yourunmarried natural, adopted or
step-children who are dependenton You for maintenance and
support and who are either under
21 years of age, or under 25 years
of age and in full-time attendance
at a recognized institution of
higher learning in Canada.
Dollars and $ meansCanadian dollars.
Eligible Expensesmean chargesfor any of the following travel
arrangements which have been
booked or reserved prior to
Trip departure and for which
all costs have been charged to
Your Account:
(i) cost of transportation
by a Common Carrier;(ii) cost of hotel or similar
accommodations; and
(iii) cost of a package tour which
has been sold as a unit and
includes at least two of
the following:

Transportation by
a Common Carrier;
Car rental;
Hotel or similar
accommodation;
Meals;
Tickets or passes for
sporting events or other
entertainment, exhibition
or comparable event; or
Lessons or the servicesof a guide.
Emergency Medical Treatmentrefers to treatment necessary
for the immediate relief of a
Medical Emergency.
Essential Items mean theminimum essential clothing and
toiletries, the purchase of whichis rendered absolutely necessary
and indispensable due to the
delay of an Insured Persons
checked luggage.
GHIP means the governmenthealth insurance plan of an Insured
Persons province or territory of
residence in Canada.
Good Standing means, withrespect to an Account, that the
primary Cardholder has not advised
the Policyholder in writing to close
it, it is in compliance with all terms
of the Cardholder Agreement and
the Policyholder has not suspended
or revoked credit privileges or
otherwise closed the Account.
Hospital means an institutionwhich is licensed to provide,
on an inpatient basis, medical
care and treatment of sick and
injured persons through medical,
diagnostic and major surgical
facilities, under the supervision
of a staff of Physicians and with
24-hour-a-day service. Hospital
does not include any institution
or part of an institution which is
licensed or used principally as a
clinic, a continued care or extended
care facility, a convalescent home,
a rest home, a nursing home or
home for the aged, a health spa
or a treatment centre for drug
addiction or alcoholism.
Hospitalization means a stayof at least 48 hours in a Hospital
for Emergency Medical Treatment,
which cannot be postponed.
Immediate Family Membermeans an Insured Persons Spouse,
daughter or son, whether natural,
adopted or stepchild, grandchild,mother, father, stepparent, sister,
stepsister, brother, stepbrother,
parent-in-law, daughter-in-law,
son-in-law, sister-in-law and
brother-in-law.
Insured Person means aCardholder and, where specied,
his or her Spouse and each
Dependent Child and certain othereligible persons as outlined under
the applicable benet.
Key Employeemeans anemployee whose continued
presence at the Insured Persons
place of business is critical to the
ongoing affairs of the Insured
Persons business during Your Trip.
Legal Business Partnermeansa person who participates with
the Insured Person in the daily
management of a shared business
and who shares the nancial risk
of the operation.

Medical Condition means any
illness, injury or symptom, whether
diagnosed or not.
Medical Emergency means any
unforeseen illness or Accidental
Bodily Injury which occurs during
a Trip and requires immediate
medical care or treatment from
a Physician. A Medical Emergency
ends when the illness or AccidentalBodily Injury has been treated such
that the Insured Persons condition
has stabilized. Treatment provided,
when medical evidence indicates
that an Insured Person could delay
treatment or return to Canada
for such treatment, is not
considered a Medical Emergency
and is not covered.
Mysterious Disappearance
means an article of personal
property cannot be located and the
circumstances of its disappearance
cannot be explained or do not
lend themselves to a reasonable
inference that a theft occurred.
Occupying means in or upon
or entering into or alighting from.Original Manufacturers
Warranty means an express
written warranty valid in Canada
and issued by the original
manufacturer of personal property,
excluding any extended warranty
offered by the manufacturer or
any third party.
Physician means a Physicianor surgeon who is registered or
licensed to practice medicine in
the jurisdiction where the medical
treatment or service is provided,
and who is not related by blood
or marriage to the Insured Person
to whom the service is rendered.
Pre-existing Condition means
any Medical Condition for which
symptoms appeared or for which
an Insured Person, or an Insured
Persons Immediate Family Member
or Travelling Companion, sought
the attention of a Physician,
had investigated, diagnosed,
treated, had treatment or further
investigation recommended or for
which medication was prescribedor altered in the 180 days prior
to (for Insured Persons age 74 and
under) or 365 days prior to (for
Insured Persons age 75 and over):
(i) the date the Trip was booked, for
Trip Cancellation Benets; and
(ii) the Trip departure date, for
Trip Interruption and Travel
Emergency Medical Insurance.
For Travel Emergency Medical
Insurance, a Pre-existing Condition
does not include a Medical
Condition which is controlled by
the consistent use of medications
prescribed by a Physician, provided
that, during the 180-day period
or 365-day period, as applicable,
before the Insured Personsdeparture, there has been no
other treatment or investigation
recommended and there has been
no change in medication. A new
medication or increase/decrease
in dosage constitutes a change.
Primary Cardholdermeans
the principal applicant for an
Account who is a natural person,resident in Canada and is issued
a Capital One MasterCard, as
dened herein, by the Policyholder.

