PURPOSE
Raise awareness and, in some cases, challenge long-standing beliefs regarding the phenomenon of Wrong Deck Landings.

• Wrong Deck Landing (WDL)
A landing made on an offshore facility which was not the intended landing location.
DEFINITIONS
• Wrong Deck Landing (WDL)
• Wrong Offshore Helideck Landing (WOHL)
An alternative descriptor for WDL.
• Wrong Airfield Landing
Onshore equivalent of WDL.
• Wrong Offshore Helideck Landing (WOHL)
Descrição alternativa para WDL.
• Wrong Airfield Landing
Equivalente onshore para WDL.
INTRODUCTION
4
WDLs have been a regular occurence throughout the period that helicopters have been used for offshore transport. On the other hand, wrong airfield landings by offshore helicopters are virtually unknown.
COMPARISON
COMPARATIVO

REASONS FOR WDL
CAUSAS DE WDL
“In all cases the primary cause is cited as Pilot Error because the pilot had misidentified the installation. It is however, important to understand why the pilot misidentified the helideck by taking a look at secondary causes”.
Offshore Technology Report 2000/067, as prepared for the UK Health & Safety Executive 2000.

HOW SHOULD WE REGARD THE RISK?
COMO DEVEMOS AGIR EM RELAÇÃO A ESTES RISCOS?
• “There’s only two types of pilots, boy – those that have and those that will”
• Nobody has been injured from a WDL, so it isn't really a serious risk
• “It is essential to maintain the frequency of these occurrences at their current level, better still, to reduce them further”
• “The problem of WDL offshore should be openly accepted by all sides of Industry as a low frequency, relatively low risk occurrence”
RISK ASSESSMENT
• “It is most unlikely that the problem of WDL can be entirely eliminated”

• you have a few seconds to verify the rig

43
MY CONCLUSION
THE WEAKEST LINK
• If the only enhancements we make are procedural or visual, eradication of WDLs will never be achieved
• An alternative for verification must be developed, if eradication is to be achieved

RECAP
FINALLY
• This is entirely a helicopter and offshore issue
RECAPITULANDO
• Esta questão é relacionada exclusivamente a helicópteros e operações offshore
• The primary cause for a WDL is Pilot Error
• A principal causa de WDL é erro do piloto
• There are many contributing factors to WDL, many that can not be eliminated (eg. Weather, high work load...)
• Some of the sources of the pilot error come from caracteristics of the human mind (target fixation and mind autopilot)
46
RECAP
FINALLY
• The frequency of WDLs have declined over time due to improvements made by operators and Oil & Gas Companies, but WDL continues to be a regular event
RECAPITULANDO
• WDL pose a significant risk and the only acceptable target for WDLs is total elimination
• The current reliance on visual verification is flawed and enhancements in visibility are unlikely to be successful
• If WDLs are to be eliminated another form of verification must be created