Reasonable and Customary
Charges means charges which
do not exceed the general level
of charges made by other providers
of similar standing in the locality
or geographical area where
the charges are incurred, when
furnishing comparable treatment,
services or supplies for a similar
Medical Emergency.
Rescheduling Expensesmeans
additional charges associated
with Eligible Expenses, including
administrative and change fees,
which result from rescheduling
a Trip prior to departure, and
which have been charged to
Your Account.
Spouse means the Cardholderslegal husband or wife, or the
person with whom the Cardholder
has lived and publicly represented
as his or her Spouse for at least
one continuous year.
Ticket means evidence of fare
paid for travel on a Common
Carrier, which has been charged
to the Account.Travelling Companion means a
person booked to travel with You
and/or Your Spouse on a Trip, and
who has prepaid accommodations
and/or transportation arrangements
for the same Trip.
Trip means a scheduled period
of time during which an Insured
Person is away from his or herprovince or territory of residence
in Canada. For Travel Emergency
Medical Insurance, an eligible
Trip is further limited in duration,
as specied under Travel
Emergency Medical Insurance.
Travel Emergency Medical
Insurance
Trips up to 22 days for
Insured Persons under age 65.
Trips up to 8 days for Insured
Persons age 65 or older.
This coverage is underwritten by
American Bankers Life Assurance
Company of Florida.
For this benet, Insured Person
means the Primary Cardholder,
his or her Spouse and his or her
Dependent Child(ren) who are
booked to travel on a Trip with
the Primary Cardholder and/or his
or her Spouse. All Insured Persons
must be permanent residents
of Canada and insured by their
provincial or territorial GHIP.
Coverage Period
For Insured Persons under age
65 (on the Trip departure date)
For the Travel Emergency Medical
benet, only the rst 22
consecutive days of a Trip, as
determined by the originally
scheduled departure and returndates, will be covered. There is
no coverage for that portion
of a Trip which extends beyond
the rst 22 consecutive days.
In the event of a claim, proof
of scheduled Trip duration will
be required.
Coverage begins at 12:01 a.m.
on the date the Insured Personleaves his or her province or
territory of residence on a Trip.
Coverage will terminate on the
earliest of the following:

(ii) the date the Account iscancelled, closed or is nolonger in Good Standing;

(iv) the date the Insured Personceases to be eligible forcoverage. (For DependentChildren, see the Denitionssection for age limits.)
For Insured Persons age 65
or older (on the Trip
departure date)
For the Travel Emergency Medicalbenet, only the rst 8 consecutivedays of a Trip, as determined bythe originally scheduled departureand return dates, will be covered.There is no coverage for that
portion of a Trip which extendsbeyond the rst 8 consecutive
days. In the event of a claim,proof of scheduled Trip durationwill be required.
Coverage begins at 12:01 a.m.on the date the Insured Personleaves his or her province orterritory of residence on a Trip.
Coverage will terminate on theearliest of the following:
(i) the date the Insured Personreturns to his or her provinceor territory of residencein Canada;
(ii) the date the Account iscancelled, closed or is nolonger in Good Standing;
(iii) the date the Insured Personhas been absent for more than8 consecutive days (includingthe day of departure andday of return) from hisor her province or territoryof residence in Canada; or
(iv) the date the InsuredPerson ceases to be eligiblefor coverage.
Automatic Extension
of Coverage
Coverage will be automaticallyextended beyond the 22-day limitfor Insured Persons under age65 and 8-day limit for InsuredPersons age 65 or older, for upto 3 days following the end ofa Medical Emergency.
In addition, Travel EmergencyMedical Insurance coverage willbe automatically extended beyondthe 22-day limit for InsuredPersons under age 65 and 8-daylimit for Insured Persons age 65and older, for up to 3 days if anInsured Persons return to his or herprovince or territory of residencein Canada is delayed solely as theresult of:
(i) the delayed departure of aCommon Carrier on which heor she is booked; or
(ii) a delayed return as a result ofan accident or the mechanicalbreakdown of an InsuredPersons personal vehicle.

Benets
In the event of a MedicalEmergency, the Reasonable andCustomary Charges for EmergencyMedical Treatment will be paidby the Insurer, less any amountpayable by or reimbursable undera GHIP, any group or individualhealth plans, insurance policiesor reimbursement programs.
Benets are limited to $2,000,000for each Insured Person, subjectto the Limitations and Exclusions.The following expenses are eligiblefor reimbursement:
(i) Hospital Accommodationand Medical Expensesincluding room and board
up to semi-private or theequivalent, treatment in anintensive or coronary care unit,medical supplies, use of anoperating room, anaesthesia,surgical dressings. Excludescosts of a private room or suite,unless one is medically required.
(ii) Physician Charges forEmergency Medical Treatment.
(iii) Private Duty Nursing coversup to $5,000 for servicesperformed by a registerednurse (not related to theInsured Person by blood ormarriage) when medicallynecessary and prescribed bya Physician. This includesmedically necessary
nursing supplies.(iv) Diagnostic Services,
including laboratory tests,x-rays when prescribedby a Physician. NOTE:magnetic resonance imaging(MRI), computerized axial
tomography (CAT scans),sonograms and ultrasoundmust be authorized in advanceby the Administrator.
(v) Ambulance Service to thenearest Hospital equippedto provide the requiredEmergency Medical Treatment.
(vi) Emergency Air Transportto the nearest Hospital, or
repatriation to a Hospital in theInsured Persons province orterritory of residence in Canada(when approved and arrangedby the Administrator) becausethe Insured Persons conditionprecludes the use of othermeans of transportation.
(vii) Prescription Drugs required
as a part of Emergency MedicalTreatment, excluding any drugsor medications which arecommonly available withoutprescription, or which are notlegally registered and approvedin Canada or the United States.
(viii)Accidental Dental care toa maximum of $2,000 perInsured Person for Emergency
Medical Treatment of naturalor permanently installed teethduring a Trip as the result of anaccidental injury to the mouth.Treatment for emergency reliefof dental pain is covered to amaximum of $150.
(ix) Medical Appliancesincluding slings, braces, splints,
and local rental of crutches,walkers and wheelchairs.There is no benet for thepurchase of either walkersor wheelchairs.
(x) Return Airfare to cover anyadditional cost incurred for a

one-way economy airfare in aMedical Emergency (less anyrefunds due on original tickets)and, if required, the charge fortransportation of a stretcherand attending medicalpersonnel to return the InsuredPerson to his or her provinceor territory of residence inCanada if further medical
treatment is warranted andwhen approved and arrangedby the Administrator.
(xi) Transportation to Bedsidefrom Canada for one of theInsured Persons ImmediateFamily Members when theInsured Person is hospitalizedand expected to remain so for
3 days or more. This benetmust be pre-approved by theAdministrator. This benetincludes one round-tripeconomy airfare, food andaccommodation expenses of$100 per day, to a maximumof $1,500 and no chargefor travel medical insurancecoverage for the person required
at bedside for the duration ofthe Medical Emergency.
(xii) Vehicle Return is coveredto a maximum of $1,000 toreturn an Insured Personsvehicle to his or her place ofresidence in Canada, or in thecase of a rented vehicle, tothe nearest appropriate rental
location, when the InsuredPerson is unable to returnthe vehicle as a result of aMedical Emergency and whenapproved and arranged bythe Administrator.
(xiii)Return of Deceased whendeath results from a Medical
Emergency, to a maximumof $5,000 for the cost ofpreparation (includingcremation) and transport ofthe Insured Person (excludingthe cost of a burial cofn orurn) to his or her provinceor territory of residencein Canada.
Limitations and ExclusionsFurther, no benets are payablefor any expenses incurred directlyor indirectly as a result of:
(i) any Pre-existing Conditionas dened;
(ii) any Medical Emergency thatoccurs other than during acovered Trip;
(iii) any experimental, electiveor non-emergency surgery,treatment, or medicationincluding ongoing care ofa chronic condition;
(iv) any Medical Emergency thatoccurs during a Trip wherethe primary purpose of theTrip was to work outsideof Canada;
(v) any portion of benets thatrequire prior authorizationand arrangement by theAdministrator if such benetswere not pre-authorized andarranged by the Administrator;
(vi) pregnancy, childbirth and/orrelated complications occurringwithin 9 weeks of the expected
delivery date;(vii) neo-natal care;(viii) participation in a criminal
offence;(ix) intentionally inicted injuries,
suicide or any attempt thereat;(x) expenses incurred directly
or indirectly as a result of the

Insured Persons abuse ofmedications, drugs, alcoholor other toxic substances;
(xi) acts of terrorism, insurrectionor war whether declaredor undeclared;
(xii) voluntary participation ina riot or civil commotion;
(xiii) mental or emotional disorders;(xiv) treatments that are not
prescribed by a Physician; or(xv) participation in professional
sports or any organized racingor speed contests, recreationalscuba diving (unless theInsured Person holds a basicscuba designation from acertied school or licensingbody), mountaineering,
bungee jumping, parachuting,hang-gliding, skydiving, orother dangerous sportsor activities.
Failure to notify the Administratorimmediately following a MedicalEmergency, or as soon as possibleunder the circumstances, will limitthe benets payable under this
Certicate as follows:(i) no benets will be payable
for surgery or invasiveprocedures (such as cardiaccatheterization) withoutprior approval by theAdministrator, except inextreme circumstances wherea request for prior approval
would delay surgery neededin a life-threatening MedicalEmergency; and
(ii) eligible expenses for whichbenets would otherwise havebeen provided will be limitedto 80% of the total, to amaximum of $30,000.
In consultation with the attendingPhysician, the Administratorreserves the right to transfer theInsured Person to an appropriateNetwork facility or to his or herprovince or territory of residencein Canada for Emergency MedicalTreatment. Refusal to comply willabsolve the Insurer of any liabilityfor expenses incurred after the
proposed transfer date.
Once a Medical Emergencyends, no further benets arepayable in respect of that MedicalEmergency or for any recurrence ofthe condition which causedthe Medical Emergency.
Medical Emergency Procedures
When a Medical Emergencyoccurs, You must contact the
Administrator without delay.Call 1-866-224-0178 fromCanada and the United States or416-642-2303 locally or collectfrom elsewhere in the world.
If calling from somewhere in theworld where a collect call is not
possible, call direct and You willbe reimbursed.
The Administrator will conrmcoverage, provide directions tothe Network facility or the nearestappropriate medical facility, providethe necessary authorization ofpayment of eligible expenses andmanage the Medical Emergency.
The Administrator will make everyeffort to pay or authorize paymentof eligible expenses to Hospitals,Physicians and other medicalproviders directly. If direct paymentor payment authorization is notpossible, an Insured Person may be

The Insurer will reimburse theCardholder for all Insured Personstravelling on the same Trip ifthe conrmed scheduled ightdeparture from any airport isdelayed by 4 hours or more, for
necessary and reasonable expensesincurred with respect to hotelaccommodations, restaurantmeals, refreshments, EssentialItems and sundry items (such asa magazine, paperback book andother such small items) within48 hours of the delay or deniedboarding, to a maximum of $250per day for all Insured Persons on
the same Trip, and a maximum of$1,000 total per Trip, provided that:
(i) the full cost of the delayedight was charged tothe Account;

For the purposes of the Car Rental
Collision/Loss Damage Waiver
benet, Insured Person means
a Cardholder and any other person
who holds a valid drivers licenseand has the Cardholders express
permission to operate the rental
vehicle. This includes drivers not
listed on Your rental contract,
provided they would otherwise
qualify under the rental contract
and are permitted to drive the
rental vehicle under the laws of
the jurisdiction in which the rentalvehicle shall be used.
Eligibility
You are eligible for Car Rental
Collision/Loss Damage Waiver
coverage when You rent most
private passenger vehicles on a
daily or weekly basis for a period
NOT to exceed 48 consecutive
days, provided that:
(i) You initiate the rental
transaction by booking or
reserving the vehicle rental
with Your Capital One
MasterCard and by providing
Your Capital One MasterCard
as payment guarantee prior to
the time You take possession
of the vehicle;(ii) You decline the rental agencys
collision damage waiver (CDW)
or loss damage waiver (LDW),

provide any form of third
party automobile property

may resist Your declining their
CDW/LDW coverage. They may
try to encourage You to take their
coverage. If You refuse, they may
insist You provide a deposit. Before
booking a vehicle, conrm that
the rental agency will accept this
Car Rental Collision/Loss Damage
Waiver insurance without requiring
a deposit. If they wont, nd one
that will, and try to get written

Coverage Period
Insurance coverage begins as soonas the Cardholder or other personauthorized to operate the rentalvehicle takes control of the vehicle,
and ends at the earliest of:(i) the time the rental agency
assumes control of the rentalvehicle, whether it be at itsplace of business or elsewhere;
(ii) the date the Account iscancelled, closed or ceasesto be in Good Standing;
(iii) the date the Insured Person
ceases to be eligible forcoverage; or
(iv) the date the Policy is cancelled.
Types of Vehicles Covered
The types of rental vehicles coveredinclude cars, sport utility vehiclesand mini-vans (as dened below).
Mini-vans are covered provided they:

Types of Vehicles NOT Covered
Vehicles which belong to thefollowing categories areNOT covered:
(i) any vehicle with amanufacturers suggestedretail price, excluding taxes,over $65,000 at the timeand place of loss;
(ii) vans (except as dened above);(iii) trucks, pick-up trucks or any
vehicle that can be reconguredinto a pick-up truck;
(iv) campers or trailers orrecreational vehicles;
(v) off-road vehicles;(vi) motorcycles, mopeds
or motorbikes;(vii) expensive or exotic vehicles;
(viii) customized vehicles;(ix) antique vehicles; and(x) leased vehicles.
An antique vehicle is one which isover 20 years old or its model hasnot been manufactured for 10years or more.
Limousines that have been
stretched or altered from theoriginal factory design areexcluded. However, standardproduction models of thesevehicles that are not used aslimousines are not excluded.
Limitations and Exclusions
Car Rental Collision/Loss DamageWaiver benets do not cover any
loss caused or contributed to by:
(i) operation of the rental vehiclein violation of the law or anyterms and conditions of therental agreement/contract;
(ii) operation of the vehicle byany driver not so authorized;

(iii) operation of the vehicle byany driver not in possessionof a drivers license that isvalid in the rental jurisdiction;
(iv) operation of the vehicleon other than regularlymaintained roads;

or over the blood alcoholconcentration limit forimpaired driving under theCriminal Code of Canada orthe jurisdiction in which thevehicle rental occurred, iflower, or the driver is chargedfor impaired driving;
(vi) use of narcotic by the driver;
(vii) nuclear reaction, radiationor radioactive contamination;(viii) damage to tires unless in
conjunction with aninsured cause;
(ix) wear and tear, gradualdeterioration, mechanicalbreakdown of vehicle;
(x) any damage caused bymoving or transporting cargo;
(xi) insects or vermin, inherentvice or damage;
(xii) hostile or warlike action,insurrection, rebellion,revolution or civil war;
(xiii) seizure or destruction underquarantine or customsregulations or conscationby any government or
public authority;(xiv) transporting contrabandor illegal trade;
(xv) transportation of propertyor passengers for hire; or
(xvi) any dishonest, fraudulentor criminal act committed orattempted by the Cardholderand/or any authorized driver.
Benets do NOT include
coverage for:
(i) vehicles rented for a periodthat exceeds 48 consecutivedays, whether or not under
one or more consecutiverental agreements;
(ii) a replacement vehicle forwhich Your personalautomobile insurance, cardealer, repair shop, or otherparty is covering all or partof the rental cost;
(iii) loss or theft of personal
belongings in the vehicle,including cellular telephones,portable computers, electronicand communication devices;
(iv) expenses assumed, waivedor paid or payable by therental agency or its insurer;
(v) third party liability (injuryto anyone or anything insideor outside the vehicle); or
(vi) any amount payable byYour employer or employersinsurance coverage,if the rental car was forbusiness purposes.
If the rental period exceeds48 days, no coverage will beprovided even for the rst

TheAdministrator will send You theapplicable claim form(s). Yourfailure to provide proof of losswithin 90 days from the date ofloss or damage may result in denialof the related claim.
You must complete and sign
the claim form which must containthe time, place, cause and amountof loss, and include the following:
(i) the customer copy of Yoursales receipt and a copy of theAccount statement showingthe charge;
(ii) the original vendorssales receipt;
(iii) a copy of the OriginalManufacturers Warranty(for Extended Warrantyclaims); and

in this Certicate of Insurance.
Notice and Proof of Claim
Immediately after learning of aloss, or an occurrence which may
lead to a loss under any of these
insurance benets, notify the
Administrator. You will then be
sent a claim form.
Written notice of claim must be
given to the Administrator as soon
as reasonably possible after the
occurrence or commencementof any loss covered by the Policy,
but in all events, provided within
90 days from the date of such
loss. Written notice given by or
on behalf of the claimant or the
beneciary, with information
sufcient to identify the
Cardholder, shall be deemed
notice of claim.
The completed claim forms
together with written proof of
loss must be delivered as soon
as reasonably possible, but in all
events within one year from the
date on which the loss occurred.
Failure to provide notice or
furnish proof of claim within the
time prescribed herein does not
invalidate the claim if the notice
or proof is given or furnished as
soon as reasonably possible, and in
no event later than one year from
the date a claim arises hereunder,
if it is shown that it was not
reasonably possible to give notice
or furnish proof within the time soprescribed. If the notice or proof is

Payment of Claim

Examination and Autopsy
The Insurer at is own expense shall
have the right and opportunity
to examine any Insured Person
whose injury or illness is the basis
of a claim hereunder when and
so often as it may reasonably
require during pendency of claim
hereunder, and also the rightand opportunity to make an
autopsy in case of death where
it is not forbidden by law.
Termination of Insurance
Coverage ends on the earliest of:
(i) the date the Account is
cancelled, closed or ceases
to be in Good Standing;

coverage; and
(iii) the date the Policy terminates.
No losses incurred after the Policy
termination date will be paid,

Subrogation
Following payment of an Insured
Persons claim for loss or damage,
the Insurer shall be subrogated
to the extent of the amount of
such payment, to all of the rights
and remedies of the Insured Person
against any party in respect of
such loss or damage, and shall be
entitled, at its own expense, to
sue in the Insured Persons name.The Insured Person shall give the
Insurer all such assistance as is
reasonably required to secure its
rights and remedies, including
the execution of all documents
necessary to enable the Insurer
to bring suit in the name of the
Insured Person.
Due Diligence
The Insured Person shall use
due diligence and do all things
reasonable to avoid or diminish any
loss or damage under the Policy.
False ClaimIf You make any claim knowing
it to be false or fraudulent in
any respect, You shall no longerbe entitled to the benets of
this insurance nor to the payment
of any claim under the Policy.
Legal Action
Every action or proceeding against
an Insurer for the recovery of
insurance money payable under
the contract is absolutely barred
unless commenced within the timeset out in the Insurance Act or
other applicable legislation.
Access to Medical Care
The Insurer, the Policyholder
and the Administrator are not
responsible for the availability,
quality or results of any medical
treatment or transport, or for
the failure of an Insured Person
to obtain medical treatment.
